Understanding Ovarian Cancer Screening and Symptoms

What Is Ovarian Cancer and Who Should Know About It

Ovarian cancer develops when malignant cells form in the ovaries, the small organs in a woman's reproductive system that produce eggs and hormones. According to the National Cancer Institute, approximately 19,000 women receive an ovarian cancer diagnosis each year in the United States, and about 12,000 deaths occur annually from this disease. Understanding ovarian cancer matters because early detection can significantly impact treatment options and outcomes.

The ovaries are part of the female reproductive system, located on either side of the uterus. Cancer can start in different parts of the ovary, and different types grow at different rates. The most common type is epithelial ovarian cancer, which accounts for about 90 percent of cases. Other types include germ cell tumors and stromal tumors, which are less common but may have different characteristics and treatment approaches.

Ovarian cancer risk varies among women based on several factors. Age plays a role—most cases occur in women over 50, though younger women can develop this cancer. Family history matters significantly; women with close relatives (mother, sister, or daughter) who had ovarian or breast cancer face higher risk. Genetic mutations, particularly in the BRCA1 and BRCA2 genes, substantially increase risk. Other factors include never having been pregnant, endometriosis, obesity, and the use of certain hormone therapies.

Women without obvious risk factors can still develop ovarian cancer, which is why knowing the signs and symptoms matters for all women. Regular conversations with healthcare providers about personal and family health history help inform individual risk assessment. Women with significant family history or genetic concerns may discuss genetic testing options with their doctors.

Practical takeaway: Gather information about your family's medical history, including any cancers in female relatives. Share this information with your healthcare provider during routine visits so they can discuss your individual risk level and any screening or monitoring approaches that might be appropriate for you.

Recognizing Common Symptoms and Warning Signs

Ovarian cancer symptoms often appear vague and may mimic other common conditions, which can delay diagnosis. Women should be aware of persistent symptoms that last more than two weeks, as this duration warrants medical evaluation. According to the American Cancer Society, common symptoms include abdominal bloating, pelvic or abdominal pain, difficulty eating or feeling full quickly, and urinary symptoms like urgency or frequency.

Abdominal bloating represents one of the most frequently reported symptoms. Women might notice their abdomen feels swollen or tight, and their clothes fit differently. This bloating persists despite dietary changes and doesn't resolve with typical remedies. Some women describe a sensation of heaviness in the pelvic area. Pelvic or abdominal pain might feel like a dull ache or sharp discomfort and may come and go or remain constant.

Gastrointestinal symptoms frequently occur. A woman might feel unusually full after eating only a small amount of food. Some experience changes in bowel habits, including constipation or diarrhea. Nausea and loss of appetite can develop. These digestive symptoms sometimes lead to initial misdiagnosis as irritable bowel syndrome, food sensitivities, or other digestive disorders, potentially delaying cancer detection.

Urinary changes warrant attention. Women might notice increased urinary frequency or urgency, particularly at night. Some experience lower back pain. Vaginal bleeding that differs from normal menstrual periods can occur, though this symptom is less common. Fatigue and low energy levels may accompany other symptoms. Pain during sexual intercourse (dyspareunia) occasionally occurs.

It's important to recognize that these symptoms appear in many common conditions unrelated to cancer. Bloating, urinary frequency, and digestive changes frequently result from infections, hormonal changes, dietary factors, or other benign conditions. However, when symptoms persist for several weeks without explanation, medical evaluation becomes important. Women experiencing symptoms should contact their healthcare provider and describe what they've noticed, how long symptoms have lasted, and any changes they've observed.

Practical takeaway: Keep a brief record of any persistent symptoms—what they are, when they started, and how often they occur. This information helps your healthcare provider understand what you're experiencing. Schedule a visit if symptoms continue beyond two weeks, and be specific about what you've noticed.

Current Screening Practices and Medical Recommendations

Unlike some cancers, ovarian cancer lacks a routine screening test recommended for all women without symptoms. The American Cancer Society and the U.S. Preventive Services Task Force do not recommend regular screening for ovarian cancer in average-risk women because available tests haven't been shown to reduce death rates. This differs from cervical cancer screening, which has proven effective and is widely recommended.

The two main tools sometimes discussed for ovarian cancer screening are the CA-125 blood test and transvaginal ultrasound. CA-125 measures a protein that may be elevated in ovarian cancer, but this protein also increases with many non-cancerous conditions, including menstruation, endometriosis, and various infections. Transvaginal ultrasound creates images of the ovaries using an ultrasound probe inserted into the vagina. While these tests can detect some cancers, research shows they haven't reduced ovarian cancer deaths in screened populations and can lead to false alarms requiring follow-up procedures.

Medical organizations currently recommend that women at high risk for ovarian cancer—particularly those with BRCA gene mutations or strong family histories—discuss screening options individually with their healthcare providers. Some high-risk women might choose to undergo regular CA-125 testing and ultrasound, even though these tests haven't proven to reduce mortality. Others might discuss the option of risk-reducing surgery, such as removing the ovaries and fallopian tubes after childbearing years are complete, which significantly reduces ovarian cancer risk.

Research continues into better screening methods. Scientists are studying multimarker blood tests that measure several proteins together, hoping to improve detection accuracy. Clinical trials are examining whether new screening approaches might help identify cancers earlier in high-risk populations. Women interested in participating in cancer screening research studies can explore opportunities through the National Cancer Institute or their healthcare provider.

Women of average risk benefit most from knowing their body and reporting any unusual or persistent symptoms to their healthcare provider promptly. Maintaining open communication with doctors about family medical history and any health changes represents the most practical approach to early detection currently available.

Practical takeaway: Understand that routine screening isn't recommended for average-risk women. Instead, know your family history and discuss it with your doctor. If you have significant risk factors, ask about whether screening or other monitoring might be appropriate for your situation. Report any persistent symptoms promptly rather than waiting for a screening test to find them.

Understanding Risk Factors and Genetic Considerations

Several factors increase the likelihood that a woman will develop ovarian cancer, though having risk factors doesn't guarantee diagnosis. Age represents a significant factor—ovarian cancer occurs most frequently in women over 50, with average age at diagnosis around 63. About 10 percent of cases occur in women under 40, and some occur in younger women.

Family history substantially increases risk. Women with one close relative who had ovarian cancer have approximately 5 percent lifetime risk. This increases to 7 percent with two affected relatives. The BRCA1 and BRCA2 gene mutations carry the highest individual risk. Women carrying BRCA1 mutations face approximately 35 to 70 percent lifetime risk of developing ovarian cancer. BRCA2 carriers face approximately 10 to 30 percent risk. These mutations also increase breast cancer risk and can run in families affecting multiple generations.

Reproductive and hormonal factors influence risk. Women who have never been pregnant face higher risk than those who have had children. Pregnancy and breastfeeding appear protective. The use of oral contraceptives for five or more years reduces ovarian cancer risk by approximately 50 percent and may provide lasting protection even after discontinuing use. Hormone replacement therapy, particularly estrogen-alone therapy, may increase risk in postmenopausal women.

Medical conditions and physical characteristics affect risk. Endometriosis, a condition where tissue normally lining the uterus grows outside it, increases ovarian cancer risk. Obesity appears to increase risk in postmenopausal women. Women with polycystic ovary syndrome (PCOS) may face elevated risk. Certain hereditary cancer syndromes beyond BRCA mutations also increase risk, including Lynch syndrome.
