Learn About Vertigo Treatment Options and When to See a Doctor

Understanding Vertigo: Types and Common Causes

Vertigo is a spinning sensation that makes you feel like the room is moving around you, even when you're standing still. It's different from general dizziness or lightheadedness. When you experience vertigo, you might feel like you're on a spinning carnival ride or that the walls are rotating. This sensation can last from a few seconds to several hours or even days, depending on the underlying cause.

There are two main types of vertigo: peripheral and central. Peripheral vertigo happens when there's a problem with your inner ear or the nerve that connects it to your brain. The inner ear controls your balance, so when something goes wrong there, your brain receives confusing signals about your body's position. This type accounts for about 80 percent of vertigo cases. Central vertigo occurs when there's an issue in your brain or brainstem. While less common, central vertigo can indicate a more serious condition.

Common causes of peripheral vertigo include benign paroxysmal positional vertigo (BPPV), vestibulitis, and Meniere's disease. BPPV happens when tiny calcium carbonate crystals in your inner ear become loose and move around, sending mixed signals to your brain. You might notice BPPV gets worse when you move your head in certain directions. Vestibulitis is inflammation of the vestibular nerve, often triggered by a viral infection. Meniere's disease causes fluid buildup in the inner ear and can produce vertigo, hearing loss, tinnitus (ringing in the ears), and a feeling of fullness in the affected ear.

Central vertigo can stem from conditions like migraines, stroke, multiple sclerosis, or tumor growth. These conditions require different treatment approaches than peripheral vertigo. Other factors that may trigger vertigo include certain medications, head injuries, and rapid movements. Understanding which type of vertigo you experience helps doctors recommend the right treatment approach.

Takeaway: Vertigo involves a spinning sensation related to balance problems, most commonly from inner ear issues. Knowing whether your vertigo is peripheral or central helps guide treatment decisions.

Recognizing Vertigo Symptoms and When They Warrant Medical Attention

The primary symptom of vertigo is a spinning sensation, but the experience varies widely. Some people describe it as the room spinning around them, while others feel like they're spinning while the room stays still. This sensation can be mild enough to work through or severe enough to make you unable to stand. You might also experience nausea, vomiting, balance problems, difficulty walking in a straight line, or involuntary eye movements called nystagmus.

Vertigo episodes can be brief or long-lasting. Someone with BPPV might experience a few seconds of intense spinning when rolling over in bed, while someone with vestibulitis might feel moderate spinning for days. The frequency also varies—some people have isolated incidents, while others experience repeated episodes over weeks or months. Associated symptoms can include hearing loss, ear fullness, tinnitus, headaches, sensitivity to light, and difficulty concentrating.

You should contact a healthcare provider if vertigo is new or severe, lasts longer than a few hours, occurs repeatedly over days or weeks, or is accompanied by other symptoms. Seek immediate medical attention if you experience vertigo along with chest pain, difficulty speaking, facial drooping, weakness on one side of your body, loss of consciousness, or severe headache. These could indicate a stroke or other serious condition. If you hit your head recently and then developed vertigo, medical evaluation is important. Similarly, if vertigo occurs after starting a new medication, report this to your doctor.

Keep track of your vertigo episodes by noting when they occur, how long they last, what triggers them, and what makes them better or worse. Note any accompanying symptoms and whether you've had recent infections, head injuries, or medication changes. This information helps your doctor understand your condition. Some people notice vertigo is triggered by specific head movements, changes in position, stress, certain foods, or caffeine. Recording these patterns provides valuable information.

Takeaway: While mild, brief vertigo can sometimes resolve on its own, new or recurring vertigo warrants a doctor's evaluation. Tracking your symptoms helps your healthcare provider make an accurate diagnosis.

Self-Care Measures and Home Treatments

For mild vertigo episodes, several self-care approaches may help manage symptoms while you're waiting to see a doctor or after a diagnosis. The Epley maneuver is a specific head-positioning technique that moves loose calcium crystals in the inner ear back to their proper place. This technique works particularly well for BPPV. The maneuver involves a series of head movements performed while sitting on a bed: you tilt your head back over the edge, turn it to one side, then move your body to lie down on that side before returning to sitting. Many people experience relief after one or two rounds of the Epley maneuver, though some need multiple sessions.

Vestibular rehabilitation therapy (VRT) involves specific exercises designed to help your brain compensate for inner ear problems. These exercises retrain your balance system and reduce dizziness over time. Examples include focusing your eyes on a fixed point while moving your head, moving your eyes to follow a moving object, or changing positions slowly and deliberately. A physical therapist specializing in vestibular disorders can teach you these exercises, though your doctor might also provide instructions. Research shows that consistent VRT practice can significantly reduce vertigo symptoms in people with peripheral causes.

During acute vertigo episodes, staying calm and still often helps. Lie down in a comfortable position, avoid sudden movements, and keep your eyes focused on a stationary object. Stay hydrated and avoid sudden position changes like sitting up too quickly or getting out of bed abruptly. Some people find that moving slowly and deliberately through activities reduces symptoms. Avoid driving, using heavy machinery, or climbing stairs until your symptoms improve, as vertigo significantly increases injury risk during these activities.

Lifestyle adjustments may reduce vertigo frequency. Limit sudden head movements and changes in position. Rise from bed or chairs slowly, sitting on the edge first before standing. Avoid activities that trigger your vertigo if you've identified specific triggers. Some people benefit from reducing caffeine and sodium intake, managing stress, and getting adequate sleep. Staying well-hydrated is also important. While these measures help manage mild cases, they're not substitutes for medical evaluation when vertigo is new, severe, or recurring.

Takeaway: Several home-based techniques—including the Epley maneuver, vestibular exercises, and lifestyle adjustments—can help manage vertigo symptoms, but these work best alongside medical guidance.

Medical Treatments and Medications

When vertigo requires medical treatment, your doctor has several medication options depending on the underlying cause. Antihistamines like meclizine (Antivert) and dimenhydrinate (Dramamine) help reduce vertigo and associated nausea by affecting the vestibular system in your inner ear. These medications work reasonably well for acute episodes but typically aren't used long-term. Anticholinergic medications like scopolamine work similarly and may be delivered through a patch worn behind the ear. Benzodiazepines like diazepam (Valium) can reduce vertigo and anxiety but carry risks of dependence with long-term use, so doctors typically prescribe these only for short-term relief during severe episodes.

For Meniere's disease specifically, doctors may prescribe diuretics like hydrochlorothiazide to reduce fluid buildup in the inner ear. Corticosteroids might be recommended to reduce inflammation, particularly in vestibulitis cases. Some conditions benefit from antibiotics if infection is involved. Migraine-related vertigo may improve with migraine medications. Your doctor selects medications based on your specific diagnosis, other health conditions, and medications you're already taking.

Physical therapy remains a cornerstone of vertigo treatment. Vestibular rehabilitation therapy, performed under a physical therapist's guidance, shows strong evidence of effectiveness, particularly for peripheral vertigo. A therapist designs a customized exercise program targeting your specific balance problems. Treatment typically involves weekly sessions for several weeks. Studies indicate that about 80 percent of people with peripheral vertigo improve significantly with vestibular rehabilitation.

More specialized procedures exist for specific conditions. The Dix-Hallpike maneuver, similar to the Epley maneuver, is a diagnostic and therapeutic technique performed in a doctor's office. Gentamicin injections directly into the middle ear may be considered for severe Meniere's disease
