Understanding Your MCV Blood Test Results

What Is MCV and Why Your Doctor Ordered This Test

MCV stands for "mean corpuscular volume." This is a measurement that tells your doctor the average size of your red blood cells. Red blood cells are the cells in your blood that carry oxygen throughout your body. Think of them like tiny delivery trucks — they pick up oxygen in your lungs and drop it off to every part of your body that needs it.

Your doctor might order an MCV test as part of a routine blood work panel, or because you're experiencing symptoms like fatigue, shortness of breath, or weakness. The test is simple — a lab technician takes a small blood sample from your arm, usually just a few milliliters. The sample goes into a machine called a hematology analyzer, which counts your red blood cells and measures their size. The results come back within hours or days, depending on your healthcare facility.

MCV measurements are reported in units called femtoliters (fL). A femtoliter is an extremely tiny unit of volume — one femtoliter is one millionth of a millionth of a liter. Despite the tiny scale, this measurement matters because the size of your red blood cells can indicate different health conditions. When red blood cells are too large or too small, it often means something is affecting how your body makes these cells or how long they survive.

The MCV test is part of a larger blood test called a complete blood count (CBC). This CBC gives doctors information about three main types of blood cells: red blood cells, white blood cells, and platelets. While other parts of the CBC measure how many cells you have, the MCV specifically measures how big those red blood cells are. This is why the MCV result is so useful — it gives doctors clues about what might be causing problems with your blood cells.

Practical Takeaway: Understanding that MCV measures red blood cell size helps you understand why your doctor cares about this number. When you get your results, remember that the MCV is just one piece of information. Your doctor looks at the complete picture, including your other blood test results, your symptoms, and your medical history.

Understanding Normal MCV Ranges and What They Mean

A normal MCV range for adults typically falls between 80 and 100 femtoliters, though this can vary slightly depending on the laboratory performing the test. Different labs may have slightly different reference ranges based on their equipment and the population they serve. When you get your results, your lab report should include the normal range that lab uses, so you can see exactly where your number falls.

If your MCV is within the normal range, it means your red blood cells are roughly the right size for healthy function. This is generally a good sign, though it doesn't mean everything about your blood is perfect — other parts of your CBC or other tests might still show issues. Many people with normal MCV values have completely healthy blood and blood cell production.

The reason doctors care so much about cell size relates to how blood cells are made. Your body produces red blood cells in your bone marrow through a process that takes about 7 to 10 days. This process requires several nutrients, particularly vitamin B12, folate, and iron. When something goes wrong with this process — whether it's a nutritional deficiency, a genetic condition, or a disease affecting the bone marrow — it often shows up as a change in MCV before it causes serious symptoms.

Think of normal MCV like a Goldilocks situation. Too small is a problem. Too large is a problem. Just right is what your doctor wants to see. The body is very precise about cell size because cells that are too small can't carry as much oxygen, and cells that are too large don't move through small blood vessels as easily. Both situations create stress on your body and your heart.

When you receive your results, your healthcare provider should explain what your specific number means. If your MCV is 87, for example, that's comfortably in the normal range. If it's 75, it's low but not drastically so. If it's 110, it's elevated but not at the extreme end. Context matters — your doctor considers your age, sex, whether you take certain medications, and whether you have any known health conditions.

Practical Takeaway: Keep your lab report in a safe place where you can refer back to it. Write down your MCV number and the normal range your lab uses. If you have future blood tests, you can compare your numbers over time to see if there's a pattern. Stable normal results over time suggest your blood cell production is working well.

Low MCV: When Red Blood Cells Are Too Small

When your MCV is below 80 femtoliters, doctors call this microcytosis, which simply means "small cells." A low MCV is one of the most common blood abnormalities that doctors encounter. According to medical literature, iron deficiency is responsible for about 50 to 75 percent of all cases of low MCV in developed countries. This makes iron deficiency the most frequent cause your doctor will investigate.

Iron is essential for making hemoglobin, the protein inside red blood cells that carries oxygen. When your body doesn't have enough iron, it can't make normal-sized red blood cells. Instead, it makes smaller cells that contain less hemoglobin. Your bone marrow keeps trying to produce enough red blood cells to meet your body's oxygen needs, but since each cell is smaller, you end up with many tiny cells instead of the right number of normal-sized cells.

Iron deficiency can develop for several reasons. In women of childbearing age, heavy menstrual bleeding is a common cause. In older adults, bleeding in the digestive tract — which might be visible or hidden — accounts for many cases. Vegetarians and vegans may develop iron deficiency if they don't consume enough plant-based iron sources or if they have difficulty absorbing iron. People who have had gastric surgery, or who have digestive conditions like celiac disease or Crohn's disease, may struggle to absorb iron properly. Blood donors who donate frequently without adequate iron supplementation can also develop low MCV.

Other conditions besides iron deficiency cause low MCV. Thalassemia, an inherited blood disorder, causes the body to make abnormal hemoglobin, which results in small red blood cells. Sideroblastic anemia, where the body can't properly use iron to make hemoglobin, also causes low MCV. Lead poisoning, which is rare in adults but can affect children exposed to lead paint or contaminated water, interferes with red blood cell production and causes low MCV. Chronic kidney disease reduces the production of erythropoietin, a hormone that stimulates red blood cell production, leading to anemia that is often microcytic.

Common symptoms associated with low MCV include fatigue, shortness of breath, dizziness, pale skin, and cold hands and feet. However, mild cases may cause no symptoms at all. You might feel fine while your MCV is slightly low, especially if your body has adapted to the gradual change. This is why routine blood work can be valuable — it catches problems that might not cause noticeable symptoms yet.

Practical Takeaway: If your MCV is low, note how much below normal it is. A reading of 75 fL is concerning, but a reading of 70 fL is more serious. Track any symptoms you've experienced, such as unusual tiredness or shortness of breath, and share this with your doctor. Your physician will likely order additional tests to determine the cause, such as iron studies (serum iron, ferritin, and TIBC) or a peripheral blood smear to examine your cells under a microscope.

High MCV: When Red Blood Cells Are Too Large

When your MCV is above 100 femtoliters, doctors call this macrocytosis, which means "large cells." High MCV is less common than low MCV, but it indicates important health issues that need attention. The most common cause of high MCV is vitamin B12 deficiency, which accounts for many cases. Folate deficiency is another major nutritional cause. Together, these two deficiencies account for a substantial portion of high MCV cases.

Vitamin B12 plays a crucial role in the process of cell division and DNA synthesis. When you don't have enough B12, your bone marrow can't divide cells properly during red blood cell production. The cells that are produced tend to be larger than normal, and there are fewer of them. This creates a situation where you have fewer red blood cells, and the ones you
