Understanding Vertigo: Causes and Medical Care Guide

What Is Vertigo and How Does It Differ From Dizziness

Vertigo is a specific type of dizziness where you feel like you or the room around you is spinning. Many people use the word "dizzy" to describe many different sensations, but vertigo has a distinct characteristic: the spinning sensation. When you have vertigo, you might feel like the floor is tilting beneath your feet, the walls are rotating around you, or you're on a merry-go-round that won't stop. This spinning feeling can range from mild to severe and can make it difficult to walk, stand, or even keep your eyes open.

The difference between vertigo and general dizziness matters for understanding what might be causing your symptoms. Dizziness is a broad term that includes lightheadedness, feeling faint, or a sense of being unsteady. You might feel dizzy if you stand up too quickly after lying down, if you're dehydrated, or if your blood pressure drops. Vertigo, on the other hand, always involves that spinning sensation and typically means something specific is happening with your balance system.

Your body maintains balance through a complex system involving your inner ear, eyes, and muscles. The inner ear contains tiny structures filled with fluid and sensors that detect head movement and position. When these structures send incorrect signals to your brain, or when there's a mismatch between what your eyes see and what your inner ear senses, vertigo can occur. For example, if you're on a boat and your inner ear senses movement while your eyes see a stable cabin, the conflicting signals can trigger vertigo and nausea.

Vertigo can be categorized into two main types based on where the problem originates. Peripheral vertigo comes from problems in the inner ear or the nerve that connects the inner ear to the brain. Central vertigo results from problems in the brain itself, including the brainstem or cerebellum. Peripheral vertigo accounts for about 80 percent of vertigo cases and is usually less serious than central vertigo, though both types require medical attention to determine the cause.

Practical Takeaway: If you experience spinning sensations, note whether the room appears to spin or if you feel like you're spinning. Describe this spinning feeling to your doctor, as this information helps them distinguish vertigo from other types of dizziness and narrows down potential causes.

Common Causes of Vertigo

Benign Paroxysmal Positional Vertigo (BPPV) is the most common cause of vertigo, responsible for about 50 percent of all vertigo cases in people over age 60. BPPV occurs when tiny calcium carbonate crystals, called otoliths or "ear rocks," become loose inside the inner ear. These crystals normally sit in a pouch called the utricle and help your body sense gravity and movement. When they break free and drift into the semicircular canals—the tubes that detect head rotation—they interfere with the fluid movement that sends balance signals to your brain. This typically happens after head trauma, but can also develop without any clear cause. BPPV causes brief episodes of intense spinning, usually lasting less than a minute, especially when you roll over in bed, tilt your head back, or bend down.

Vestibular neuritis is inflammation of the vestibular nerve, which carries balance information from the inner ear to the brain. This condition often develops after a viral infection and can cause severe vertigo that comes on suddenly and may last for days or weeks. People with vestibular neuritis often experience nausea, vomiting, and difficulty with balance during the acute phase. The inflammation typically improves on its own over time as your body recovers, though some people experience lingering balance problems.

Ménière's disease is a chronic condition affecting the inner ear that causes a combination of symptoms: vertigo, hearing loss, tinnitus (ringing in the ears), and a feeling of fullness in the affected ear. The condition is believed to result from a buildup of fluid in the inner ear, though the exact cause remains unclear. Ménière's disease typically affects one ear and produces episodes of severe vertigo that can last from minutes to hours. Between episodes, people may feel fine, making this an unpredictable condition that can significantly impact daily life and work.

Other causes of vertigo include migraines (vestibular migraines can trigger vertigo with or without a headache), head injuries, stroke or other brain conditions, certain medications, and infections of the inner ear. Some medications that can cause vertigo include aminoglycosides (a type of antibiotic), diuretics, and aspirin. In some cases, vertigo may develop without a clear identifiable cause, or it may result from multiple factors working together. This is why consulting with a healthcare provider is important—they can evaluate your specific symptoms and medical history to identify the underlying cause.

Practical Takeaway: Keep a symptom diary noting when vertigo occurs, how long it lasts, what activities trigger it, and any accompanying symptoms like hearing changes or nausea. This information helps your doctor identify which cause is most likely and guides treatment decisions.

Recognizing Symptoms and When to Seek Medical Care

Vertigo symptoms vary in intensity and duration depending on the underlying cause. The primary symptom is always the spinning sensation, but this can feel different for different people. Some describe it as the room spinning around them, while others feel like they are spinning. The sensation can be mild enough that you can continue with daily activities, or severe enough that you cannot stand or walk without falling. Many people with vertigo also experience nausea and vomiting, which can last longer than the spinning sensation itself. Some people feel nauseated for days even after the vertigo resolves, making it difficult to eat or maintain normal activities.

Associated symptoms often accompany vertigo episodes. These may include unsteadiness or difficulty walking, involuntary eye movements (called nystagmus) where your eyes move back and forth rapidly, sensitivity to light, difficulty concentrating, and a sense of unease or anxiety about moving. Some people experience symptoms that come and go, while others have continuous symptoms that gradually improve over time. The pattern of symptoms can provide clues about the cause—for example, BPPV typically causes short episodes triggered by specific head positions, while vestibular neuritis usually causes persistent symptoms that gradually improve.

Certain situations warrant contacting a healthcare provider urgently. You should seek immediate medical attention if vertigo is accompanied by severe headache, chest pain, difficulty speaking or understanding speech, facial drooping, weakness or numbness, difficulty walking or loss of coordination, vision changes, or high fever. These symptoms could indicate stroke, brain infection, or other serious conditions affecting the central nervous system. Additionally, if vertigo is accompanied by hearing loss, ear pain, or drainage from the ear, medical evaluation is important to rule out ear infection or other inner ear problems.

Even if your symptoms seem mild, you should schedule an appointment with a healthcare provider if vertigo persists for more than a few days, if episodes are frequent and affecting your ability to work or function, if you've had a head injury recently, or if this is your first experience with vertigo. Your primary care doctor can perform an initial evaluation or refer you to a specialist like an otolaryngologist (ear, nose, and throat doctor) or neurologist. Keeping track of when symptoms occur, what triggers them, and how long they last gives your doctor valuable information for diagnosis.

Practical Takeaway: Create a simple list of concerning symptoms that warrant urgent care (severe headache, weakness, difficulty speaking) and less urgent symptoms that still need evaluation (persistent mild spinning, hearing changes). Know the difference so you can respond appropriately and seek the right level of care.

Medical Diagnosis and Testing for Vertigo

When you visit a healthcare provider about vertigo, they will begin with a detailed history of your symptoms. They'll ask when the vertigo started, how often it occurs, what it feels like, how long episodes last, and what triggers the episodes if any are obvious. They'll also ask about recent illnesses, head injuries, medications you take, and whether you experience hearing changes, tinnitus, or ear fullness. This history is crucial because the pattern of symptoms often points toward a specific cause. For instance, vertigo triggered by rolling over in bed suggests BPPV, while vertigo following a viral illness suggests vestibular neuritis.
