Understanding Tinnitus: What It Is and Why It Happens

Tinnitus is the experience of hearing sound in your ears when no external sound is present. The sound may be a ringing, buzzing, hissing, chirping, whistling, or roaring noise. According to the National Institute on Deafness and Other Communication Disorders, about 10% of American adults—roughly 25 million people—experience tinnitus lasting at least five minutes in a given year. For some people, tinnitus is occasional and mild. For others, it occurs constantly and significantly affects daily life, sleep, concentration, and emotional well-being.

Tinnitus itself is not a disease but rather a symptom of an underlying condition. The sound originates in the inner ear, auditory nerve, or brain, rather than from an external source. When you hear this phantom sound, it means your auditory system is sending signals to your brain even though there's no actual noise in your environment.

Many different factors can trigger tinnitus. Hearing loss is one of the most common causes. As people age or experience damage to the inner ear from loud noise exposure, certain hair cells in the cochlea (the hearing organ) become damaged or die. This damage can cause the auditory system to misfire and produce the sensation of sound. Other causes include earwax buildup, ear infections, problems with the Eustachian tube (the passage that drains fluid from the middle ear), head or neck injuries, and certain medications. Some people develop tinnitus after exposure to very loud sounds, such as explosions, gunfire, or prolonged noise in industrial settings.

Medical conditions can also lead to tinnitus. These include high blood pressure, heart disease, anemia, thyroid disorders, and diabetes. Stress and fatigue may make tinnitus more noticeable or bothersome, even if they don't directly cause it. In some cases, doctors cannot identify a specific cause, which is called idiopathic tinnitus.

Practical Takeaway: If you experience tinnitus, tracking when it started, what it sounds like, how often it occurs, and what makes it better or worse can help you describe your symptoms to a healthcare provider. Keep notes about whether the sound is in one ear or both, and whether other symptoms accompany it, such as hearing loss, vertigo, or ear pain.

Medical Evaluation: Getting an Accurate Diagnosis in New York

The first step in exploring tinnitus treatment options is obtaining a proper medical evaluation. In New York, you can seek tinnitus assessment through several types of healthcare providers, including your primary care physician, an otolaryngologist (ear, nose, and throat specialist), an audiologist, or a neurotologist (a specialist in inner ear and skull base disorders). Each provider brings different expertise to understanding your condition.

During an evaluation, your healthcare provider will take a detailed history of your tinnitus. They'll ask when it started, whether it's constant or comes and goes, which ear or ears are affected, what the sound is like, and how much it bothers you. They'll inquire about any recent loud noise exposure, head injuries, infections, or changes in medication. You'll also discuss other symptoms you may experience, such as hearing loss, balance problems, or ear fullness. This conversation helps the provider narrow down possible causes.

Physical examination typically includes looking inside your ears with an otoscope to check for earwax, infection, or eardrum problems. Your provider may perform tests to assess your hearing. An audiogram is a common test where you sit in a soundproof booth and indicate when you hear tones at different pitches and volumes. This test measures your hearing sensitivity and can reveal hearing loss patterns associated with tinnitus. Tympanometry tests how well your eardrum vibrates, which can indicate middle ear problems.

Depending on initial findings, your provider may recommend additional imaging or testing. An MRI or CT scan may be ordered if symptoms suggest a structural problem, tumor, or vascular condition. Blood tests can screen for thyroid disease, anemia, or other metabolic conditions. Some New York clinics offer tinnitus-specific assessments that measure the loudness and pitch of your tinnitus or evaluate how much it affects your quality of life using standardized questionnaires.

New York City and surrounding areas have numerous ear and hearing centers equipped with advanced diagnostic technology. Many major hospitals, including New York-Presbyterian, NYU Langone, Mount Sinai, and Weill Cornell Medicine, have otolaryngology and audiology departments. Smaller practices and community health centers throughout New York State also provide tinnitus evaluations. The key is finding a provider who takes your symptoms seriously and conducts a thorough evaluation to identify any treatable underlying cause.

Practical Takeaway: Before your appointment, write down details about your tinnitus, any recent illnesses or injuries, medications you take, and how tinnitus affects your sleep, work, and mood. Bring this information to your evaluation. If possible, have a family member or friend accompany you to help remember what the provider discusses and to describe how tinnitus has changed from their perspective.

Treatment Options for Underlying Causes

When tinnitus results from a specific, treatable cause, addressing that underlying condition often reduces or eliminates the tinnitus. This is why accurate diagnosis matters so much. Several common causes of tinnitus in New York residents can be effectively treated.

Earwax impaction is among the most straightforward causes to treat. When earwax buildup blocks the ear canal, it can trigger tinnitus. A healthcare provider can safely remove impacted earwax through irrigation, suction, or manual removal using specialized instruments. Many patients experience immediate relief once the earwax is cleared. This procedure takes just a few minutes and can often be performed in a primary care office or urgent care clinic.

Ear infections, whether in the outer, middle, or inner ear, frequently cause tinnitus along with ear pain, drainage, or hearing loss. Outer ear infections (otitis externa) are treated with antibiotic or antifungal ear drops. Middle ear infections (otitis media) may be treated with oral antibiotics, decongestants, or observation, depending on whether bacteria or viruses cause the infection. Once the infection clears, tinnitus typically resolves. Inner ear infections are more serious and require prompt medical attention; treatment may include antibiotics, antivirals, or corticosteroids depending on the cause.

Eustachian tube dysfunction occurs when the small tube connecting your middle ear to your throat doesn't open and close properly, trapping fluid in the middle ear. This can produce a fullness sensation, conductive hearing loss, and tinnitus. Treatment options include nasal decongestants, antihistamines, nasal corticosteroid sprays, or a procedure called myringotomy, where a small tube is inserted through the eardrum to drain fluid. Many New York otolaryngology practices offer this common procedure.

Medication-related tinnitus can sometimes be addressed by adjusting or discontinuing the offending medication, though this must always be done under medical supervision. Certain antibiotics, cancer medications, water pills (diuretics), and high-dose aspirin are known to cause or worsen tinnitus. If your provider identifies that a medication you're taking contributes to tinnitus, they may suggest an alternative medication that doesn't have this side effect, or they may adjust your dosage. Never stop taking a medication on your own; discuss options with your doctor.

Tinnitus related to high blood pressure or cardiovascular conditions may improve when those conditions are brought under control through medication, lifestyle changes, or other treatments. Similarly, tinnitus caused by thyroid disease, anemia, or diabetes may decrease once the underlying condition is managed effectively.

Practical Takeaway: Ask your provider whether your tinnitus likely stems from a treatable cause. If so, discuss the specific treatment plan, how long improvement might take, and what signs would indicate the treatment is working. Keep follow-up appointments to allow time for the underlying condition to resolve and for any medication changes to take effect.

Sound Therapy and Hearing Aids for Tinnitus Management

For tinnitus that persists despite treatment of underlying causes, or when no specific cause can be identified, sound-based therapies offer ways to reduce how much tinnitus bothers you. The principle behind these approaches is that tinnitus becomes less noticeable or bothersome when it's masked or blended with other sounds.
