Learn How Health Allowance Cards Work

What Health Allowance Cards Are and How They Function

Health allowance cards are payment cards linked to tax-favored accounts that let people use pre-tax money for medical costs. These cards work similarly to debit cards but are specifically designed to pay for health-related expenses. The main types include Flexible Spending Account (FSA) cards, Health Savings Account (HSA) cards, and Health Reimbursement Arrangement (HRA) cards. Each type connects to a different kind of account with its own rules about how money can be used and what happens to unused funds.

The cards function by drawing from money that employers or individuals have set aside before taxes are taken out of paychecks. This arrangement reduces the amount of income subject to federal income tax, Social Security tax, and Medicare tax. For example, if a person contributes $2,500 to an FSA during the year, that $2,500 is not counted as taxable income. When they use their FSA card to buy prescription medications or copayments, the transaction is deducted directly from their FSA account balance.

According to the U.S. Bureau of Labor Statistics, about 34% of workers at private companies have access to FSAs through their employers. HSAs, which are available to people with high-deductible health plans, have grown significantly—the number of HSA accounts reached approximately 33 million in 2023, up from roughly 6 million in 2006. This growth shows that more people are using these cards as part of their healthcare and financial planning.

Health allowance cards typically look like regular debit or credit cards but have labels or markings that indicate they are health-specific. They come with a PIN number for security and usually can be used at pharmacies, doctors' offices, hospitals, and other medical providers that accept them. Some cards can also be used online or by mail for mail-order prescriptions and medical supplies.

Practical takeaway: Health allowance cards are tools that help people pay for medical expenses with money that has not been taxed. Understanding which type of card is connected to your account is the first step in using it properly and avoiding problems.

Types of Health Allowance Cards and Their Account Sources

Three main types of health allowance cards exist, each tied to a different account structure. FSA cards are connected to Flexible Spending Accounts, which are employer-sponsored plans. Employees choose how much money to set aside before taxes during their company's open enrollment period, usually once per year. The employer may also contribute to the FSA. FSA money is generally "use-it-or-lose-it," meaning that money not spent by the end of the plan year is forfeited, though some plans allow a grace period of up to 2.5 months or let workers carry over a small amount (up to $660 in 2024).

HSA cards are tied to Health Savings Accounts, which are tax-advantaged accounts available to people enrolled in high-deductible health plans (HDHPs). Unlike FSAs, HSA funds roll over from year to year—money not spent does not disappear. This allows HSA balances to grow over time and function somewhat like a retirement account for healthcare costs. People can contribute to their own HSAs, employers can contribute, or both can contribute. In 2024, individuals can contribute up to $4,150 per year to an HSA, and families can contribute up to $8,300 per year.

HRA cards are connected to Health Reimbursement Arrangements, which are employer-funded accounts. The employer controls the terms of the HRA, including how much money is available and what counts as an allowable expense. Unlike FSAs and HSAs, employees do not contribute their own money to HRAs. The rules and rollover policies vary by employer, so workers using HRA cards need to check their specific plan documents.

A fourth type, called a Dependent Care FSA card, exists for childcare and adult dependent care expenses but operates under different rules than medical FSA cards. These cards can only pay for qualifying care services, such as daycare centers, preschools, after-school programs, or in-home caregivers for dependents under age 13 or adult dependents who cannot care for themselves.

Practical takeaway: Knowing which type of card and account you have matters because the rules about contribution limits, spending deadlines, and rollover policies differ. Check your plan documents or ask your employer's benefits administrator to confirm what type of card you have.

What Expenses You Can Pay With Health Allowance Cards

Health allowance cards can only be used to pay for qualified medical expenses as defined by the Internal Revenue Service (IRS). These include prescription medications, copayments and coinsurance, deductibles, and over-the-counter medications (with a prescription from a doctor or certain items like insulin that are allowed without a prescription). Dental work, vision care including eyeglasses and contact lenses, and hearing aids are also covered. Mental health services, physical therapy, chiropractic care, and acupuncture by licensed practitioners count as qualified expenses.

Medical equipment and supplies that are covered include blood glucose monitors, needles and syringes, blood pressure monitors, thermometers, crutches, wheelchairs, walkers, and bandages. Prescription medical supplies like diabetic testing strips, wound dressings, and ostomy supplies are allowed. Transportation costs directly related to medical care, such as ambulance services or mileage to and from medical appointments, can qualify, though limitations apply to standard taxi or ride-sharing services.

However, certain healthcare-related items cannot be paid for with these cards. Cosmetic procedures and treatments are not covered unless they are medically necessary due to injury or disease. Over-the-counter pain relievers, cold medicines, vitamins, and supplements are generally not covered unless prescribed by a doctor. Health club memberships, even if recommended for medical reasons, do not count. Maternity clothes, toiletries, and cosmetics are excluded. Insurance premiums themselves cannot be paid from FSA or HRA cards, though in some cases HSA funds can be used for certain types of insurance premiums.

The IRS publishes a detailed list of qualified medical expenses in Publication 502, and this list is updated periodically. Rules can be complex—for example, laser eye surgery (LASIK) is covered, but regular eyeglasses may or may not be covered depending on the plan. Some expenses require itemized receipts showing what was purchased and proof that it was for medical care. When there is doubt about whether an expense qualifies, contacting the card issuer or consulting the plan's documentation is advisable.

Practical takeaway: Before using your health allowance card, verify that the purchase qualifies. Keep receipts and documentation of medical expenses. When uncertain, ask your card issuer or plan administrator before making the purchase to avoid spending money on non-qualifying items.

How to Use Your Health Allowance Card in Daily Transactions

Using a health allowance card is straightforward in most situations. At a pharmacy, simply present the card at checkout when purchasing prescription medications or over-the-counter items that meet plan requirements. The pharmacist's system should communicate with the card network to verify that the card can be used for that purchase. The transaction processes like a debit card, and the amount is deducted from your account balance.

At a doctor's office or hospital, you can use the card to pay copayments, coinsurance, or portions of your deductible. Front desk staff are familiar with these cards and can process them during check-in or at checkout. For some offices, you may need to call ahead to confirm they accept the specific card type you have. Many larger medical practices and all major hospital systems accept FSA and HSA cards without issue.

Online purchases present a different situation. Some online pharmacies and medical supply retailers accept health allowance cards directly. Others may require you to pay out of pocket and then submit a receipt for reimbursement. When buying from online retailers, look for payment options that specifically mention FSA, HSA, or dependent care cards. If the option is not available, you can use a credit or debit card and then request reimbursement from your plan administrator by submitting the itemized receipt and a claim form.

For mail-order prescriptions and supplies, you typically can use your card directly if the vendor participates in the card network. Alternatively, you can send a check or use a credit card and submit documentation for reimbursement. Some mail-order pharmacy services are set up specifically
