Learn What Happens in an SSDI Quality Review

Understanding What a Quality Review Is

The Social Security Administration (SSA) conducts Quality Reviews to check whether people currently getting Social Security Disability Insurance (SSDI) still meet the requirements for their benefits. A Quality Review is a routine examination of your case file. It is not an investigation, and it does not mean you did something wrong. The SSA performs these reviews to make sure that benefits are going to people who continue to qualify under the program rules.

Quality Reviews happen for different reasons. Sometimes the SSA randomly selects cases to review. Other times, a review occurs because of information the SSA receives—such as a change in your medical condition, a work report, or earnings data. The agency may also review cases when you reach certain age milestones, such as turning 19 or reaching full retirement age.

According to SSA data, the agency conducts thousands of Quality Reviews each year across all SSDI beneficiaries. These reviews are part of routine program management and are required by federal law. The review process is meant to confirm that you still have a medical condition severe enough to prevent substantial work activity, and that you continue to meet other program requirements.

Understanding the difference between a Quality Review and a Medical Evidence Review (MER) is important. A Medical Evidence Review is typically more focused and may require updated medical records. A Quality Review is broader and examines your entire case to see if circumstances have changed.

Takeaway: A Quality Review is a routine check to confirm you still meet SSDI requirements. It is a normal part of the SSA's oversight of the program, not a sign of a problem.

How the SSA Selects Cases for Quality Review

The SSA uses several methods to decide which SSDI cases to review. One approach is statistical sampling. The agency selects a random sample of cases based on mathematical formulas designed to represent the overall population of beneficiaries. This random selection helps the SSA monitor the accuracy of payments across the program without reviewing every single case each year.

Another selection method involves risk factors. The SSA identifies certain characteristics that suggest a case may need review. For example, if you have a condition that may improve with treatment, your case might be flagged for periodic review. Conditions like back pain, some mental health disorders, or injuries that typically improve over time fall into this category. The SSA calls these "Medical Improvement Expected" (MIE) cases. People with MIE conditions typically receive review notices every one to three years.

Cases where medical improvement is possible but not expected ("Medical Improvement Possible" or MIP) are reviewed less frequently—usually every three to seven years. Cases where medical improvement is not expected ("Medical Improvement Not Expected" or MINE) may receive reviews even less often, though some do occur.

Changes in your circumstances can also trigger a review. If the SSA receives information that you are working, earning above certain limits, or living in a different country, this may prompt a Quality Review. Additionally, when you reach age 18, 19, or transitional work incentive ages, the SSA often schedules a review to confirm ongoing medical severity.

The SSA also reviews cases based on periodic earnings reports and work activity information. If your earnings records show substantial income, this may signal that a review is needed to determine if you can still be considered unable to work.

Takeaway: Cases are selected through random sampling, risk category assessment, and detection of changes in your circumstances. Understanding your condition's medical improvement category helps predict how often reviews may occur.

What Information the SSA Examines During a Quality Review

During a Quality Review, the SSA looks at many aspects of your case. The agency reviews your medical records first. These records include reports from your doctors, hospital visits, lab results, test findings, and any treatments you have received. The SSA examines whether your medical condition remains as severe as when you were first approved for benefits, or whether it has improved, worsened, or stayed roughly the same.

The SSA also reviews your work activity and earnings. The agency checks Social Security earnings records to see if you have worked and how much you earned. If you have earned income, the SSA determines whether your earnings are substantial. In 2024, earning more than $1,550 per month generally counts as substantial gainful activity (SGA), which could affect your benefits. However, certain work incentive programs may allow earnings above this level while maintaining benefits.

Your living situation is part of the review as well. The SSA examines where you live, whether you are in the United States, and any changes in your household or family status that might affect your case.

The SSA also looks at your age. As beneficiaries age, the rules for what counts as a severe condition may change. When you reach full retirement age, the program rules shift, and different medical standards may apply.

The agency checks whether you have reported any work activity through the Ticket to Work program or other SSA work incentive programs. If you are using one of these programs, your case will reflect that, and the SSA will evaluate your work and earnings under the program rules.

Finally, the SSA verifies that you have fulfilled your reporting responsibilities. If you were required to report changes in your condition, work activity, living arrangements, or other facts, the SSA checks whether you did so.

Takeaway: The SSA examines medical records, work and earnings history, living situation, age-related factors, and your compliance with reporting requirements during a Quality Review.

What Happens After the SSA Completes the Review

After the SSA finishes reviewing your case, one of several outcomes may occur. The most common outcome is that your benefits continue without change. This happens when the SSA finds that you still meet the requirements for SSDI. You will receive a notice in the mail telling you that your case has been reviewed and that your benefits will continue.

In some cases, the SSA may determine that you no longer meet the medical requirements for SSDI. This could happen if your condition has improved significantly, if new medical evidence shows you are able to work, or if the SSA believes there was a mistake in your original approval. If this occurs, the SSA will schedule a continuing disability review (CDR) medical examination. You may be asked to see a doctor chosen by the SSA so that the agency can gather current medical information before making a final decision.

If the SSA decides to stop your benefits, you have the right to appeal. The SSA must provide written notice explaining the reason for the proposed termination. You then have 65 days to request an appeal. During the appeal process, your benefits typically continue while the case is being reviewed, a rule called "Continuation of Benefits Pending Appeal" (CBPA). This protection means you do not lose income immediately if you disagree with the SSA's decision.

Sometimes the SSA needs more information to complete the review. If medical records are incomplete or outdated, the SSA may request that you provide recent reports from your doctors. You will receive a letter telling you what information is needed and the deadline for providing it. If you do not respond, the SSA may make a decision based on the information it already has, which could result in a negative decision.

In rare cases, the SSA may discover that you received overpayments—meaning you were paid more than you were supposed to receive. If this happens, the SSA will explain how the overpayment occurred and discuss payment options with you.

Takeaway: Quality Review outcomes include continuation of benefits, a request for medical examination, a proposed termination (with appeal rights), or a request for additional information. Understanding each possible outcome helps you respond appropriately.

Documents and Information You May Need to Provide

When the SSA conducts a Quality Review, you may be asked to provide various documents and information. The specific items requested depend on your individual case, but understanding what might be needed helps you prepare.

Medical records are the most common item requested. This includes recent reports from your doctors, specialists, therapists, or other healthcare providers. The SSA may ask for records from the past 12 months or since your last review. You can obtain these records from your healthcare providers' offices. Most providers have a records department that can send copies to the SSA on your behalf, though you may have to pay a copying fee.

If you have had any recent hospitalizations, emergency room visits, or surgeries, the SSA may request documentation
