Learn How Your GFR Number Works

What GFR Means and Why It Matters for Kidney Health

GFR stands for glomerular filtration rate. It's a number that shows how well your kidneys are working by measuring how much blood they filter each minute. Your kidneys contain tiny structures called glomeruli, which act like filters in a water treatment plant. Every day, your kidneys filter about 120 to 150 quarts of blood to produce roughly 1 to 2 quarts of urine. The GFR number tells your doctor how many milliliters of waste your kidneys can filter from your blood in one minute.

The GFR number ranges from 0 to over 90, with higher numbers indicating better kidney function. A normal GFR for adults is 90 or above. As you age or if kidney disease develops, this number typically declines. Understanding your GFR helps you and your healthcare provider track kidney health over time and make decisions about treatment or lifestyle changes.

Your kidneys perform several critical jobs beyond just filtering waste. They help regulate blood pressure, produce hormones that control red blood cell production, and balance electrolytes like potassium and sodium in your body. When kidneys don't filter properly, waste and extra water build up in your bloodstream, leading to serious health problems. This is why monitoring GFR matters—it catches kidney problems early when interventions may help slow progression.

Many people have kidney disease without knowing it because there are often no symptoms in early stages. You might feel fine while your kidneys are gradually losing function. Regular GFR testing through blood work becomes increasingly important as you age or if you have conditions like diabetes or high blood pressure that affect kidney function.

Practical Takeaway: Think of your GFR number as a report card for your kidneys. Just as a school report card shows how well a student is performing, your GFR shows how well your kidneys are performing their filtering job. A higher GFR score indicates better kidney function.

How GFR Gets Calculated From Your Blood Test

Your GFR isn't measured directly. Instead, it's calculated using information from a blood test combined with other personal information. The blood test measures a substance called creatinine, which is a waste product that your kidneys filter out. Creatinine comes from the normal breakdown of muscle tissue. Because muscle breakdown is relatively consistent, the amount of creatinine in your blood reflects how well your kidneys are filtering.

Doctors use a mathematical formula to estimate your GFR from your creatinine level. The most common formula is called the CKD-EPI equation (Chronic Kidney Disease Epidemiology Collaboration). This formula takes into account your creatinine level, age, sex, and race. The formula was updated in 2021 to remove race as a variable because research showed it was leading to underestimation of kidney disease in Black patients.

The calculation looks something like this: a 45-year-old woman with a creatinine level of 0.9 might have a calculated GFR of around 85. The same creatinine level in a 75-year-old man might calculate to a GFR of around 65, because age affects how muscle breaks down. This is why the formula needs multiple pieces of information, not just the creatinine number alone.

It's important to know that estimated GFR (often written as eGFR) is not a perfect measurement. It's an estimate based on mathematical models. Sometimes a healthcare provider might order additional tests if they have concerns about kidney function, especially if your eGFR seems inconsistent with your health status. Other tests might include measuring cystatin C, a different protein filtered by kidneys, or performing a 24-hour urine collection for a more accurate assessment.

Practical Takeaway: When you get lab results showing your GFR, remember it's calculated from your creatinine blood test along with your age and sex. You don't need to do anything special before this test—it's a regular blood draw. Ask your doctor to explain your specific results since they vary based on your individual characteristics.

Understanding GFR Stages and What Each Number Range Means

Medical professionals divide kidney function into five stages based on GFR ranges. Understanding these stages helps you grasp what your specific GFR number means for your kidney health. Stage 1 is a GFR of 90 or higher, which indicates normal kidney function. People in Stage 1 typically have no kidney disease, though they might have protein in urine or structural kidney damage that hasn't affected filtration yet.

Stage 2 is a GFR between 60 and 89, representing mildly decreased kidney function. At this stage, you likely won't have symptoms, but your kidneys are beginning to lose some ability to filter waste. Many people discover they're in Stage 2 during routine blood work. Stage 2 kidney disease is relatively common in older adults and doesn't necessarily mean serious problems will develop, especially with lifestyle management.

Stage 3 splits into two parts. Stage 3a is a GFR between 45 and 59, representing mild to moderate kidney function loss. Stage 3b is a GFR between 30 and 44, representing moderate to severe kidney function loss. People in Stage 3 should begin discussing kidney health management with their doctor. Symptoms like swelling in the feet or ankles, fatigue, or shortness of breath might start appearing, though many people still have no symptoms.

Stage 4 is a GFR between 15 and 29, representing severely decreased kidney function. At this point, waste accumulates noticeably in the blood, and people often experience symptoms. Planning for future treatment becomes important. Stage 5 is a GFR below 15, representing kidney failure. At this stage, kidneys cannot filter enough waste to keep you healthy, and dialysis or a kidney transplant becomes necessary for survival.

It's crucial to understand that having a GFR in Stage 2 or even early Stage 3 doesn't mean you'll definitely progress to kidney failure. Many people's kidney function stabilizes or declines very slowly, especially if they manage their health conditions carefully. The stage your GFR falls into is one piece of information your doctor uses, along with other factors, to create a health plan suited to you.

Practical Takeaway: Learn which stage your GFR falls into, but remember that the stage is just a category. Your individual situation depends on many factors including your age, other health conditions, and how your GFR changes over time. Track how your GFR number changes from one test to the next—stable or slowly declining is generally better than rapidly declining.

Factors That Affect Your GFR Number

Many different factors influence your GFR, and understanding them helps you interpret your results. Age is one major factor—GFR naturally declines as people get older. A GFR of 60 might be normal for an 80-year-old but concerning for a 30-year-old. Sex affects GFR as well, with men generally having slightly higher GFR than women of the same age because men typically have more muscle mass, which produces more creatinine.

Certain medical conditions significantly affect GFR. Diabetes is one of the leading causes of kidney disease. High blood pressure damages kidney blood vessels and reduces filtration over time. Glomerulonephritis, which is inflammation in the kidney's filtering units, can cause GFR to drop rapidly. Lupus and other autoimmune diseases sometimes attack the kidneys. Chronic infections, especially untreated urinary tract infections, can damage kidney tissue. Kidney stones and urinary blockages can also impair kidney function.

Medications can impact GFR in multiple ways. Some blood pressure medications actually protect kidney function, while others might temporarily affect creatinine levels. Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can reduce kidney function, especially in people already at risk. Certain antibiotics, contrast dyes used in imaging, and some cancer medications can harm kidneys. Always tell your doctor about all medications and supplements you take, as they might affect your kidney health or your GFR test results.

Lifestyle factors play a significant role in kidney function. Dehydration concentrates creatinine in your blood, potentially making GFR appear lower than it actually is. A diet high in sodium and processed foods increases blood pressure, which strains kidneys. Excess body weight
