Learn About Heart Palpitations and When to Seek Care

Understanding Heart Palpitations: What They Are and Why They Happen

Heart palpitations are sensations where you become aware of your heartbeat. Most people don't notice their heart beating during normal daily activities, but palpitations make that beating suddenly noticeable. You might feel your heart racing, skipping a beat, fluttering, or pounding in your chest, neck, or throat. These sensations can last from a few seconds to several minutes.

Your heart normally beats 60 to 100 times per minute when you're at rest. During palpitations, your heart may beat faster than normal, slower than normal, or with an irregular rhythm. The experience varies significantly from person to person. Some people describe palpitations as a gentle flutter, while others report a forceful pounding sensation that feels alarming.

Palpitations occur when your heart's electrical system—the network controlling when and how your heart contracts—becomes temporarily disrupted. This can happen for many reasons. Common triggers include caffeine consumption, stress, anxiety, strenuous exercise, fever, dehydration, or changes in blood pressure. For example, someone drinking three or four cups of coffee within a short period might experience palpitations for 30 minutes to an hour afterward. Similarly, a person undergoing a stressful situation at work might feel their heart racing as their body releases adrenaline.

It's important to understand that occasional palpitations are quite common. Research suggests that about 16 percent of people experience palpitations at some point in their lives. Many palpitations are harmless and resolve on their own without medical intervention. However, understanding what triggers your palpitations and recognizing patterns can help you manage them better.

Practical Takeaway: Keep a simple log noting when you experience palpitations, what you were doing beforehand, and how long they lasted. This information helps you identify personal triggers and provides valuable details if you need to discuss palpitations with a healthcare provider.

Common Causes of Heart Palpitations

Heart palpitations can stem from numerous causes, ranging from everyday substances to underlying medical conditions. Caffeine stands as one of the most common culprits. Caffeine is a stimulant that increases heart rate and can trigger palpitations in sensitive individuals. Beyond coffee, caffeine appears in tea, energy drinks, chocolate, and some medications. A person sensitive to caffeine might experience palpitations after consuming just one energy drink, while another might need several cups of coffee to notice any effect.

Anxiety and stress significantly contribute to palpitations. When you feel anxious or stressed, your body releases hormones like adrenaline that increase heart rate and force of contractions. Some people with anxiety disorders experience palpitations as a primary symptom. Interestingly, this can create a cycle: you feel palpitations, which causes anxiety about your heart, which triggers more palpitations. Breaking this cycle sometimes requires addressing the underlying anxiety rather than focusing solely on the heart sensation.

Physical triggers also commonly cause palpitations. Intense exercise, particularly sudden strenuous activity when you're not conditioned for it, can cause your heart to beat irregularly or very rapidly. Additionally, fever, dehydration, or low blood sugar can all trigger palpitations. For instance, someone with the flu who hasn't eaten properly and is running a temperature may experience multiple palpitation episodes throughout the day.

Hormonal changes play a significant role as well. Pregnancy often brings palpitations due to increased blood volume and hormonal shifts. Menopause and thyroid disorders can also cause palpitations. Women may notice palpitations worsen during particular phases of their menstrual cycle when hormone levels fluctuate.

Certain medications and substances can trigger palpitations too. Decongestants, asthma medications, thyroid medications (if the dose is too high), and stimulant medications can all potentially cause palpitations. Alcohol, particularly in excessive amounts, and tobacco use both increase palpitation risk. Some over-the-counter weight loss supplements containing stimulants frequently cause palpitations.

Practical Takeaway: Review your recent habits and circumstances when you experience palpitations. Look at caffeine intake, stress levels, sleep quality, recent illness, and any new medications or supplements. Reducing or eliminating triggers often decreases palpitation frequency without requiring medical treatment.

Types of Palpitations and What Different Patterns Mean

Palpitations manifest in different patterns, and recognizing these patterns provides information about what might be happening. Racing palpitations feel like your heart is beating much faster than normal—possibly 120 to 150 beats per minute or higher. These often occur with panic attacks, fever, hyperthyroidism, or intense exercise. Someone experiencing racing palpitations during a panic attack might feel their heart beating so fast they worry they're having a heart attack, even though the cause is anxiety.

Skipping or fluttering palpitations involve the sensation of your heart missing a beat or fluttering like a bird in your chest. These often reflect premature beats—contractions that occur earlier than expected in the normal heart rhythm. Premature atrial contractions (PACs) affect the heart's upper chambers, while premature ventricular contractions (PVCs) affect the lower chambers. These are generally benign and extremely common, occurring in most people occasionally. You might notice them more after caffeine consumption or during stress.

Pounding palpitations feel forceful, as though your heart is hitting hard against your ribs. These often happen with anxiety, exercise recovery, or when lying on your left side. Some people describe pounding palpitations as feeling like someone is thumping their chest from the inside. While frightening, pounding palpitations are often benign, particularly if they occur infrequently and resolve quickly.

Irregular rhythm palpitations involve your heart beating in an uneven pattern—beats come faster or slower, or in an unpredictable sequence. Conditions like atrial fibrillation, a common arrhythmia affecting 2-3 percent of the adult population, cause persistent irregular rhythms. Unlike occasional skipped beats, atrial fibrillation causes continuous irregularity that typically requires medical evaluation and treatment.

The duration and frequency of palpitations matter too. Palpitations lasting seconds to minutes that occur occasionally are typically benign. However, palpitations lasting hours, occurring daily, or accompanied by other symptoms warrant medical evaluation to rule out underlying conditions.

Practical Takeaway: When palpitations occur, note the pattern: Do they feel like racing, skipping, fluttering, or pounding? How long do they last? Do they occur regularly or sporadically? Describing these details to a healthcare provider helps them determine whether further evaluation is necessary.

When Heart Palpitations May Indicate a Serious Condition

While many palpitations are harmless, certain characteristics warrant medical evaluation. You should seek care if palpitations occur alongside chest pain or pressure, shortness of breath, dizziness, fainting, or severe weakness. These symptoms together might suggest a more serious heart rhythm problem or other cardiac issue. For example, if you experience racing palpitations combined with chest pain and shortness of breath, this warrants prompt medical attention even if the palpitations resolve.

Palpitations lasting more than a few minutes, particularly if they occur regularly, deserve medical evaluation. Someone experiencing palpitations for 15 minutes or longer, or someone having multiple palpitation episodes daily, should consult a healthcare provider. Similarly, if palpitations intensify or change in character—for instance, if they become more frequent or feel different than before—this change warrants attention.

People with known heart disease, high blood pressure, diabetes, or family history of heart problems should report any new palpitations to their healthcare provider promptly. These underlying conditions increase the significance of palpitations and make professional evaluation more important. Additionally, if palpitations cause significant anxiety or impact your daily functioning, medical evaluation provides reassurance or identifies treatable conditions.

Palpitations accompanied by fainting episodes particularly require evaluation. Fainting during palpitations can indicate dangerous heart rhythm problems like ventricular tachycardia. Conversely, some people faint from benign arrhythmias, and medical evaluation distinguishes between these possibilities. Anyone experiencing palpitations followed by loss of consciousness needs prompt medical attention.

Signs that palpitations might reflect a serious condition include: palpitations with a very rapid rate (
