Learn About SSDI and How the Program Works

What Is SSDI and Who It Serves

Social Security Disability Insurance (SSDI) is a federal program that provides monthly payments to people who have worked and paid Social Security taxes but can no longer work due to a medical condition. Unlike Supplemental Security Income (SSI), which is needs-based, SSDI is an insurance program. Think of it like car insurance or health insurance — you pay into the system while working, and the program pays benefits if you experience a covered event.

The program was created in 1956 and is managed by the Social Security Administration (SSA). As of 2024, approximately 8.1 million people receive SSDI benefits. The program serves workers of all ages, including young adults and children through the disabled adult child (DAC) program, which allows adult children who were disabled before age 22 to receive benefits based on a parent's work record.

SSDI differs from other disability programs in important ways. Workers' compensation covers job-related injuries. Veterans benefits serve former military members. State disability programs operate in a few states. SSDI is the largest federal disability insurance program and serves people regardless of when or where their disability occurred.

To receive SSDI, a person generally must have worked long enough and recently enough to have paid sufficient Social Security taxes. The exact work history requirements depend on age — younger workers need fewer work credits than older workers. A work credit is earned when you make a certain amount of income (in 2024, $1,730 in earnings equals one credit, and you can earn up to four credits per year). Most people need 40 work credits total, with 20 of those credits earned in the 10 years before disability began.

Practical takeaway: SSDI is an insurance program for people who worked and paid taxes but cannot work due to a serious medical condition. Understanding this distinction helps clarify how SSDI differs from welfare or need-based programs.

Understanding the Medical Criteria for SSDI

The SSA uses a strict definition of disability for SSDI. The condition must be severe enough that it prevents substantial gainful activity (SGA). In 2024, SGA means earning more than $1,550 per month for non-blind individuals and $2,590 for blind individuals. The disability must be expected to last at least 12 months or result in death.

The SSA maintains a Listing of Impairments, which is a detailed guide of medical conditions that automatically meet disability standards. These listings exist for musculoskeletal disorders, cancer, cardiovascular disease, respiratory disease, digestive disease, genitourinary disease, hematological disorders, skin disease, endocrine disorders, neurological disorders, mental disorders, neoplastic diseases, immune system disorders, and special senses and speech disorders. To meet a listing, a person's medical evidence must match specific criteria outlined for that condition.

Not all conditions on the Listing automatically result in SSDI approval. Medical evidence must demonstrate that the person's condition matches the listing criteria exactly. For example, someone with cancer may meet a listing if they have received treatment and their cancer is at a certain stage, but the SSA will review medical records from the treating physicians. Someone with depression may meet a mental disorder listing only if their symptoms and functional limitations match the specific criteria.

If a condition does not meet a listing, the SSA can still find someone disabled through a process called "medical-vocational allowance." This approach considers the person's age, education, work experience, and residual functional capacity (RFC). RFC describes what physical and mental activities someone can still do despite their condition. A 58-year-old with a high school education whose RFC shows they cannot lift more than 10 pounds may be found disabled even without meeting a specific listing, because returning to work would be very difficult given their age and abilities.

Mental health conditions are evaluated the same way as physical conditions. The SSA considers psychiatric medications, frequency of mental health treatment, hospitalization history, therapy participation, and functional limitations. Common mental health conditions that have resulted in SSDI approval include schizophrenia, bipolar disorder, major depressive disorder, anxiety disorders, and autism spectrum disorder in adults. The key is showing that the condition causes significant functional limitations that prevent work.

Practical takeaway: The SSA has specific medical standards and requires documentation from healthcare providers. Understanding these criteria helps people gather appropriate medical evidence if they seek SSDI.

The SSDI Application and Review Process

The SSDI process begins with an application, which can be started online through the SSA's website, by phone, or in person at a local Social Security office. The online application (iSSAL) allows people to complete the form at their own pace. Phone applications are available by calling 1-800-772-1213. In-person applications can be made at any Social Security office, and an appointment is encouraged but not required.

The application asks for detailed information: Social Security number, work history, medical conditions, healthcare providers, medications, and dates of treatment. Applicants must provide the names and addresses of all doctors, hospitals, and mental health providers who have treated them. The SSA uses this information to obtain medical records directly from these providers. Applicants should be thorough and accurate because incomplete information can delay the review process.

After application, the case goes to a Disability Determination Services (DDS) office in the applicant's state. These state agencies, working under contract with the SSA, make the initial determination. A team reviews the application and medical records. This typically includes a disability examiner (who reviews the non-medical aspects) and a medical or psychological consultant (who evaluates the medical evidence). The DDS team may request additional information or medical records.

Initial decisions typically take 3 to 6 months, though this varies significantly by state and case complexity. About 35% of initial applications are approved. If denied, applicants can request reconsideration, which sends the case to a different DDS examiner for review. This step takes another 3 to 6 months. The reconsideration approval rate is lower than initial applications.

If reconsideration is denied, the person may request a hearing before an Administrative Law Judge (ALJ). This is where many people eventually receive approval — ALJ approval rates are higher than initial or reconsideration rates, ranging from 50% to 70% depending on the judge and case specifics. Hearings can take 1 to 2 years to schedule. At a hearing, the person can present evidence, testify, and have legal representation.

The final appeal level is to the Appeals Council. If this is denied, a person may file suit in federal court, which is rare and typically pursued only with legal representation. Throughout all appeals, the SSA continues to review medical evidence and may request updated medical records or new evaluations.

Practical takeaway: The SSDI process involves multiple review stages and can take several years from application to approval. Having complete medical documentation from the beginning can support the review process.

Income, Work Rules, and Benefit Amounts

SSDI benefit amounts are based on a person's earnings record. The SSA calculates a Primary Insurance Amount (PIA) using a formula that considers average indexed monthly earnings. In 2024, the average SSDI benefit was approximately $1,537 per month, though individual benefits range from about $623 to $3,822 depending on work history. People who earned higher wages during their working years typically receive higher SSDI payments.

A key feature of SSDI is the ability to work while receiving benefits through a program called Trial Work Period (TWP). During the TWP, which lasts 9 months, a person can work and earn any amount without affecting their SSDI benefits. This allows someone to test whether they can sustain work before risking their benefits. The 9 work months do not need to be consecutive — they can be spread over 60 months.

After Trial Work Period ends, SSDI beneficiaries enter the Extended Period of Eligibility (EPE), which lasts 36 months. During EPE, if earnings exceed SGA ($1,550 in 2024 for non-blind individuals), benefits stop for that month. However, benefits can restart if earnings later drop below SGA. This provides a safety net for people whose income fluctuates.

Beneficiaries can also use several work incentives. The Plan to Achieve Self-Support (PASS) allows someone to set aside income and resources for a work goal
