Understanding Plaque Buildup and Artery Health

What Is Plaque and How Does It Form in Your Arteries

Plaque is a sticky substance that builds up inside your arteries over time. It's made up of cholesterol, fat, calcium, and other materials from your blood. When plaque accumulates on artery walls, the condition is called atherosclerosis, or hardening of the arteries.

The process of plaque formation starts when your arteries develop tiny tears or damage in their inner lining. This can happen for several reasons, including high blood pressure, smoking, or high cholesterol levels. Once these small injuries occur, your body's repair system kicks in, but not always in a healthy way.

Low-density lipoprotein (LDL) cholesterol—often called "bad" cholesterol—begins to collect at these damaged spots. Your immune system responds by sending white blood cells to the area, which try to clean up the cholesterol but end up getting stuck themselves. Over months and years, more cholesterol, white blood cells, and other substances accumulate, forming a plaque deposit. This buildup can be soft and unstable, or it can harden with calcium deposits.

According to the American Heart Association, plaque typically starts forming in childhood and progresses slowly throughout adulthood. By age 40, about half of Americans have some plaque buildup in their arteries, even if they don't have symptoms. The rate and extent of buildup vary significantly based on lifestyle factors and genetics.

The plaque doesn't usually cause problems until it narrows the artery significantly—typically when it blocks 70% or more of the artery's opening. However, even smaller plaques can become dangerous if they rupture, causing a blood clot that suddenly blocks blood flow.

Practical Takeaway: Plaque forms gradually and silently over decades. Understanding this slow process helps explain why prevention and monitoring are important, even when you feel fine. Regular check-ups and risk assessments can help detect problems before they become serious.

Risk Factors That Increase Plaque Buildup

Multiple factors influence how quickly plaque accumulates in your arteries. Some of these are things you can control, while others are determined by your genetics or age. Understanding which factors apply to you can help guide decisions about lifestyle and medical care.

High cholesterol is one of the primary drivers of plaque buildup. When your LDL cholesterol is elevated, more cholesterol circulates in your blood and deposits in artery walls. The American Heart Association recommends that LDL cholesterol be below 100 mg/dL for most people. People with existing heart disease should aim for below 70 mg/dL. Many Americans have LDL levels higher than recommended—about 71 million American adults have high total cholesterol.

High blood pressure damages artery walls by putting excessive force on them over time. This repeated injury creates the small tears where plaque begins to form. A blood pressure reading of 130/80 mmHg or higher is considered elevated. Unlike high cholesterol, high blood pressure typically has no symptoms, which is why it's sometimes called the "silent killer."

Smoking is particularly damaging to artery health. Chemicals in tobacco smoke injure the artery lining, reduce oxygen in the blood, and increase blood clotting. Smokers have two to four times higher risk of heart disease compared to non-smokers. Even secondhand smoke exposure increases risk.

Diabetes dramatically accelerates plaque formation. High blood sugar damages artery walls and increases inflammation throughout the body. People with diabetes are two to four times more likely to develop heart disease than those without diabetes. About 37 million Americans have diabetes, and many don't realize the significant impact it has on artery health.

Other important risk factors include:

• Family history of early heart disease (before age 55 for men, before age 65 for women)
• Age (risk increases after 45 for men and 55 for women)
• Obesity and being overweight
• Physical inactivity
• Poor diet high in saturated fats and processed foods
• Chronic stress and depression
• Sleep problems and sleep apnea
• Chronic inflammation from conditions like rheumatoid arthritis

Many people have multiple risk factors working together, which multiplies their overall risk rather than simply adding it. For example, someone with high cholesterol, high blood pressure, and who smokes faces a much higher risk than someone with just one of these factors.

Practical Takeaway: Review the risk factors listed above and identify which ones apply to you. Those you can control—like smoking, diet, exercise, and weight—are good starting points for making changes. Discuss your personal risk profile with your doctor to determine what monitoring or treatment may be appropriate.

How Plaque Narrows Arteries and Restricts Blood Flow

As plaque accumulates on artery walls, it gradually narrows the space through which blood can flow. This narrowing process happens slowly, often over many years without causing noticeable symptoms. Understanding how this progression works can help explain why people sometimes have heart attacks or strokes without warning.

In a healthy artery, blood flows freely through a wide, open channel. The inner wall, called the endothelium, is smooth and flexible. As plaque builds up, it reduces the diameter of this channel. When plaque blocks less than 50% of an artery's opening, blood flow is usually not significantly impaired during rest. However, during physical activity when your heart needs to pump more blood, the narrowed artery can't deliver enough oxygen-rich blood to meet the demand.

This limitation during exertion causes angina—chest pain or pressure that typically occurs with activity and improves with rest. Angina affects about 9 million Americans. It's an important warning sign that plaque buildup is restricting blood flow. Some people describe angina as pressure, squeezing, heaviness, or tightness in the chest. It can also cause shortness of breath, fatigue, or pain in the arm, neck, or jaw.

Plaque deposits can form in multiple arteries throughout your body. Major arteries affected include the coronary arteries (which supply the heart), carotid arteries (which supply the brain), and peripheral arteries (which supply the legs and other areas). A person might have significant narrowing in one artery with minimal plaque in others, or plaque can be distributed across several arteries.

A critical danger occurs when stable plaque ruptures. The rupture exposes the inner contents of the plaque, triggering rapid blood clot formation. This clot can suddenly block blood flow through the artery completely. If this happens in a coronary artery, it causes a heart attack. If it happens in a carotid artery, it causes a stroke. The amount of plaque blocking the artery before rupture doesn't predict whether this will happen—sometimes a smaller plaque ruptures while a larger one remains stable.

Doctors use several tests to measure how much an artery is narrowed. A coronary angiogram involves inserting a thin tube into an artery and injecting dye to visualize blockages using X-rays. A noninvasive stress test checks how your heart responds to increased activity. CT or MRI imaging can show plaque buildup without injecting dye. The degree of narrowing helps determine treatment options.

Practical Takeaway: Chest pain, pressure, or shortness of breath during activity is a signal that something is restricting blood flow and warrants immediate medical evaluation. Don't assume chest discomfort is indigestion or anxiety without getting it checked. Call 911 or go to an emergency room if you experience chest symptoms that don't improve with rest, or if symptoms are new or more severe than usual.

How to Monitor and Detect Plaque Buildup

Because plaque buildup typically causes no symptoms in early stages, detection usually requires medical testing. Several tools are available to measure plaque, ranging from simple blood tests to advanced imaging. Knowing what's available can help you make informed decisions about screening with your healthcare provider.
