Learn How To Stop a Nosebleed That Won't Stop

Understanding Why Nosebleeds Happen and When to Act

Nosebleeds, known medically as epistaxis, occur when blood vessels inside the nose rupture and bleed. The inside of your nose contains many small blood vessels that sit close to the surface. When these vessels break, blood flows out through your nostrils. Understanding what causes nosebleeds helps you know when you can handle one at home and when you need medical attention.

Most nosebleeds fall into two categories: anterior nosebleeds and posterior nosebleeds. Anterior nosebleeds start in the front part of the nose and account for about 90% of all nosebleeds. These typically look dramatic with visible bleeding but are usually easier to stop. Posterior nosebleeds occur deeper in the nose and can be more serious because blood may flow down the throat instead of out the nostrils, making it harder to notice how much you're bleeding.

Common triggers for nosebleeds include dry air, particularly during winter months or in heated indoor spaces, trauma to the nose from picking or blowing too hard, allergies causing inflammation, high blood pressure, blood thinning medications, and nasal infections. Dry climates create an especially high risk—people living in arid regions experience nosebleeds more frequently than those in humid areas. Certain medical conditions like hemophilia or von Willebrand disease affect how blood clots and can lead to frequent nosebleeds.

You should seek immediate medical care if your nosebleed lasts longer than 30 minutes despite following proper stopping techniques, if you're bleeding from both nostrils simultaneously, if blood is flowing down your throat in large amounts, if you've had multiple nosebleeds in a short time period, if you take blood thinning medications and cannot stop the bleeding, or if the nosebleed followed a significant head injury. These situations may indicate a more serious underlying condition requiring professional evaluation.

Practical Takeaway: Most nosebleeds stop within 10-15 minutes with proper technique. Knowing whether you have an anterior or posterior nosebleed and understanding your personal risk factors helps you respond appropriately and recognize when professional medical evaluation is necessary.

Immediate First Steps to Control Active Bleeding

The moment a nosebleed starts, your first action should be to remain calm. Panic increases heart rate and blood pressure, which can actually make bleeding worse. Sit upright or lean slightly forward—never tilt your head back. This common misconception comes from older medical advice, but tilting back causes blood to drain down your throat, which can lead to nausea, vomiting, or aspiration of blood into your lungs. Leaning forward allows blood to flow out of your nose where you can monitor it.

Immediately pinch the soft part of your nose—the area below the hard bridge. Use your thumb and index finger to apply firm, direct pressure to both sides of the nose. Pinch for a full 10 to 15 minutes without releasing to check progress. Releasing frequently prevents clotting and restarts the bleeding. Set a timer on your phone or watch to avoid the temptation to peek. During this time, breathe through your mouth slowly and calmly.

While pinching, apply ice to the bridge of your nose or the area between your eyebrows. Cold constricts blood vessels and promotes clotting. Wrap ice in a clean cloth rather than applying it directly to skin. Many people find that ice applied for five to ten minutes while maintaining pressure significantly reduces bleeding time. Some individuals report success with applying ice inside the mouth against the upper lip, though pinching remains the primary technique.

Stay seated and keep your head level with or slightly above your heart. Lying down flat or hanging your head low increases pressure in the nose and makes bleeding continue longer. Avoid strenuous activity, bending over, or lifting heavy objects for at least 24 hours after the bleeding stops, as these actions increase blood pressure and can restart bleeding.

Practical Takeaway: Firm, sustained pressure for 10-15 minutes with forward-leaning position stops the majority of nosebleeds. Using ice during this time enhances clotting. Patience during these minutes is crucial—releasing pressure too early is the main reason nosebleeds continue.

What to Do If the Nosebleed Continues Past 15 Minutes

If bleeding continues after your first 15-minute pressure session, don't panic. Some nosebleeds require multiple cycles of pressure, particularly if they're more severe or if you're taking blood thinning medications. After the initial 15 minutes, you may gently release pressure to assess whether bleeding has slowed. If blood still flows freely, apply pressure again for another 10 to 15 minutes.

Between pressure cycles, you can rinse your mouth gently with cool water to remove blood clots that may have formed in your mouth or throat. However, avoid vigorous rinsing or spitting forcefully, as this can dislodge developing clots. If you need to blow your nose in future days, do so gently and only if necessary.

During extended nosebleeds, you may consider using topical treatments to help with hemostasis. Nasal decongestant sprays containing phenylephrine or oxymetazoline cause blood vessels to constrict, potentially reducing blood flow. Spray the product into the bleeding nostril while maintaining pressure. These medications typically work within minutes. Avoid using decongestant sprays more than three times daily or for more than three consecutive days, as overuse can cause rebound congestion and increased bleeding.

Another option involves applying topical hemostatic agents. Hydrogen peroxide applied with gauze can help clear old blood and facilitate clotting. Some people use petroleum jelly or antibiotic ointment to coat the nasal passages, which reduces drying and irritation. Specialized products like absorbable gelatin sponges or topical thrombin are available over-the-counter at pharmacies but work best when applied by medical professionals.

Practical Takeaway: Multiple pressure cycles over 30-45 minutes are sometimes necessary. Decongestant sprays and gentle rinsing between cycles may help. If bleeding persists beyond 30 minutes total, or if you cannot control it through these methods, medical evaluation becomes important.

Managing Nosebleeds When Blood Thinning Medications Are Involved

People taking blood thinning medications like warfarin, apixaban, rivaroxaban, or dabigatran often experience more frequent and more difficult-to-stop nosebleeds. These medications intentionally thin blood to prevent clots in conditions like atrial fibrillation or after certain surgical procedures. If you take blood thinners and have a nosebleed, the same basic approach applies—lean forward and apply direct pressure—but you should be prepared for longer stopping times.

Blood thinner medications can increase nosebleed duration from the typical 10-15 minutes to 20-45 minutes or longer. Some individuals report bleeding that continues intermittently for several hours. This extended timeline doesn't automatically mean something is wrong; it's an expected side effect of these medications. However, you should monitor closely and contact your healthcare provider if bleeding lasts longer than one hour despite proper technique or if you experience multiple nosebleeds in a short time period.

If you take aspirin for heart health, you might wonder whether to stop taking it during a nosebleed. Generally, continuing aspirin as prescribed is recommended unless your doctor has specifically instructed otherwise. Stopping aspirin suddenly can increase your risk of heart attack or stroke. However, contact your healthcare provider or pharmacist during the nosebleed for personalized guidance, as your specific medical situation matters.

Preventive measures become especially important for people on blood thinners. Keep nasal passages moist by using saline rinses or nasal moisturizers daily. Use a humidifier in dry climates or during winter heating season. Avoid picking at your nose or inserting anything that could traumatize nasal tissue. Avoid nonsteroidal anti-inflammatory drugs like ibuprofen or naproxen unless approved by your doctor, as these can further thin blood and increase bleeding risk. Some antihistamines can dry nasal passages, so discuss allergy management with your healthcare provider.

Practical Takeaway: Blood thinner medications significantly extend how long nosebleeds take to stop, but the stopping technique remains the same
