Understanding Your Kidneys and How They Work

Your kidneys are two bean-shaped organs, each about the size of your fist, that sit on either side of your spine below the rib cage. Most people don't think much about their kidneys until something goes wrong, but these organs perform several critical functions every single day. Understanding how your kidneys work is the first step toward protecting them and maintaining your overall health.

Each kidney contains about one million tiny filtering units called nephrons. These nephrons work constantly to filter waste products and excess water from your blood. This process happens 24 hours a day, seven days a week, without you having to do anything. The filtered waste becomes urine, which travels through tubes called ureters to your bladder, where it is stored until you urinate. A healthy person produces about one to two quarts of urine daily, depending on how much fluid they drink and how much they sweat.

Beyond filtering waste, your kidneys have several other important jobs. They help regulate blood pressure by controlling the amount of fluid in your body. They produce a hormone called erythropoietin, which signals your bone marrow to make red blood cells. Your kidneys also activate vitamin D, which helps your body absorb calcium and keep your bones strong. They balance electrolytes like sodium, potassium, and phosphorus, which are minerals your body needs to function properly. When your kidneys work well, they maintain a delicate balance that keeps your whole body running smoothly.

Most people function normally with just one kidney, which is why kidney donation is possible. However, when both kidneys stop working properly, waste builds up in your blood to dangerous levels. This is why regular checkups that include kidney function tests are important, especially as you age or if you have risk factors like diabetes or high blood pressure. Practical takeaway: Ask your doctor about getting your kidney function checked during your annual physical, particularly if you have diabetes, high blood pressure, or a family history of kidney disease.

Common Kidney Health Risks and Who Is Most Affected

Kidney disease develops gradually in many cases, often without obvious symptoms in the early stages. According to the National Kidney Foundation, approximately 37 million American adults have chronic kidney disease, though many don't know it. Understanding your risk factors helps you take steps to protect your kidneys before problems develop. Several medical conditions and lifestyle factors increase the likelihood of kidney disease.

Diabetes is the leading cause of kidney disease, accounting for about 30% to 40% of all cases of kidney failure in the United States. When blood sugar levels stay high over many years, the small blood vessels in your kidneys can be damaged. This damage reduces the kidneys' ability to filter waste effectively. High blood pressure is the second leading cause of kidney disease, responsible for about 25% to 30% of cases. High blood pressure damages the tiny blood vessels in your kidneys, which eventually leads to reduced kidney function. People with both diabetes and high blood pressure face an even greater risk.

Other conditions that strain your kidneys include autoimmune diseases like lupus and rheumatoid arthritis, repeated kidney infections, and kidney stones. Certain medications, including some pain relievers called NSAIDs and some antibiotics, can damage kidneys if used frequently or at high doses. Obesity and smoking also increase kidney disease risk. Additionally, age plays a role—kidney function naturally declines with age, starting around age 30. African Americans, Native Americans, and Hispanic Americans have higher rates of kidney disease than other groups, partly due to higher rates of diabetes and high blood pressure in these populations.

A family history of kidney disease is another important risk factor. If your parents or siblings have had kidney problems, your risk is higher. Some people are born with genetic conditions that affect the kidneys, such as polycystic kidney disease, where cysts grow on the kidneys and interfere with their function. Practical takeaway: Make a list of your risk factors and discuss them with your doctor. If you have diabetes, high blood pressure, or a family history of kidney disease, ask how often you should have kidney function tests and what steps you can take to protect your kidneys.

Early Warning Signs and Symptoms of Kidney Problems

In the early stages of kidney disease, many people have no symptoms at all. This is why kidney disease is sometimes called a "silent" disease. By the time symptoms appear, kidney function may already be significantly reduced. However, learning about possible warning signs means you can seek medical attention sooner if you notice changes in your body. The earlier kidney problems are detected, the more options you have for treatment and management.

One of the most common signs of kidney problems is changes in how often you urinate or changes in the appearance of your urine. You might notice that you need to urinate more frequently, especially at night. Your urine might look darker, foamy, or bloody. Swelling in your hands, feet, ankles, or face can occur because your kidneys aren't removing extra fluid as they should. This swelling, called edema, often starts in the feet and ankles and gets worse as the day goes on. Some people also notice puffiness around their eyes, particularly in the morning.

Fatigue and weakness are common symptoms because your kidneys may not be producing enough erythropoietin, the hormone that helps your body make red blood cells. Without enough red blood cells, your body doesn't get enough oxygen, which causes tiredness and weakness. You might feel exhausted even after sleeping well. Some people experience shortness of breath, particularly when lying down or during physical activity. This can happen because fluid builds up in the lungs. Back pain or flank pain (pain in your sides) can occur, though not everyone with kidney problems experiences this symptom.

Other warning signs include nausea and vomiting, loss of appetite, metallic taste in your mouth, difficulty concentrating, itching, and muscle cramps. High blood pressure that's difficult to control may indicate kidney problems, since kidneys regulate blood pressure. It's important to understand that having one or two of these symptoms doesn't necessarily mean you have kidney disease—many conditions cause similar symptoms. However, if you notice several of these signs or if symptoms persist, contact your doctor for evaluation. Practical takeaway: Keep track of any changes you notice in your urination, energy level, appetite, or swelling. Write down when these changes started and mention them at your next doctor's appointment or call your doctor if symptoms seem severe.

Kidney Pain: Understanding Causes and When to Seek Care

Kidney pain is different from general back pain because it occurs deeper in the body and on the sides of your spine, below the rib cage. It's important to understand what kidney pain feels like because it can signal a serious condition that needs medical attention. Kidney pain is typically felt as a dull, constant ache, though some people describe it as sharp or stabbing. The pain may be on one side or both sides of your body. Unlike muscle pain that might feel sore when you touch it or move, kidney pain is usually deeper and doesn't change much with movement or position.

Several conditions cause kidney pain. Kidney stones are one of the most common causes. Kidney stones are hard mineral deposits that form in your kidneys and can cause intense pain as they pass through your urinary system. The pain from kidney stones is often sudden and severe, and people sometimes describe it as the worst pain they've ever experienced. Kidney infections, also called pyelonephritis, cause kidney pain along with fever, chills, nausea, and painful urination. Polycystic kidney disease, where multiple cysts grow in the kidneys, causes chronic kidney pain. Blood clots in the blood vessels that supply the kidneys or bleeding within the kidneys can cause sudden flank pain. Traumatic injury to the kidneys from an accident or fall also causes kidney pain.

Because kidney pain can indicate serious conditions, it's important to see a doctor when you experience it. You should seek immediate medical attention if you have sudden severe pain in your back or side, pain combined with fever, pain with nausea or vomiting, pain with blood in your urine, or pain that doesn't go away or keeps getting worse. These symptoms might indicate kidney stones, kidney infection, or another condition requiring urgent care. Chronic mild kidney pain should also be evaluated by a doctor, even if it doesn't feel emergency-level severe. Your doctor can perform tests like ultrasound or CT scan to see what's causing your pain.

It's worth noting that not all back or side pain is kidney-related. Muscle strain, arthritis, and other spine problems cause similar pain patterns. This is why getting a proper diagnosis from a healthcare provider is important. Your doctor will ask about your symptoms, examine you, and may
