Understanding Nerve Pain: Types and Causes

Nerve pain, also called neuropathic pain, happens when nerves themselves are damaged or not working correctly. This is different from other types of pain. When you touch a hot stove, pain signals travel through healthy nerves to tell your brain there's danger. With nerve pain, the nerves send pain signals even when there's no injury. The nerves may be sending signals constantly, or they may misinterpret normal sensations as painful.

There are many different types of nerve pain. Peripheral neuropathy affects nerves in your hands, feet, arms, and legs. Diabetic neuropathy is nerve damage caused by high blood sugar levels over time. Postherpetic neuralgia is pain that continues after a shingles infection clears up. Sciatica happens when the sciatic nerve, which runs from your lower back down your legs, gets compressed or irritated. Trigeminal neuralgia causes sharp, sudden pain in the face. Fibromyalgia causes widespread muscle and nerve pain throughout the body.

Nerve pain can develop from many causes. Diabetes is one of the most common causes, affecting about 60% of people with diabetes according to the National Institute of Diabetes and Digestive and Kidney Diseases. Infections, injuries, vitamin deficiencies, chemotherapy drugs, and autoimmune diseases can all damage nerves. Some people have nerve pain from no clear cause at all, called idiopathic neuropathy.

The symptoms of nerve pain are often distinctive. People describe tingling, numbness, burning sensations, or sharp shooting pain. Some feel like they're wearing a tight sock or glove even when they're not. The pain might be constant or come and go. It's often worse at night. Some people experience weakness in affected areas. These symptoms can affect sleep, work, and daily activities significantly.

Practical Takeaway: Identifying what type of nerve pain you have is the first step toward finding treatment. Write down when your pain happens, what it feels like, where it occurs, and what makes it better or worse. This information helps medical providers understand your condition and recommend suitable treatment approaches.

Over-the-Counter Treatment Options

Many people start with over-the-counter medicines to manage nerve pain. Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen can help with some types of nerve pain, though they work better for inflammatory pain than pure nerve pain. These medications reduce inflammation, which may ease pressure on affected nerves. Most people can buy these at pharmacies without a prescription, and they're relatively inexpensive.

Topical creams and patches offer another over-the-counter option. Capsaicin cream comes from hot peppers and can reduce nerve pain signals when applied to skin. It works by depleting substance P, a chemical that carries pain signals. People typically apply it several times daily. Lidocaine patches numb the skin surface and can reduce pain in specific areas. These patches contain a local anesthetic and stick directly to skin over the painful area. You can wear them for up to 12 hours at a time.

Acetaminophen is another option some people use, though research shows it's less effective for nerve pain than for other pain types. Unlike NSAIDs, it doesn't reduce inflammation. Some people combine acetaminophen with other treatments for better results. It's important to follow dosing instructions carefully since taking too much can damage the liver.

Vitamin supplements may help in certain situations. If nerve pain results from vitamin B12 or B6 deficiency, supplements might improve symptoms. Vitamin B12 is especially important for nerve health. Alpha-lipoic acid is an antioxidant that some research suggests may help with diabetic nerve pain. Magnesium plays a role in nerve function, and deficiency can contribute to nerve pain. However, supplements work best when deficiency is the actual cause of pain.

Over-the-counter pain management also includes lifestyle approaches. Ice packs can numb painful areas temporarily. Compression socks help some people with peripheral neuropathy by improving circulation. Soaking feet in warm water (not hot, to avoid burns) can provide temporary relief. Loose, soft clothing reduces irritation from fabrics touching sensitive skin.

Practical Takeaway: Keep a simple record of which over-the-counter treatments you try and how well they work. Note the dose, how long relief lasts, and any side effects. This information is valuable when discussing treatment with a doctor, as it shows what has and hasn't worked for your specific situation.

Prescription Medications for Nerve Pain

When over-the-counter options don't provide enough relief, doctors may prescribe medications specifically designed for nerve pain. Gabapentin is one of the most commonly prescribed medications for neuropathic pain. Originally developed to treat seizures, doctors discovered it also reduces nerve pain signals. It works by calming overactive nerves. Gabapentin typically starts at low doses and increases gradually as the body adjusts. Many people find significant relief with this medication, though it can cause drowsiness, dizziness, or weight gain in some patients.

Pregabalin is similar to gabapentin and works through comparable mechanisms. It was specifically approved by the FDA for diabetic neuropathy and postherpetic neuralgia. Some people find pregabalin more effective than gabapentin, while others respond better to gabapentin. Like gabapentin, it may cause drowsiness and dizziness, especially when starting treatment.

Certain antidepressants treat nerve pain even in people without depression. Tricyclic antidepressants like amitriptyline and nortriptyline can reduce pain signals. They work partly by affecting neurotransmitters like serotonin and norepinephrine that influence pain perception. Newer antidepressants like duloxetine and venlafaxine also help with nerve pain. Duloxetine was specifically approved by the FDA for diabetic peripheral neuropathic pain. These medications typically take several weeks to show full effectiveness.

Topical prescription medications provide another option. Prescription-strength capsaicin patches deliver higher doses than over-the-counter versions. Compounded creams can combine multiple pain-relieving ingredients tailored to individual needs. Some doctors prescribe compounded medications with ketamine, amitriptyline, or other substances to target specific pain patterns.

Other prescription medications treat nerve pain through different mechanisms. Tramadol is an opioid pain reliever that also affects serotonin and norepinephrine, providing some benefit for neuropathic pain. However, opioids carry risks of dependence and are typically used cautiously for nerve pain. Lidocaine injections around affected nerves can provide temporary relief. Some doctors use other medications off-label, meaning for purposes not specifically approved by the FDA but supported by research evidence.

Practical Takeaway: Prescription medications for nerve pain often require patience. Many take several weeks to reach full effectiveness. Side effects frequently improve over time as your body adjusts. Keeping notes about your response to medications—including when relief starts, how well it works, and any side effects—helps your doctor make adjustments or try different options if needed.

Medical Procedures and Interventional Treatments

Beyond medications, several medical procedures can reduce nerve pain. Nerve blocks involve injecting medication directly around the affected nerve to block pain signals. These injections typically contain local anesthetic and sometimes a corticosteroid to reduce inflammation. Nerve blocks can provide relief lasting from days to months, depending on the type and location. They're often used for specific areas like the foot, leg, or face. Doctors use ultrasound or other imaging to guide the needle to the correct location, improving accuracy and safety.

Radiofrequency ablation uses heat from radio waves to damage nerve fibers carrying pain signals. The procedure targets specific nerves that transmit pain from a particular area. Relief can last several months to a year before nerve fibers regenerate. This procedure works particularly well for pain in localized areas like the neck, lower back, or knees. It's minimally invasive, requiring only small needles, and most people return to normal activities within days.

Spinal cord stimulation involves implanting a small device that sends electrical signals to the spinal cord, interrupting pain messages before they reach the brain. A trial period lets people test the device for several days to see if it helps before committing
