Understanding Syphilis: What It Is and How It Spreads

Syphilis is a sexually transmitted infection (STI) caused by a bacterium called Treponema pallidum. This infection has affected humans for centuries and remains a public health concern today. According to the Centers for Disease Control and Prevention (CDC), syphilis cases have been rising in recent years, with over 200,000 cases reported in the United States in 2022. The infection can affect anyone, regardless of age, gender, or sexual orientation, though certain populations experience higher rates of infection.

The bacterium spreads primarily through direct contact with a syphilis sore, called a chancre, during sexual activity. This includes vaginal, anal, or oral sex. The infection can also spread from an infected pregnant person to their baby during pregnancy or childbirth, a condition known as congenital syphilis. Importantly, syphilis cannot spread through casual contact like hugging, shaking hands, sharing food or drinks, or using the same toilet seat. The bacteria need direct access to the bloodstream or mucous membranes to cause infection.

Understanding how syphilis spreads is crucial for prevention. The infection progresses through distinct stages if left untreated, each with different characteristics and levels of contagiousness. Early detection and treatment can prevent serious complications and stop transmission to others. People can reduce their risk of infection by using condoms or dental dams during sexual activity, maintaining open communication with sexual partners about STI testing, and seeking regular health screenings.

Practical takeaway: Syphilis spreads through sexual contact with sores or through mother-to-baby transmission. Knowing how it spreads helps you understand prevention methods and why testing matters for sexual health.

Primary Syphilis: The First Stage and Early Warning Signs

Primary syphilis is the earliest stage of infection, typically appearing between 3 to 90 days after exposure, with an average of 21 days. During this stage, the most noticeable sign is a chancre—a firm, round, painless sore that appears at the site where the bacteria entered the body. This could be on the genitals, anus, lips, mouth, or other areas. The chancre often has a raised, reddish border and a clean base, though appearance can vary. Many people notice only one sore, but multiple chancres can develop in some cases.

A key characteristic of primary syphilis is that the chancre is typically painless, which sometimes leads people to delay seeking care. The sore will feel firm if touched, almost like a button. Even without treatment, the chancre will heal on its own within 3 to 6 weeks, but this does not mean the infection is gone. The bacteria continue to multiply in the body, and the infection progresses to the secondary stage. During primary syphilis, a person is highly contagious and can transmit the infection to sexual partners.

Some people also experience swollen lymph nodes near the chancre during primary syphilis. These might feel like small, tender lumps in the groin, neck, or armpits. A person might also have mild symptoms like fatigue, fever, or headache, though these are less common in primary syphilis than in later stages. Because the chancre is painless and may be in a location a person doesn't regularly see or think about, it can go unnoticed for weeks.

Practical takeaway: Watch for painless sores at the site of sexual contact. Even if a sore disappears, the infection continues. Getting tested during primary syphilis is important because antibiotics can cure the infection and prevent serious complications.

Secondary Syphilis: Systemic Symptoms and Body-Wide Effects

Secondary syphilis develops weeks to months after primary infection, typically between 2 to 8 weeks after the chancre first appears, though it can occur up to 2 years later. This stage reflects the spread of the bacteria throughout the body, causing a range of symptoms. During secondary syphilis, a person remains highly contagious and can transmit infection through sexual contact. Additionally, secondary syphilis can occasionally spread through contact with sores, so infected individuals should be cautious about blood donations and avoid sharing toothbrushes or other items that contact the mouth or sores.

The most common symptom of secondary syphilis is a skin rash that typically appears on the trunk and extremities. Unlike many rashes, the syphilis rash characteristically appears on the palms of the hands and soles of the feet, which is relatively uncommon for other infections. The rash may consist of small, flat, reddish-brown spots or slightly raised bumps. The rash is usually not itchy and does not hurt. Some people develop additional skin findings, including patches of hair loss (alopecia) or a condition called condyloma latum, which appears as moist, flat sores in warm, moist body areas.

Systemic symptoms during secondary syphilis can include fever, fatigue, headache, muscle aches, sore throat, and swollen lymph nodes throughout the body. Some people experience a condition called syphilitic hepatitis, affecting the liver, while others develop eye inflammation or vision problems. In some cases, secondary syphilis affects the nervous system, causing meningitis or other neurological symptoms. These varied presentations can make secondary syphilis difficult to diagnose without testing, as symptoms resemble many common illnesses like influenza or mononucleosis.

Practical takeaway: Secondary syphilis causes widespread body symptoms including rash on palms and soles, fever, and swollen lymph nodes. If you develop these symptoms after potential exposure, getting tested allows doctors to diagnose and treat the infection before it causes long-term damage.

Latent Syphilis: The Silent Stage and Hidden Progression

Latent syphilis is a stage where the infection remains in the body but produces no visible symptoms. This stage begins after secondary syphilis resolves, either spontaneously or with treatment, and can last for years or even a lifetime if untreated. The latent stage is divided into two periods: early latent syphilis (within the first year after infection) and late latent syphilis (after one year). During early latent syphilis, people may still transmit the infection to sexual partners, and infected pregnant people can still pass the infection to their babies. Late latent syphilis carries minimal risk of sexual transmission, though transmission to babies during pregnancy remains possible.

During latent syphilis, a person may feel completely healthy and have no symptoms whatsoever. Blood tests reveal the infection, but the person experiences no physical signs. This can create a false sense of security, as individuals may not realize they are infected and continue transmitting the disease to partners. Some people remain in the latent stage indefinitely, never developing additional symptoms. However, others progress to tertiary syphilis, which can develop anywhere from a few years to several decades after initial infection. The CDC estimates that about 15% to 30% of people with untreated syphilis will develop tertiary complications.

The length of latent syphilis varies significantly between individuals and is not fully understood. Factors such as immune system strength, overall health status, and the strain of bacteria involved may influence progression. During this stage, the bacteria can continue to affect multiple body systems silently. This is why regular STI testing is recommended for sexually active individuals, even when no symptoms are present. Testing during latent syphilis allows treatment to prevent progression to tertiary syphilis and protects future sexual partners.

Practical takeaway: Latent syphilis has no symptoms but remains in the body and can be discovered through testing. Untreated latent syphilis may eventually cause serious organ damage, making periodic testing an important part of sexual health even when you feel fine.

Tertiary Syphilis: Long-Term Complications and Serious Health Effects

Tertiary syphilis is the most serious stage, developing in people with untreated syphilis after many years or decades. This stage causes severe, permanent damage to body systems and organs. According to medical literature, tertiary syphilis can affect the brain and nervous system (neurosyphilis), the heart and blood vessels (cardiovascular syphilis), and bones and skin (gummatous syphilis). The complications of tertiary syphilis are often irreversible, which is why early detection and treatment are so important.
