What Is Achilles Tendon Pain and Why It Happens

The Achilles tendon is the largest tendon in the human body. It connects your calf muscle to your heel bone and allows you to walk, run, jump, and point your toes downward. When you move, this tendon bears significant stress—sometimes up to 1.5 times your body weight with each step. Over time, repetitive stress, sudden increases in activity, or direct injury can cause the tendon to become inflamed or damaged, resulting in pain.

Achilles tendon pain typically develops gradually rather than suddenly. People often notice a mild ache in the back of the heel or lower leg that worsens over days or weeks. The pain may feel sharp during activity and dull during rest. Some individuals describe a stiffness in the morning that improves after moving around.

Several factors increase the risk of developing Achilles tendon problems. These include rapidly increasing training intensity or duration, tight calf muscles, flat feet or high arches, age (people over 40 are more susceptible), and previous calf injuries. Certain activities like running, basketball, tennis, and dancing place more stress on the Achilles tendon than others. Wearing shoes with inadequate heel support can also contribute to pain.

Two main conditions affect the Achilles tendon. Tendinitis involves inflammation of the tendon itself, usually from overuse. Tendinopathy refers to chronic degeneration of the tendon tissue. Partial tears create sudden, severe pain and weakness, while complete ruptures are rare but require urgent medical evaluation. Understanding which condition is causing your pain matters because treatment approaches differ.

Practical Takeaway: Achilles tendon pain develops from repetitive stress or injury to the tissue connecting your calf to your heel. Recognizing early symptoms like heel ache or morning stiffness helps catch problems before they become more serious.

Common Symptoms and When to Seek Medical Attention

Achilles tendon pain presents in various ways depending on the underlying problem. The most common symptom is pain in the back of the heel or along the lower leg, often worse in the morning or when starting physical activity. This pain may decrease with continued movement as the tendon warms up. Some people experience sharp, stabbing pain during specific movements like pushing off during running or climbing stairs.

Additional symptoms that may accompany tendon pain include swelling along the back of the heel or lower leg, stiffness that improves with gentle movement, weakness in the ankle or difficulty rising onto your toes, and a creaking or popping sensation when moving the ankle. The skin over the tendon may feel warm or appear reddened if inflammation is present. Pain typically worsens with activities that stress the tendon and improves with rest, though chronic cases may hurt even during rest periods.

Certain warning signs indicate you should seek medical evaluation rather than attempting to treat the problem at home. These include sudden, severe pain that prevents normal walking, visible deformity in the heel area or lower leg, complete loss of calf strength, or the feeling that your ankle is giving way. Additionally, if pain persists for more than two weeks despite rest and self-care, professional assessment becomes important. Pain that developed following a specific injury or accident should be examined promptly.

A healthcare provider can perform physical tests to determine what is causing your pain. Common evaluation methods include assessing your ability to rise onto your toes, palpating the tendon to locate tender areas, and checking ankle range of motion. Imaging studies like ultrasound or MRI may be ordered to visualize the tendon structure and identify tears, inflammation, or degeneration. Early diagnosis prevents minor problems from becoming chronic or leading to more serious injury.

Practical Takeaway: Monitor your symptoms carefully. Mild heel pain that improves with rest may respond to self-care, but persistent pain lasting over two weeks, significant swelling, or sudden severe pain warrant professional medical evaluation to prevent complications.

Rest, Ice, and Initial Self-Care Strategies

The first step in addressing Achilles tendon pain is reducing the stress placed on the injured tissue. Rest does not mean complete immobilization—rather, it means avoiding or modifying activities that trigger pain. If running causes your pain, switching to swimming or cycling may allow you to maintain fitness while the tendon recovers. If climbing stairs hurts, taking them slowly or holding a railing reduces stress on the tendon. The goal is finding the balance between resting the injured tissue and maintaining some level of movement to prevent stiffness and loss of strength.

Applying ice to the painful area can reduce inflammation and numb pain, making it a useful tool in the first few days after an injury begins or pain flares up. Ice should be applied for 15 to 20 minutes at a time, typically three to four times daily. Wrap ice packs in a thin towel to protect your skin from direct freezing. Some people find that an ice massage—rubbing a frozen water bottle directly over the tendon—helps reduce pain and inflammation. Continue icing for the first 48 to 72 hours after pain begins or worsens, then reassess whether additional icing is helpful.

Compression and elevation also support healing. Wearing a supportive compression sleeve or wrapping the ankle and lower leg with a compression bandage provides external support and may reduce swelling. Elevation—keeping your leg raised above heart level—helps fluid drain from the injured area, further reducing swelling. When sitting or lying down, prop your leg on pillows so your foot is higher than your hip.

Over-the-counter pain medications like ibuprofen or naproxen may provide relief and reduce inflammation when taken as directed on the packaging. However, these should be used for the shortest period possible and ideally combined with other self-care measures rather than used alone. Some people find that gentle stretching—holding a calf stretch for 20 to 30 seconds, performed several times daily—helps reduce pain once the acute phase passes. Always avoid stretching through sharp pain; mild discomfort with gentle stretching is acceptable, but sharp pain indicates you should stop.

Practical Takeaway: Initial self-care involves reducing activity that triggers pain, icing the area for 15-20 minutes several times daily, using compression and elevation to reduce swelling, and gentle stretching once acute pain improves. These basic measures often prevent minor tendon issues from becoming chronic problems.

Physical Therapy and Strengthening Exercises

Physical therapy plays a central role in treating Achilles tendon pain because it addresses the underlying weakness or imbalance that often contributes to the problem. A physical therapist can assess your movement patterns, muscle strength, flexibility, and the mechanics of how you walk or run. This personalized assessment identifies specific deficits that, when corrected, reduce stress on the Achilles tendon and prevent recurrence.

Strengthening exercises for the calf muscles are fundamental to tendon recovery. The calf consists of two muscles—the gastrocnemius and soleus—that both attach to the Achilles tendon. Strengthening these muscles transfers some of the stress from the tendon to the muscle tissue, which recovers more readily. Common exercises include heel raises (standing and rising onto your toes, performed slowly for 3 seconds up and 3 seconds down), seated calf raises with weight on your lap, and resistance band exercises where the band provides opposition as you point and flex your foot. Beginning with body weight and progressing to added weight or resistance allows gradual strengthening without overloading the tendon.

Eccentric exercises—where you lengthen the muscle while resisting—show particularly strong results for chronic Achilles tendon problems. One effective eccentric exercise involves standing with your toes on a step and your heels hanging off the edge. Rise onto your toes using both legs, then slowly lower your heel below step level using only one leg, taking 3 to 5 seconds for the lowering phase. Repeat for 15 repetitions, rest, then perform another set. This exercise can be done twice daily and often produces noticeable improvement within 4 to 6 weeks.

Flexibility work addresses tightness in the calf and surrounding tissues. Stretches should be held gently for 20 to 30 seconds without bouncing, performed several times daily. A wall stretch, where you face a wall with one leg extended behind you and gently lean forward until feeling a stretch in the calf, effectively lengthens the gastrocnemius muscle.
