Learn How to Use Hemorrhoid Cream Properly

Understanding What Hemorrhoid Cream Does

Hemorrhoid creams are topical medications designed to treat the symptoms of hemorrhoids, also called piles. These are swollen veins in the lower rectum and anus that can cause discomfort, itching, and bleeding. According to the American Society of Colon and Rectal Surgeons, about 75% of people will experience hemorrhoids at some point in their lives, though not all require medical treatment.

Hemorrhoid creams work through several active ingredients. Hydrocortisone is a corticosteroid that reduces inflammation and swelling. Phenylephrine is a vasoconstrictor that narrows blood vessels to decrease swelling. Witch hazel is an astringent that helps tighten tissue and reduce irritation. Benzocaine or lidocaine are local anesthetics that numb the area and provide pain relief. Most over-the-counter creams combine two or more of these ingredients to address multiple symptoms simultaneously.

The creams are meant to be used externally on hemorrhoids outside the body or internally if the formula includes an applicator for internal hemorrhoids. Using the right product for your situation matters because internal and external hemorrhoids respond differently to treatment. External hemorrhoids are visible and can be felt around the anus opening, while internal hemorrhoids are inside the rectum and you typically cannot see or feel them directly.

It is important to understand that hemorrhoid cream treats symptoms rather than the underlying cause. This means the hemorrhoids themselves may not disappear, but the pain, itching, and swelling should decrease. Most creams provide relief within minutes to a few hours of application. Results vary based on the severity of the hemorrhoid and the specific product used.

Practical Takeaway: Before using any hemorrhoid cream, determine whether your hemorrhoids are internal or external, as this affects which product and application method will work best for you.

Preparing the Area Before Application

Proper preparation of the affected area is one of the most important steps for getting the most benefit from hemorrhoid cream. The area must be clean and dry before you apply the medication. Bacteria and moisture can interfere with how the cream works and may increase the risk of infection. Many people skip this step, which reduces the cream's effectiveness.

Start by gently cleaning the area with warm water. You can use a soft cloth or cotton ball, or you can use the toilet and splash water gently. Pat the area completely dry with a clean tissue or soft cloth. Do not rub harshly, as hemorrhoids are sensitive and aggressive cleaning can cause additional pain or bleeding. If you have just had a bowel movement, waiting 10 to 15 minutes before applying cream allows the area to settle and become less irritated.

Some people benefit from taking a warm sitz bath before applying cream. A sitz bath involves sitting in 2 to 3 inches of warm (not hot) water for 10 to 15 minutes. This relaxes the muscles around the anus, reduces pain, and makes the area more receptive to medication. The National Institutes of Health notes that sitz baths can reduce hemorrhoid pain and promote healing. After the sitz bath, make sure to dry the area thoroughly before applying cream.

If you are using hydrocortisone cream, understand that corticosteroids should not be applied to broken or infected skin without medical guidance. If you notice signs of infection such as pus, warmth, or spreading redness, do not apply the cream and contact a healthcare provider instead. Similarly, if there is active bleeding, it may be better to wait a few minutes for the bleeding to slow before applying the cream.

Practical Takeaway: Always clean and completely dry the area before applying hemorrhoid cream, and consider a warm sitz bath beforehand to reduce pain and improve the cream's effectiveness.

Correct Application Techniques for External Hemorrhoids

Applying hemorrhoid cream correctly to external hemorrhoids requires a gentle touch and the right technique. External hemorrhoids are located on the outer surface of the anus and are the most visible and often the most painful type. The good news is that applying cream to external hemorrhoids is straightforward compared to internal application.

For external hemorrhoids, squeeze a small amount of cream—roughly the size of a pea—onto your fingertip or a clean applicator if the tube includes one. Using your finger is acceptable as long as your hands are clean. Gently apply the cream directly to the external hemorrhoid, using light circular motions. Do not press hard or massage aggressively, as this can irritate the area further. Most people find that gentle dabbing or light spreading works better than rubbing.

You may apply external hemorrhoid cream up to four times per day, or as directed on the package label. Many people apply it after bowel movements, before bed, and once or twice during the day. Keep track of when you apply the cream so you maintain consistent timing. If you forget a dose, simply apply it when you remember and continue with your regular schedule—do not double up on the next dose.

Wash your hands thoroughly after application, even if you used an applicator, to prevent spreading bacteria. Some creams can stain clothing, so be aware of where you sit immediately after application. Wearing clean underwear and loose clothing can help the area stay clean and reduce irritation. If the cream feels greasy, you can place a small piece of clean gauze over the area to prevent it from transferring to your clothes.

Most people notice symptom relief within a few hours to one day of starting hemorrhoid cream. If you do not notice any improvement after three to five days of consistent use, or if symptoms worsen, contact a healthcare provider for further evaluation.

Practical Takeaway: Apply a pea-sized amount of cream directly to external hemorrhoids using gentle motions, up to four times daily, and wash your hands afterward.

Using Applicators Correctly for Internal Hemorrhoids

Internal hemorrhoids require a different application approach than external hemorrhoids because they are located inside the rectum, out of direct reach. Many over-the-counter hemorrhoid creams designed for internal use include a small plastic applicator with the tube. This applicator allows you to place the medication inside the rectum where the internal hemorrhoid is located.

To use the applicator, first wash your hands thoroughly. Attach the applicator to the tube according to the package directions—typically by twisting or pushing it onto the tip of the tube. Squeeze a small amount of cream into the applicator, about the amount specified in the instructions, which is usually one-half inch to one inch of cream. Do not overfill the applicator, as this makes insertion uncomfortable and wastes medication.

Before inserting the applicator, consider lubricating the tip with a small amount of water-soluble lubricant to make insertion smoother and more comfortable. While not required, this step can reduce discomfort. Take a position that feels natural and comfortable—many people find it easiest to insert the applicator while sitting on the toilet, standing with one leg elevated, or lying on their side with knees bent.

Gently insert the applicator into the rectum until you feel mild resistance, typically one to one and one-half inches inside. Do not force it or insert it too far. Slowly squeeze the tube or applicator to dispense the cream, then gently withdraw the applicator. Remain in position for a few moments to allow the cream to settle into the area. Wash the applicator immediately with warm water and soap, and allow it to air dry.

The FDA states that hydrocortisone should not be used internally for more than seven consecutive days without consulting a healthcare provider. This is because corticosteroids can have different effects in the rectum compared to external skin. If you need to use internal hemorrhoid cream for longer than one week, consult your doctor for guidance.

Practical Takeaway: Use the included applicator to dispense the correct amount of cream into the rectum, insert gently about one to one and one-half inches, dispense the cream, and clean the applicator thoroughly after each use.

Managing Potential Side Effects and When to Seek Medical Care

While hemorrhoid creams are generally safe when used as directed, some people experience side effects. The most common side effect is mild irritation or a stinging sensation when first applying the cream. This typically goes away after a few applications as your skin adjusts. If the stinging continues or wors
