Understanding Tennis Elbow: What Happens to Your Arm

Tennis elbow, medically called lateral epicondylitis, is a condition where the tendons on the outer part of your elbow become irritated and inflamed. The tendons are thick cords that connect your muscles to your bones. When you use your forearm muscles repeatedly—especially with gripping, twisting, or lifting motions—tiny tears can develop in these tendons. This inflammation causes pain on the outside of your elbow that can range from mild discomfort to sharp, severe pain.

You don't have to play tennis to develop tennis elbow. In fact, most people who get this condition don't play tennis at all. Studies show that only about 5% to 10% of tennis elbow cases occur in actual tennis players. The condition is common in people who do repetitive arm work, such as painters, plumbers, carpenters, butchers, and office workers who use computers for many hours each day. The Occupational Safety and Health Administration (OSHA) reports that repetitive strain injuries account for a significant portion of workplace injuries in the United States.

The pain typically starts gradually and worsens over time. You might notice it when you grip objects, shake hands, or lift something with your palm facing down. Some people describe a burning sensation on the outer part of the elbow, while others feel a dull ache that radiates down the forearm toward the wrist. The pain may be worse at night or after activity, and you might lose grip strength in the affected hand.

Understanding what's happening inside your elbow helps you recognize why rest and specific treatment methods work. The inflammation develops because the tendons have been stressed beyond what they can handle. Your body responds by sending inflammatory cells to the area, which causes swelling, warmth, and pain. This is actually your body's attempt to protect and heal the injured tissue, but the inflammation itself can become painful and limiting.

Practical Takeaway: Tennis elbow develops from overuse of forearm tendons and affects people in many different professions, not just tennis players. Recognizing the gradual onset of outer elbow pain and understanding that it's an inflammatory response to repetitive stress helps you take early action to prevent the condition from worsening.

Home Treatment Strategies That Reduce Pain and Swelling

The foundation of home treatment for tennis elbow is the R.I.C.E. protocol, which stands for Rest, Ice, Compression, and Elevation. Rest means reducing or stopping the activity that caused the problem. You don't need complete immobility—movement is actually important for healing—but you should avoid motions that trigger pain. If gripping hurts, modify your grip by using larger-handled tools or spreading the load across your whole hand rather than squeezing with your fingers alone.

Ice reduces inflammation and numbs pain. Applying ice for 15 to 20 minutes at a time, several times per day, can significantly decrease swelling. Many people find that icing works best in the first 48 to 72 hours after symptoms begin, though it remains useful for several weeks. You can use a commercial ice pack, a bag of frozen vegetables, or a cloth-wrapped ice cube. Never apply ice directly to skin, as this can cause ice burn. Some people alternate ice with heat after the first few days—ice for acute inflammation and heat for stiffness.

Compression with an elastic bandage or a specialized elbow strap reduces swelling and provides support. The strap should be snug but not so tight that it cuts off circulation or causes numbness. A lateral epicondyle strap, which places pressure on the tendon rather than directly over the elbow joint, is particularly effective for tennis elbow. Studies published in the British Journal of Sports Medicine found that wearing a strap reduced pain and improved function in people with tennis elbow. Wearing the strap during activities that trigger pain can reduce symptoms by up to 50% in some cases.

Elevation means keeping your arm raised above heart level when possible. This uses gravity to reduce swelling. Prop your elbow on pillows while sitting or lying down. Combining all four elements of R.I.C.E.—rest from aggravating activities, ice to reduce inflammation, compression to limit swelling, and elevation to drain fluid—addresses the underlying inflammation rather than just masking pain.

Anti-inflammatory medications available over the counter, such as ibuprofen or naproxen, can reduce pain and swelling when used as directed on the package. These work by blocking the chemicals in your body that cause inflammation. Taking these medications consistently during the first few weeks of symptoms, rather than waiting until pain is severe, produces better results. However, these medications come with potential side effects, so follow package instructions and talk with a pharmacist or doctor if you have questions.

Practical Takeaway: Combining rest from aggravating activities, ice therapy, compression with a strap, and elevation provides a multi-pronged approach to reducing inflammation. These methods work together to address the root cause of tennis elbow rather than just covering up symptoms.

Exercises and Stretches for Recovery and Strength

While rest is important, complete immobility actually slows healing. Physical therapists recommend specific exercises that gently strengthen the affected tendons and muscles while avoiding movements that cause sharp pain. The key is finding the balance between activity and rest. When symptoms first begin, gentle range-of-motion exercises help prevent stiffness. As pain decreases over one to two weeks, you can gradually introduce gentle strengthening exercises.

One common and effective exercise is the "eccentric wrist extension." This involves lowering your hand slowly from an extended position using only the affected arm while your other hand helps lift it. Here's how to perform it: Hold your arm out with your palm facing down, and use your other hand to help lift your wrist backward slightly. Then slowly lower your hand downward over a period of about three seconds using only the injured arm, resisting the motion with gentle tension. Repeat this 15 times, several times per day. Research published in the American Journal of Sports Medicine shows that eccentric exercises reduce tennis elbow pain by 50% or more in many patients after four to six weeks of consistent practice.

Stretching the forearm muscles reduces tension on the tendons. A simple stretch involves extending your affected arm straight out with your palm facing up. Use your other hand to gently press the back of your fingers downward, creating a mild stretch along the inside of your forearm. Hold this stretch for 15 to 30 seconds and repeat several times. Another effective stretch is to press your palm against a wall with your arm extended, fingers pointing downward at a 45-degree angle, and lean gently forward until you feel a stretch along the outer forearm.

Grip strength training, performed correctly, helps rebuild the tendons' capacity to handle stress. Begin with a soft stress ball or therapy putty. Squeeze gently without causing pain—you should feel resistance but no sharp discomfort. Hold the squeeze for a few seconds and release. Progress gradually to more resistant objects as pain decreases. Some people find that squeezing a tennis ball or using a hand-grip strengthener works, but start very gently; forcing grip strength too early can set back recovery by several weeks.

These exercises should never cause sharp or shooting pain. Mild discomfort or a gentle ache is normal, but pain that worsens during or after exercise suggests you're doing too much. Progress slowly over several weeks. A common mistake is advancing exercises too quickly because pain has improved. Tennis elbow takes time to heal—the tendons need weeks to months to fully strengthen, depending on severity.

Practical Takeaway: Starting with gentle range-of-motion exercises and gradually progressing to eccentric strengthening exercises, stretches, and grip training helps rebuild tendon strength while avoiding re-injury. Progress slowly and listen to your body's pain signals.

When to Seek Doctor Care and What to Expect

While many cases of tennis elbow improve with home treatment, certain situations warrant seeing a doctor. If pain persists beyond two to three weeks despite consistent home treatment, if pain is severe enough to significantly limit your daily activities, or if your symptoms keep returning, a medical evaluation is worthwhile. Similarly, if you suddenly develop severe pain from an obvious injury or if you have numbness, tingling, or significant weakness, these may indicate a different condition requiring professional evaluation.
