Learn About Shingles Vaccine Timing for Older Adults

Understanding Shingles and Why Vaccination Matters for Older Adults

Shingles is a painful skin condition caused by the varicella-zoster virus, the same virus that causes chickenpox. If you had chickenpox at any point in your life, the virus remains dormant in nerve cells in your body. Years or decades later, the virus can reactivate and cause shingles, also called herpes zoster. About one in three Americans will develop shingles during their lifetime, according to the CDC. The risk increases significantly after age 50, with the incidence roughly doubling every decade after that age.

The condition typically starts with burning pain, tingling, or itching on one side of the body, usually in a band-like pattern. Within a few days, a painful rash appears with fluid-filled blisters. The rash usually clears up within 2 to 4 weeks, but the pain can persist for months or even years after the rash heals—a condition called postherpetic neuralgia (PHN). This lingering pain affects approximately 10 to 18 percent of people who have shingles, with older adults facing higher rates. PHN can interfere with daily activities, sleep, and quality of life.

Shingles vaccination offers a way to reduce the risk of developing shingles and PHN. The newer shingles vaccine, called Shingrix (recombinant zoster vaccine), became available in 2017 and has proven highly effective. In clinical trials, Shingrix prevented shingles in more than 90 percent of vaccinated individuals across all age groups. The vaccine's protective effect remains strong over at least seven years of follow-up, based on current research.

Practical takeaway: Understanding that shingles risk increases with age and that vaccination can prevent this painful condition helps older adults make informed decisions about their health. Even if you had shingles before, you can still benefit from vaccination to prevent future occurrences.

Overview of Shingrix: How the Vaccine Works and What to Expect

Shingrix is an inactivated recombinant vaccine, meaning it does not contain any live virus. Instead, it contains a piece of the virus plus an adjuvant—a component that boosts the immune system's response. This design makes it safe for older adults and immunocompromised individuals, including those with weakened immune systems. The vaccine works by training the immune system to recognize and fight the varicella-zoster virus before it can reactivate and cause shingles.

Shingrix comes as a two-dose series. You receive the first dose at an initial appointment, and then the second dose anywhere from 2 to 6 months after the first dose. Both doses are necessary for full protection. The CDC recommends spacing the doses 2 to 3 months apart for optimal timing, though the vaccine is still considered effective with spacing up to 6 months. Each injection is given in the upper arm muscle (deltoid).

After receiving Shingrix, some people experience side effects, though serious side effects are rare. Common side effects at the injection site include redness, soreness, swelling, or warmth. Systemic side effects—affecting the whole body—may include fatigue, muscle aches, headache, fever, or chills. These side effects typically appear within 2 to 3 days after vaccination and resolve within 2 to 3 days. About half of people who receive Shingrix report some side effect, but most describe them as mild to moderate. Serious allergic reactions are very rare, occurring in fewer than one person per million vaccinations.

Unlike some older vaccines, Shingrix can be given at any time of year and does not require consideration of seasonal timing. You do not need to wait for a specific season or avoid getting it during certain months. The vaccine can also be given regardless of whether you had chickenpox or whether you had shingles previously.

Practical takeaway: Shingrix requires two doses spaced 2 to 6 months apart. Mild side effects are common and temporary. Because the vaccine contains no live virus, it is safe for older adults with various health conditions.

Age-Based Vaccination Timing Recommendations

The CDC recommends Shingrix for all adults age 50 and older. Within this broad age range, timing considerations differ based on your specific age and health status. For adults age 50 to 59, Shingrix is recommended but timing is more flexible than for older age groups. There is no urgency to vaccinate at exactly age 50; you can receive the vaccine any time during your 50s. However, because shingles risk increases with advancing age, getting vaccinated during your 50s means you receive protection during a time when risk is beginning to climb.

For adults age 60 and older, the recommendation takes on greater importance, particularly if you have not yet been vaccinated. About 1 in 2 older adults will develop shingles at some point if not vaccinated, compared to 1 in 3 among younger groups. By age 70, approximately 50 percent of unvaccinated people will have had shingles. The protective benefit of vaccination remains strong across this age range, with effectiveness around 90 percent or higher regardless of whether you are 60, 70, 80, or older. This means it is never too late to get vaccinated.

Special timing considerations apply if you are chronically ill, immunocompromised, or take medications that affect immunity. People with conditions like diabetes, heart disease, chronic obstructive pulmonary disease (COPD), or autoimmune conditions face higher risks of shingles and PHN. These individuals benefit from vaccination and should discuss timing with their healthcare provider to coordinate around their medical appointments or treatment schedules. Immunocompromised individuals can safely receive Shingrix, unlike some other vaccines, but may want to time it alongside other medical visits.

If you previously had shingles, you can still receive Shingrix. The CDC recommends waiting until the shingles rash heals before getting vaccinated, which typically means waiting a few weeks. Vaccination after shingles helps prevent future episodes, as the virus can reactivate again. About 1 to 4 percent of people who had shingles will experience it again, and vaccination reduces this risk.

Practical takeaway: Adults age 50 and older should plan to receive Shingrix; timing is flexible for those in their 50s but becomes more important for those 60 and older. It is never too late to get vaccinated, even in your 80s or beyond, and vaccination is recommended even if you had shingles previously.

Practical Steps for Scheduling Your Vaccination

Once you decide to get vaccinated, the process of scheduling involves several straightforward steps. Start by contacting your primary care doctor's office, a local pharmacy, or a community health center to ask whether they offer Shingrix. Many healthcare settings, including traditional medical offices, urgent care clinics, pharmacies like CVS and Walgreens, and federally qualified health centers, have the vaccine available. When you call, let them know you need two doses and ask about their scheduling process for the two-dose series.

Most healthcare settings can schedule both doses at your first appointment or can give you specific dates for both injections in advance. If they schedule you for the first dose only, ask when and how to schedule the second dose. Planning ahead for both doses helps ensure you complete the series within the recommended 2- to 6-month window. Write down your second dose appointment or set a reminder on your calendar to call and schedule it if your provider does not do this at the first visit.

Before your appointment, gather basic information you will likely need to provide: your date of birth, insurance information if you have it, a list of any medications you take, and information about any allergies. If you have a history of severe allergic reactions, particularly to any vaccine components, let your provider know in advance. When you arrive for your vaccination, you will likely complete a health history form. Be honest about any recent illnesses, medications, or health conditions, as this helps your provider determine whether the timing is appropriate.

After receiving your first dose, your provider will likely give you an appointment card or written information about when to return for your second dose. Do not lose this information. If you do not receive specific scheduling information, contact the office within a week to set up your second appointment. The ideal spacing is 2 to 3 months between
