Learn About Drinks to Avoid With AFib

Understanding Atrial Fibrillation and Beverage Choices

Atrial fibrillation, commonly called AFib, is a heart condition where the upper chambers of the heart beat irregularly and often too fast. According to the Centers for Disease Control and Prevention, about 2.7 to 6.1 million Americans have AFib. When someone has AFib, the heart may not pump blood as effectively as it should, which can lead to blood clots, stroke, and other complications.

People living with AFib often need to make changes to their daily habits, including what they drink. The beverages someone consumes can directly affect heart rhythm and how well AFib medications work. Certain drinks can trigger AFib episodes or make symptoms worse, while others are generally safer choices. Understanding which drinks to limit or avoid is an important part of managing this condition.

This guide provides information about beverages that people with AFib may want to reduce or avoid. The information comes from medical research and guidance from heart health organizations. It's important to note that each person's AFib is different, and what affects one person may not affect another in the same way. Someone with AFib should always discuss their specific diet and beverage choices with their doctor or cardiologist, who knows their individual health situation.

Practical Takeaway: Keep a simple log of what you drink and when you experience AFib symptoms. This personal record can help you and your doctor identify patterns specific to your own condition.

Caffeine and Its Effects on Heart Rhythm

Caffeine is a stimulant found in many common beverages. It works by blocking chemicals in the brain that make people feel tired, and it also increases heart rate and blood pressure. For people with AFib, caffeine can be particularly troubling because a faster heart rate and increased blood pressure may trigger irregular heartbeats.

Research published in the Journal of the American Heart Association found that people with AFib who consumed caffeine sometimes experienced more frequent arrhythmia episodes. However, the research also showed that the effect varies from person to person. Some people with AFib can tolerate small amounts of caffeine without problems, while others find that even modest amounts trigger symptoms.

Common beverages containing caffeine include:

• Coffee (95-200 mg of caffeine per 8-ounce cup, depending on brewing method)
• Black tea (25-50 mg per 8-ounce cup)
• Green tea (25-50 mg per 8-ounce cup)
• Energy drinks (80-300+ mg per serving)
• Cola and other sodas (30-40 mg per 12-ounce can)
• Chocolate drinks (5-10 mg per 8-ounce serving)

For people with AFib, doctors often recommend limiting caffeine intake or avoiding it completely. Some people find they can handle decaffeinated versions of their favorite beverages. Decaffeinated coffee and tea still contain trace amounts of caffeine but significantly less than regular versions.

Practical Takeaway: If you enjoy coffee or tea, talk with your doctor about whether you might try decaffeinated alternatives. You can gradually reduce regular caffeine intake over a few weeks to avoid withdrawal headaches while you adjust to lower-caffeine options.

Alcohol Consumption and AFib Risk

Alcohol has a well-documented connection to AFib. The relationship is so clear that doctors sometimes use the term "holiday heart syndrome" to describe AFib episodes triggered by heavy alcohol use, particularly around holidays when drinking increases. Research from the American Heart Association indicates that regular alcohol consumption significantly increases AFib risk, and even moderate amounts can trigger episodes in people who already have the condition.

Alcohol affects the heart in several ways. It can increase blood pressure, speed up heart rate, and cause inflammation in the heart muscle. Alcohol also interferes with the heart's electrical system, which controls the rhythm of heartbeats. For someone with AFib, this disruption can turn irregular beats into a full episode requiring medical attention.

The risk rises with the amount consumed. Studies show that people who drink more than 10 drinks per week have significantly higher AFib rates than non-drinkers. However, even moderate drinking—defined as one drink per day for women and two for men—may trigger AFib episodes in susceptible people. Some people with AFib find that even a single alcoholic drink causes their heart to become irregular.

Types of alcoholic beverages include beer, wine, liquor, and mixed drinks. All contain alcohol, and all carry the same risk for AFib triggers. Some people wonder whether certain types are safer; however, the problem is the alcohol itself, not the type of drink it comes in. A glass of wine has similar effects on AFib as a beer or a cocktail.

Non-alcoholic beer and wine are available options that provide similar taste without the alcohol. These products contain little to no alcohol and do not carry the same AFib risks. Some people find these alternatives satisfying when they want to participate in social situations involving drinks.

Practical Takeaway: If you have AFib and drink alcohol, work with your doctor to determine what level of drinking, if any, is safe for your specific situation. Some people with AFib are advised to avoid alcohol completely, while others may handle small amounts. Never assume that any amount is safe without discussing it with your healthcare provider first.

Energy Drinks and High-Stimulant Beverages

Energy drinks have become increasingly popular, particularly among younger adults. These beverages are specially formulated to provide a quick energy boost and typically contain high levels of caffeine along with other stimulating ingredients. For people with AFib, energy drinks present a significant risk and are generally considered something to avoid.

A typical energy drink can contain 80 to 300 milligrams of caffeine per serving—sometimes even more. To put this in perspective, that's equivalent to drinking two to four cups of coffee in one can. Beyond caffeine, many energy drinks contain additional stimulants like taurine, guarana (which contains caffeine), and ginseng. These ingredients work together to increase heart rate, raise blood pressure, and stimulate the nervous system.

Medical case reports have documented AFib episodes and other serious heart problems occurring shortly after people consumed energy drinks. The American Heart Association has expressed concern about energy drink consumption among people with heart conditions. Some energy drinks have been associated with heart palpitations, arrhythmias, and in rare cases, heart attacks.

Energy drinks marketed as "natural" or "organic" still contain high amounts of stimulants and carry the same risks. Labels showing ingredients like "proprietary blend" or "natural energy complex" often hide significant amounts of caffeine and other stimulants. Reading the nutrition facts label is the only reliable way to know the actual caffeine content.

Sports drinks formulated for hydration and electrolyte replacement are different from energy drinks. Most sports drinks contain little to no caffeine and focus instead on replacing fluids and electrolytes lost during exercise. These are generally considered safer options for people with AFib, though they can be high in sugar, so drinking them in moderation remains important.

Practical Takeaway: Check the nutrition label on any beverage labeled "energy drink," "performance drink," or "power drink." If it contains more than 100 mg of caffeine, it's likely not recommended for someone with AFib. When in doubt, ask your doctor whether a specific drink is appropriate for your condition.

Sugar, Sweeteners, and Metabolic Effects

While sugar itself doesn't directly cause AFib in the way that caffeine or alcohol does, high sugar consumption can contribute to conditions that worsen AFib. Excess sugar intake is linked to weight gain, type 2 diabetes, and high blood pressure—all conditions that increase AFib risk and severity. Many beverages contain surprisingly high amounts of added sugar, and regularly consuming sugary drinks can affect overall heart health.

A single 12-ounce can of regular soda contains about 39 grams of sugar, roughly the entire daily recommended limit for added sugar. Sweetened iced tea, fruit juices, and energy drinks often contain similar amounts. Over time, regularly consuming these drinks contributes to weight gain and metabolic problems that strain the heart and worsen AFib.

Artificial sweeteners were developed as alternatives to sugar, and they do not contribute calories or raise blood sugar
