Understanding POTS: What the Guide Covers

Postural Orthostatic Tachycardia Syndrome, commonly known as POTS, is a condition that affects how your body regulates blood pressure and heart rate. This guide provides information about what POTS is, how it develops, and the signs that may indicate you have this condition. According to the Dysautonomia International organization, POTS affects between 1 and 3 million Americans, yet many people go years without receiving a diagnosis because the symptoms can mimic other conditions.

The guide explains how POTS works in your body. When you stand up from sitting or lying down, your heart rate may increase abnormally—typically by 30 beats per minute or more within the first 10 minutes of standing, without a corresponding drop in blood pressure. This happens because your autonomic nervous system (the part of your nervous system that you cannot consciously control) has difficulty managing blood vessel constriction and heart rate adjustments during position changes.

POTS comes in several types, and the guide outlines the differences between them. Neuropathic POTS involves nerve fiber damage, hyperadrenergic POTS involves excessive adrenaline release, hypovolemic POTS relates to low blood volume, and secondary POTS develops as a result of another condition or illness. Understanding which type you may have matters because different presentations sometimes respond differently to various management strategies.

The guide also discusses how POTS develops. Some people develop POTS after viral infections, including COVID-19. Others experience POTS onset following surgery, pregnancy, or bed rest. Certain people are born with genetic factors that make them more likely to develop POTS. For example, research has shown that some cases cluster in families, suggesting hereditary factors play a role.

Practical Takeaway: Before pursuing diagnostic testing, understanding POTS basics helps you recognize whether your symptoms align with this condition. Take note of when your symptoms occur (during standing, after meals, during stress) and share these patterns with a healthcare provider during your visit.

Recognizing POTS Symptoms and When to See a Doctor

POTS symptoms vary widely from person to person, and the guide details the most common presentations. The hallmark symptom is excessive dizziness or lightheadedness when standing up. Many people describe feeling like they might faint, though complete loss of consciousness is less common. Other frequent symptoms include heart palpitations (feeling your heart racing or skipping), fatigue that doesn't improve with rest, brain fog or difficulty concentrating, shortness of breath, and chest discomfort.

Physical symptoms often accompany the main complaints. People with POTS frequently report tremors or shakiness, excessive sweating or inability to sweat normally, headaches, nausea, and exercise intolerance (becoming exhausted very quickly during physical activity). Some people experience blurred vision, particularly when standing. Others notice their symptoms worsen in heat, after prolonged sitting, during menstrual cycles, or after eating large meals. These patterns provide valuable information to discuss with a healthcare provider.

The guide emphasizes that POTS symptoms exist on a spectrum. Some people experience mild symptoms that only appear occasionally, while others face severe daily limitations. A person with mild POTS might feel dizzy after standing too quickly but otherwise function normally. Someone with severe POTS might struggle to shower, prepare meals, or attend school or work without significant difficulty. Symptom severity can also fluctuate over time or worsen during stressful periods.

Knowing when to contact a doctor matters for getting appropriate evaluation. You should consider reaching out to a healthcare provider if you regularly experience dizziness upon standing, particularly if it affects your daily activities; if your heart rate races frequently without obvious causes; if you have fainting episodes; if you feel persistently fatigued despite adequate sleep; or if multiple symptoms occur together. The guide explains that some people see their primary care doctor first, while others may need referral to a cardiologist, neurologist, or autonomic dysfunction specialist for proper diagnosis.

Practical Takeaway: Keep a symptom journal for two to four weeks before your appointment. Note the time of day, what you were doing, what you ate, where you were (hot room, cool room), and what symptoms appeared. This detailed information helps doctors assess whether POTS or another condition might explain your experiences.

The Diagnostic Process: Tests and What to Expect

The guide explains the various tests used to diagnose POTS and how each one works. The tilt table test stands as the gold standard for POTS diagnosis. During this test, you lie on a special table with heart rate and blood pressure monitoring equipment attached. The table gradually tilts upward to simulate standing, while medical staff observe your heart rate and blood pressure responses. A POTS diagnosis typically occurs when heart rate increases by 30 or more beats per minute within the first 10 minutes of tilting, without a significant blood pressure drop. The test usually lasts 20 to 40 minutes.

Before undergoing a tilt table test, doctors typically perform other evaluations. A basic blood panel checks for anemia, thyroid problems, blood sugar issues, and electrolyte imbalances—conditions that can cause similar symptoms. An electrocardiogram (ECG) records your heart's electrical activity and reveals any irregular rhythms. Some doctors order echocardiograms to visualize heart structure and function. These preliminary tests help rule out other conditions and ensure you're a suitable candidate for tilt table testing.

The guide describes the Holter monitor test, which records your heart rhythm continuously for 24 to 48 hours as you go about your normal day. This test shows how your heart responds to various activities and can reveal patterns in symptoms. Blood pressure monitoring over 24 hours similarly tracks your body's pressure responses throughout daily activities.

Home-based testing methods are also discussed. Some people use portable heart rate monitors or blood pressure cuffs at home to document how their heart rate changes when they stand. While not diagnostic on their own, these measurements provide useful information for healthcare providers. The guide explains that some doctors may suggest a poor man's tilt test—simply lying down and having heart rate and blood pressure taken, then standing and taking measurements again—as an informal screening tool.

Practical Takeaway: Before diagnostic testing, avoid caffeine for several hours beforehand, stay well-hydrated, eat a light meal, and wear comfortable clothing. Write down any questions for your healthcare provider and bring a list of all medications and supplements you take, as some substances affect heart rate and blood pressure readings.

Medical Conditions and Factors That May Mimic or Coincide with POTS

The guide addresses why POTS diagnosis often takes years—other conditions produce similar symptoms, and POTS can exist alongside other medical problems. Anxiety and panic disorder frequently get confused with POTS because both cause rapid heart rate, dizziness, and trembling. However, anxiety symptoms typically appear in response to stressful thoughts, while POTS symptoms consistently occur upon standing regardless of emotional state. Some people have both conditions simultaneously, which complicates diagnosis.

Thyroid dysfunction commonly causes heart palpitations, fatigue, and anxiety-like symptoms. Anemia reduces oxygen-carrying capacity, leading to dizziness and exhaustion. Diabetes and blood sugar dysregulation produce dizziness and lightheadedness. Medication side effects—particularly stimulants, some antidepressants, and decongestants—can trigger rapid heart rate and palpitations. The guide emphasizes that thorough evaluation with blood work helps distinguish these possibilities from POTS.

The guide discusses how POTS frequently coexists with other conditions. Ehlers-Danlos Syndrome (a connective tissue disorder) occurs alongside POTS in approximately 40 to 60 percent of POTS cases. Mast Cell Activation Syndrome involves immune system dysfunction and overlaps with POTS. Long COVID—the persistent symptoms some people experience after COVID-19 infection—frequently includes POTS-like symptoms. Myalgic Encephalomyelitis/Chronic Fatigue Syndrome shares significant overlap with POTS, particularly regarding exercise intolerance and orthostatic symptoms.

Understanding comorbid conditions matters because they affect treatment decisions. Someone with POTS and Ehlers-Danlos Syndrome may need different management than someone with isolated POTS. A person with POTS and mast cell activation syndrome requires attention to both autonomic dysfunction and immune system regulation. The guide
