Understanding Breast Self-Examination: What You Should Know

Breast self-examination (BSE) is a method of checking your own breasts for changes in size, shape, texture, or appearance. This practice involves looking at and feeling your breasts on a regular basis to become familiar with how they normally look and feel. By knowing what is normal for your body, you can more easily notice if something changes and report it to a healthcare provider.

The American Cancer Society reports that about 1 in 8 women in the United States will develop invasive breast cancer during their lifetime. While self-examination is not a replacement for clinical breast exams or mammograms, learning your breast tissue through regular observation and touch can help you stay aware of your body. Research shows that women who perform self-examinations often develop a better understanding of their breast tissue characteristics, which can be valuable information to share with healthcare providers during routine check-ups.

Many women find that self-examination offers peace of mind and a sense of control over their health monitoring. The practice takes only a few minutes and requires no special equipment or preparation. It is something you can do in the privacy of your home as part of your regular health routine. Some women incorporate breast self-examination into their monthly self-care practices, while others check their breasts less frequently. The key is finding a routine that works for you and sticking with it consistently.

Understanding the normal characteristics of your own breasts is the foundation of effective self-examination. Breast tissue varies greatly from person to person. Some women have dense breast tissue, while others have more fatty tissue. Some breasts are naturally lumpy, and this lumpiness may change throughout the menstrual cycle. Hormonal fluctuations can cause breasts to feel tender, swollen, or bumpy at certain times of the month. By performing regular self-examinations, you learn what is typical for your body, making it easier to identify when something feels different.

Practical Takeaway: Start by scheduling a time each month to perform a breast self-examination. Mark it on your calendar—many women choose to do this a few days after their period ends, when breast swelling and tenderness are typically at their lowest. This consistency helps you remember to check regularly and makes it easier to notice any changes over time.

The Best Timing and Preparation for Self-Examination

Timing plays an important role in performing an effective breast self-examination. The menstrual cycle affects breast tissue, causing it to swell, become tender, or feel lumpy at different times of the month. For menstruating individuals, the best time to perform a self-examination is about seven to ten days after the start of your period. During this window, hormone levels are lower, breasts are typically less swollen and tender, and the tissue is easier to examine and evaluate.

If you are post-menopausal, no longer menstruating due to other reasons, or using hormonal birth control, the timing is less critical. In these cases, you can choose any day of the month that works for your schedule. The key is to pick the same day each month so that self-examination becomes part of your routine. Some women choose the first day of each month, while others select a date that is meaningful to them personally. Whatever day you select, consistency matters more than the specific date.

Preparation for self-examination is straightforward and requires minimal setup. You will need access to a mirror and a private space where you feel comfortable. Some women prefer to perform their self-examination in the bathroom after showering, as the skin is clean and the mirror is readily available. Others choose their bedroom. The location matters less than having adequate lighting and privacy. If you are uncomfortable performing self-examination alone initially, you may ask a trusted partner or healthcare provider to guide you through the process.

Before beginning, take a moment to relax and clear your mind. Self-examination should not feel rushed or stressful. If you are anxious about finding a lump or noticing changes, remind yourself that most breast changes are not cancer. Fibrocystic changes, hormonal fluctuations, and benign cysts are common and non-cancerous. Having a calm mindset helps you focus on what you are observing and reduces unnecessary worry. If you do notice something unusual, make a note of it and plan to discuss it with your healthcare provider at your next appointment.

Practical Takeaway: Set a calendar reminder on your phone for your chosen examination date each month. Include a note with the reminder that says "Perform breast self-examination." This simple step removes the burden of remembering and makes the practice automatic over time. The reminder takes just seconds to set up but can help you maintain consistency throughout the year.

Visual Inspection: What to Look For

Visual inspection is the first part of breast self-examination and involves carefully looking at your breasts in a mirror. This step helps you notice any changes in appearance, such as differences in size, shape, color, or skin texture. Start by standing in front of a mirror in a well-lit room, wearing no shirt or bra. Take a moment to observe your breasts in a relaxed position with your arms at your sides. Look at the overall shape, size, and skin color of each breast. Note whether the skin appears smooth or whether you see any dimpling, puckering, or bulging.

Next, raise your arms above your head and observe your breasts again. This position stretches the skin and chest muscles, which can make skin changes more visible. Look for the same features you noticed before: size, shape, symmetry, skin texture, and color. It is normal for breasts to be slightly different sizes or shapes from each other. Many women have one breast that is noticeably larger than the other, and this is completely normal. What matters is whether there is a new difference that was not present before.

After raising your arms, place your hands on your hips and look at your breasts once more. This position also tightens the chest muscles and can reveal skin changes. As you examine your breasts from these different angles, watch for specific signs that warrant discussion with a healthcare provider. These include dimpling or puckering of the skin, redness or unusual color changes, swelling or thickening of the skin, changes in the appearance of the nipple such as it turning inward or becoming inverted if it was previously outward, discharge from the nipple, or a rash around the nipple area.

Some women also notice their breasts appear different at various times of the month due to hormonal changes. Swelling before a period is common and typically resolves afterward. However, if you notice persistent redness, scaling, or a rash on the nipple or areola that does not go away after your period, this should be discussed with a healthcare provider. Similarly, nipple discharge that is spontaneous, occurs from one breast only, or is associated with blood or an unusual color may warrant medical evaluation.

Practical Takeaway: Take a photo of your breasts during your self-examination each month using your phone or camera. Store these images in a secure, private folder on your device. Over time, comparing photos from different months makes it easier to spot gradual changes in appearance that you might otherwise miss. This visual record provides helpful information to share with your healthcare provider if you have concerns.

Palpation Technique: How to Feel Your Breasts Correctly

Palpation, or the act of feeling your breasts with your hands, is the second major component of self-examination. This step allows you to check the texture of your breast tissue and notice any lumps, thickening, or other changes. To perform palpation, you can use one of two methods: the vertical strip method or the circular method. Both are effective; choose the one that feels more natural to you. The vertical strip method involves dividing each breast into vertical sections and systematically checking each section from top to bottom. The circular method involves moving your fingers in circular patterns around the breast, starting from the outer edge and moving toward the nipple.

Begin by lying on your back, as this position flattens breast tissue against the chest wall and makes it easier to feel changes. Place a pillow under your head for comfort. Start with your right breast. Using the pads of your fingers—not just your fingertips—begin examining the tissue. Apply gentle to moderate pressure as you move across your breast. You should be able to feel your rib cage beneath the breast tissue. Move slowly and deliberately, taking time to feel different areas rather than rushing through the examination. Many women find it helpful to use the opposite hand; for example, use your left hand to examine your right breast.
