Understanding TB Testing and Your Test Results

What Is TB Testing and Why It Matters

Tuberculosis (TB) is a serious infectious disease caused by bacteria called Mycobacterium tuberculosis. This disease primarily affects the lungs, though it can spread to other parts of the body like the kidneys, spine, and brain. TB spreads through the air when an infected person coughs, sneezes, or speaks, making it a public health concern in many communities.

According to the World Health Organization, approximately 10 million people develop active TB each year worldwide, with about 1.6 million deaths annually. In the United States, the Centers for Disease Control and Prevention reported about 7,400 TB cases in 2022. While these numbers may seem alarming, TB is preventable and treatable with proper medical care and medication.

TB testing is critical because there are two forms of the disease: latent TB infection and active TB disease. A person with latent TB infection has the bacteria in their body but doesn't feel sick and cannot spread the disease to others. However, without treatment, latent TB can progress to active TB disease, which is serious and contagious. Understanding your TB test results helps you and your healthcare provider determine your status and what steps to take next.

Testing is particularly important for certain populations. People who work in healthcare, live in crowded settings, have weakened immune systems, or have close contact with someone who has TB should consider regular testing. Recent immigrants from countries where TB is common may also benefit from testing.

Practical Takeaway: TB testing is a straightforward way to learn your status regarding this serious but treatable disease. Knowing whether you have latent or active TB helps guide treatment decisions.

Types of TB Tests Available

Healthcare providers use two main types of tests to detect TB: the tuberculin skin test (TST) and interferon-gamma release assays (IGRAs). Each test works differently and has specific uses depending on your situation.

The tuberculin skin test, also called the Mantoux test, involves injecting a small amount of purified protein derivative (PPD) under the skin on your forearm. A healthcare worker injects this substance intradermally, creating a small bump on your skin. You return to the clinic 48 to 72 hours later so a nurse can measure the size of any reaction. The size of the raised area, measured in millimeters, helps determine your TB status. This test has been used for over a century and remains widely available at clinics, hospitals, and health departments.

Interferon-gamma release assays are blood tests that detect the immune response to TB antigens. Common IGRAs include the QuantiFERON Gold test and the T-SPOT.TB test. These tests measure how your immune cells respond when exposed to TB proteins in a laboratory setting. Results typically come back within 24 to 48 hours. IGRAs are increasingly preferred in clinical settings because they require only one visit, have fewer false positives from BCG vaccination, and are more specific for TB infection.

Chest X-rays are often used alongside these tests, particularly when active TB disease is suspected. An X-ray can show characteristic patterns in the lungs that suggest active TB, such as cavitary lesions or infiltrates in the upper lobes. For people with active TB symptoms like persistent cough, fever, and night sweats, additional tests like sputum smear microscopy or TB culture tests may be performed to confirm the diagnosis and check for drug-resistant TB.

Each test has advantages and limitations. The TST is inexpensive and widely available but requires two clinic visits and can be affected by prior BCG vaccination. IGRAs offer a single-visit option and are more specific but may be more expensive. Your healthcare provider will recommend the most appropriate test based on your risk factors, medical history, and local guidelines.

Practical Takeaway: Understanding the differences between TST and IGRA tests helps you know what to expect during testing and why your provider chose a particular test for your situation.

Understanding Your Test Results

TB test results fall into several categories: negative, positive, or indeterminate (for certain tests). The meaning of your result depends on which test you received and your individual circumstances.

A negative TST result means you have no TB infection. The skin reaction measures less than 5 millimeters at 48 to 72 hours. However, the threshold for what counts as positive varies. A reaction of 5 millimeters or larger is considered positive for people with HIV infection, those in close contact with TB patients, or those with chest X-ray findings suggestive of TB. For healthcare workers and other at-risk groups, 10 millimeters or larger is considered positive. For the general population with no special risk factors, 15 millimeters or larger is considered positive. This variation exists because people at higher risk need earlier detection.

A positive TST result means your immune system reacted to the TB antigen, suggesting TB infection. However, a positive test doesn't automatically mean you have active TB disease. It may indicate latent TB infection. The test cannot distinguish between the two, which is why additional testing becomes necessary.

For IGRA blood tests, results are typically reported as positive, negative, or indeterminate. A positive result suggests TB infection. A negative result suggests you do not have TB infection. An indeterminate result means the test didn't produce a clear response and may need to be repeated.

Important factors that affect test interpretation include prior BCG vaccination, which can cause a false positive TST result years after vaccination. IGRAs are not affected by BCG vaccination. Certain medical conditions, medications, or recent infections can affect results. People who are severely immunocompromised, such as those with advanced HIV disease, may have falsely negative results because their immune systems cannot mount an adequate response.

If you have a positive test result, your healthcare provider will evaluate you further. This typically includes reviewing your medical history, assessing TB symptoms, and obtaining a chest X-ray. Blood tests may also be performed to check your overall health and assess for any contraindications to TB medication.

Practical Takeaway: A positive test means TB infection is likely present, but it doesn't diagnose active TB disease. A negative test suggests TB infection is unlikely, but the timing of your test matters—testing too soon after exposure may yield false negative results.

Latent TB Infection Versus Active TB Disease

Understanding the difference between latent and active TB is essential for interpreting your results and understanding treatment options. These are fundamentally different conditions requiring different management approaches.

Latent TB infection occurs when TB bacteria are in your body but not actively multiplying. People with latent TB feel healthy, have no symptoms, and cannot spread TB to others. Their chest X-rays typically appear normal, and they test negative on sputum tests. According to the CDC, approximately 13 million people in the United States have latent TB infection. The bacteria can remain dormant for years or even a lifetime without causing illness.

However, the risk of progression is real. Without treatment, approximately 5 to 10 percent of people with latent TB infection will develop active TB disease during their lifetime. The risk is higher in the first two years after infection. People with weakened immune systems, including those with HIV infection, diabetes, kidney disease, or those taking immunosuppressive medications, have much higher progression rates.

Active TB disease occurs when TB bacteria multiply in the body and cause illness. Common symptoms include a persistent cough lasting three or more weeks, chest pain, coughing up blood or sputum, fever, chills, and night sweats. Weight loss and fatigue are also frequent signs. People with active TB disease are contagious and can spread the infection to others through respiratory droplets.

Treatment differs significantly between the two conditions. Latent TB infection is treated with preventive therapy, typically using one or more TB medications taken for several weeks to months. This prevents the bacteria from multiplying and progresses to active disease. Common regimens include isoniazid monotherapy for nine months or shorter courses with multiple drugs. Treatment of latent TB is recommended for people at high risk of progression.

Active TB disease requires more intensive treatment. The standard treatment regimen includes four medications—isoniazid, rifampin, pyrazinamide, and ethambutol—taken together for two months, followed by isoniazid and rifampin for four additional months. Treatment must be completed fully to prevent drug-resistant TB and ensure cure.
