Understanding RSV and Why Seniors Need Protection

Respiratory Syncytial Virus, or RSV, is a common virus that spreads through respiratory droplets when an infected person coughs or sneezes. Most people encounter RSV at some point in their lives, and younger, healthier individuals often recover without serious complications. However, RSV poses significantly greater risks for older adults, particularly those aged 60 and above.

According to the Centers for Disease Control and Prevention (CDC), RSV causes an estimated 160,000 hospitalizations and 6,000 to 10,000 deaths annually among adults aged 60 and older in the United States. This makes RSV a serious public health concern for the senior population. The virus attacks the respiratory tract, causing inflammation in the lungs and airways. In seniors, this inflammation can lead to severe pneumonia, bronchitis, or other serious complications.

Risk factors that increase severity of RSV infection in older adults include chronic lung disease (such as chronic obstructive pulmonary disease or COPD), heart disease, weakened immune systems, and diabetes. Seniors living in congregate settings like nursing homes face higher exposure risks. Even seniors who appear healthy can experience severe RSV infection because the aging immune system becomes less effective at fighting infections over time.

The symptoms of RSV in seniors often differ from symptoms in younger people. Older adults may experience fever, cough, wheezing, shortness of breath, and fatigue. Some seniors may have only mild respiratory symptoms but still develop serious lower respiratory tract disease. This is why prevention through vaccination has become an important health consideration for this age group.

Practical Takeaway: Understanding your personal risk for RSV helps inform conversations with your healthcare provider about whether vaccination may be appropriate for your situation.

The New RSV Vaccines Available for Seniors

As of 2023-2024, two RSV vaccines have been authorized by the Food and Drug Administration (FDA) for use in adults aged 60 and older. These are Arexvy (made by GSK, formerly GlaxoSmithKline) and Abrysvo (made by Pfizer). Both represent significant developments in preventing serious RSV disease in seniors, though they work through slightly different approaches.

Arexvy is a recombinant glycoprotein F (RSV F) vaccine. It contains a single protein component from the RSV virus that triggers the immune system to produce protective antibodies. The vaccine is administered as a single 0.5-milliliter injection into the muscle of the upper arm. Clinical trials showed that Arexvy reduced the risk of RSV lower respiratory tract disease by approximately 82% in adults aged 60 and older.

Abrysvo is a bivalent RSV prefusion F vaccine, meaning it targets the virus using a slightly different molecular form of the RSV F protein. This vaccine is also given as a single intramuscular injection. In clinical trials with adults aged 60 and older, Abrysvo demonstrated approximately 83% efficacy in preventing RSV lower respiratory tract disease. Additionally, Pfizer conducted studies in pregnant individuals showing that maternal vaccination could provide protection to newborns, though vaccination during pregnancy is a separate decision from senior vaccination.

Both vaccines work by stimulating the immune system to recognize and fight RSV before infection occurs. Neither vaccine contains the live virus itself, so they cannot give you RSV. The vaccines help your body's immune system produce antibodies that can neutralize the virus if you encounter it. Because these are relatively new vaccines, researchers continue to gather data on how long protection lasts and whether booster doses might be needed in future years.

Practical Takeaway: Both available RSV vaccines for seniors show strong effectiveness rates in clinical trials, with protection against severe RSV disease in the range of 82-83%.

How RSV Vaccines Compare to Other Senior Vaccinations

Seniors typically receive multiple vaccines to stay protected against various diseases. Understanding how RSV vaccines fit into the broader vaccination picture can help you and your healthcare provider make informed decisions about your vaccination plan.

Unlike the annual flu shot, RSV vaccines are administered as a single dose. Flu vaccines must be updated yearly because the flu virus changes frequently, and immunity from the previous year's vaccine wanes. RSV vaccines, by contrast, contain components that remain relevant because RSV changes less dramatically than influenza. Current information suggests that one RSV vaccine dose may provide lasting protection, though long-term data is still being collected as these vaccines are relatively new.

The pneumococcal vaccines (such as Pneumovax 23 and the newer Pneumococcal Conjugate vaccines) protect against bacterial pneumonia caused by Streptococcus pneumoniae, which is different from the viral pneumonia that RSV can cause. Some seniors may develop both bacterial and viral pneumonia simultaneously, so these different vaccines protect against different pathogens. The shingles vaccine (Shingrix) protects against herpes zoster and is unrelated to RSV protection.

RSV vaccines can generally be given at the same time as other vaccines, though they would be administered as separate injections in different arms. The COVID-19 vaccine and updated flu vaccines are also typically given during the same visit as RSV vaccination. However, your healthcare provider will review your specific vaccination history and health status to determine the best timing and sequence for your vaccines.

The CDC and FDA do not currently recommend that all seniors receive RSV vaccination. Instead, these organizations have indicated that RSV vaccination may be considered for adults aged 60 and older, particularly those with chronic conditions or higher risk factors. This differs from universal recommendations for flu shots in seniors, making individual medical consultation especially important for RSV vaccine decisions.

Practical Takeaway: RSV vaccines work alongside, not instead of, other senior vaccinations and require individual discussion with your healthcare provider rather than being universally recommended.

Side Effects and Safety Considerations

Clinical trials of both RSV vaccines included monitoring for side effects in thousands of participants. Understanding common side effects and serious adverse events helps you know what to expect and when to contact a healthcare provider.

The most frequently reported side effects of RSV vaccines are injection-site reactions and mild systemic effects. Injection-site reactions include redness, swelling, and tenderness at the spot where the needle entered the arm. These reactions typically appear within a day or two and resolve within a few days without treatment. Systemic side effects—those affecting your whole body—have included muscle aches, fatigue, headache, and low-grade fever. These effects are generally mild and temporary, usually resolving within one to three days.

In clinical trials, serious adverse events were rare. The FDA and CDC continue to monitor vaccine safety through systems like VAERS (Vaccine Adverse Event Reporting System) and VSD (Vaccine Safety Datalink). Healthcare providers are trained to recognize and report any unexpected or serious reactions that occur after vaccination. Seniors should contact their healthcare provider if they experience symptoms that concern them or that do not resolve within the expected timeframe.

Certain medical conditions may make RSV vaccination less suitable or require special consideration. Seniors with severe allergies to any vaccine component should inform their healthcare provider before vaccination. If you have had a previous allergic reaction to any vaccine, discuss this with your doctor. People with acute illness, moderate to severe fever, or certain immunocompromised conditions should consult with their healthcare provider about timing of vaccination.

Because RSV vaccines are new, healthcare providers continue to learn about which seniors may benefit most and which may need to avoid vaccination. This is an area where personalized medical guidance from your own doctor is particularly valuable. Your healthcare provider knows your complete medical history and can weigh the benefits of protection against RSV disease against any individual risk factors you may have.

Practical Takeaway: Common side effects are typically mild and short-lived, but discuss your complete health history with your healthcare provider before vaccination to identify any personal factors that might affect your decision.
