Understanding What Menstrual Periods Are

A menstrual period is part of the body's monthly cycle, called the menstrual cycle. This cycle involves hormonal changes that prepare the uterus (womb) for pregnancy. If pregnancy doesn't occur, the body sheds the uterine lining through bleeding that typically lasts three to seven days. This bleeding is what people refer to as a period or menstruation.

The menstrual cycle repeats roughly every 28 days, though cycles can range anywhere from 21 to 35 days and still be considered normal. Each person's cycle length is different, and the same person may experience variation from month to month. Understanding your own pattern helps you recognize what's typical for your body.

During a period, the body releases hormones called estrogen and progesterone at different levels. These hormonal shifts trigger physical and emotional changes. The bleeding occurs because the endometrium—the tissue lining the uterus—breaks down and leaves the body through the vagina. Most people lose between 30 and 40 milliliters of blood during their period, though some may lose more or less.

Not everyone with periods experiences the same symptoms or has the same cycle length. Factors like age, stress, exercise, weight changes, and certain medical conditions can affect the menstrual cycle. Additionally, some people use birth control or other medications that change how their cycle works. This variation is normal and expected.

Practical Takeaway: Track the start and end dates of your periods for three to six months. Write down the date you start bleeding and when it stops. This personal record shows you your individual pattern and helps identify what's normal for your body.

Early Signs Your Period May Be Coming

Most people experience signs one to two weeks before bleeding actually starts. These signs are called premenstrual symptoms or PMS. Common early indicators include breast tenderness, bloating in the abdomen, and mild cramping. Some people also notice their breasts feel fuller or more sensitive to touch.

Mood changes often appear several days before a period begins. These may include irritability, sadness, anxiety, or increased emotional sensitivity. Many people feel more tired than usual and may have trouble concentrating. Sleep patterns sometimes change—some people sleep more, while others struggle to fall asleep or stay asleep.

Physical sensations can provide clues as well. Headaches, muscle aches, and food cravings are common experiences. Some people crave specific foods like chocolate, salty snacks, or carbohydrates. Others notice digestive changes such as constipation or diarrhea. These symptoms vary greatly between individuals and can differ from cycle to cycle in the same person.

Skin changes may also signal an approaching period. Some people develop acne breakouts or notice their skin looks oilier than usual. Water retention is another common sign—clothes may feel tighter, fingers or feet might swell slightly, and the number on the scale may increase by a few pounds due to fluid retention rather than weight gain.

A few people notice ovulation pain, called mittelschmerz, around the middle of their cycle (roughly 14 days before the period starts). This is a mild ache on one side of the lower abdomen where an ovary is releasing an egg. This sensation, though not a direct sign the period is coming, can help you understand your cycle timeline.

Practical Takeaway: Keep a simple log of how you feel in the week or two before your period. Note mood changes, physical sensations, cravings, and energy levels. After tracking two to three cycles, patterns will emerge showing which signs are most reliable for your body.

The Complete Menstrual Cycle Timeline

The menstrual cycle has four distinct phases, each with different hormonal activity and physical changes. Understanding this timeline helps you recognize where you are in your cycle at any given time.

Phase One: Menstruation (Days 1-5) Bleeding begins on day one of your cycle. During this phase, the uterine lining exits the body. Most people bleed for three to seven days. Blood flow is typically heaviest on days two and three. Cramping pain is common during this phase as the uterus contracts to help shed the lining. Pain usually decreases as bleeding slows.

Phase Two: Follicular Phase (Days 1-13) This phase overlaps with menstruation and continues after bleeding stops. The pituitary gland releases follicle-stimulating hormone (FSH), which signals the ovaries to produce estrogen. Estrogen levels rise, the uterine lining begins to thicken, and energy and mood typically improve. Many people feel their best during this phase. The phase ends when a surge in luteinizing hormone (LH) triggers ovulation.

Phase Three: Ovulation (Day 14) The ovary releases a mature egg. This typically happens around day 14 of a 28-day cycle, but timing varies. An egg can survive for 12 to 24 hours. If sperm meets the egg during this window, pregnancy can occur. Some people notice ovulation pain, increased sex drive, or a slight rise in body temperature during this phase.

Phase Four: Luteal Phase (Days 15-28) After ovulation, the empty ovarian follicle produces progesterone. This hormone prepares the uterus for a fertilized egg. If the egg isn't fertilized, progesterone levels eventually drop, signaling the body to begin menstruation again. This phase includes the time when PMS symptoms typically appear. The phase ends when bleeding starts and day one of a new cycle begins.

Practical Takeaway: Mark day one of your period on a calendar for the next three months. Then count forward to estimate when your next ovulation might occur (typically around the middle of your cycle) and when PMS symptoms may appear (usually in the last week before your next period).

Physical Symptoms During Your Period

Once bleeding begins, physical sensations are common. Cramping is perhaps the most frequent symptom. These cramps happen because the uterus contracts to help expel blood and tissue. Cramping typically starts just before or as bleeding begins and lessens over a few days. Mild cramping is normal and expected. The pain usually feels like aching in the lower abdomen, lower back, or both.

Bleeding flow varies throughout a period. Flow is usually heaviest on the first two days and gradually decreases. A normal period involves soaking one to seven pads or tampons per day, though this varies widely between individuals. Some people pass small clots, which is normal. Large clots or extremely heavy bleeding that soaks through protection hourly may warrant conversation with a healthcare provider to rule out any concerns.

Fatigue during a period is extremely common. The combination of blood loss, hormonal changes, and physical discomfort can make you feel more tired than usual. Some people need more sleep than normal, while others feel drained despite adequate rest. This is a temporary condition that improves as bleeding decreases.

Digestive issues often accompany periods. Diarrhea or constipation can occur as hormonal changes affect the digestive system. Nausea happens to some people, particularly on heavier flow days. Appetite may increase or decrease. Staying hydrated and eating balanced meals helps manage these symptoms.

Headaches and body aches frequently occur during menstruation. These may be related to hormonal shifts, dehydration, or muscle tension. Pain levels typically decrease as the cycle progresses. Some people experience migraines during their period. Others notice general muscle soreness similar to what follows physical exercise.

Practical Takeaway: For the next three to four periods, note which physical symptoms you experience, how intense they are on a scale of one to ten, and how many days they last. This record shows what's normal for your body and helps you plan for managing discomfort during future periods.

Emotional and Mental Changes Around Your Period

Hormonal shifts during the menstrual cycle affect brain chemistry and emotional well-being. In the days leading up to a period, progesterone levels drop, which can influence mood and mental clarity. Many people experience increased irritability, anxiety, or sadness in the week before their period starts. These emotional changes are biologically based and
