Learn About Nursing Homes and How They Work

What Is a Nursing Home and Who Lives There

A nursing home, also called a skilled nursing facility (SNF), is a residential facility that provides medical care and personal assistance to people who cannot live independently. According to the Centers for Medicare & Medicaid Services, there are approximately 15,600 nursing homes in the United States, serving roughly 770,000 residents. These facilities range in size from small homes with 40 beds to large operations with over 200 beds.

Nursing homes serve people with various needs. Some residents are recovering from surgery or hospitalization and need temporary care before returning home. Others have chronic illnesses like heart disease, diabetes, or COPD that require ongoing medical monitoring. Many residents have dementia or Alzheimer's disease and need supervision and assistance with daily activities. Some people enter nursing homes after strokes, accidents, or other events that affect their mobility and independence. The average length of stay varies widely—some people stay a few weeks, while others remain for years.

The residents in nursing homes come from all backgrounds and socioeconomic levels. About 9 out of 10 nursing home residents are over age 65, though younger people with disabilities also live in these facilities. Women make up roughly two-thirds of the nursing home population, partly because women typically live longer than men. Many residents had active careers and full lives before entering a facility. They come from professional backgrounds like teaching, business, healthcare, and skilled trades. Understanding that nursing home residents are diverse individuals helps family members and visitors approach their care with respect and sensitivity.

Practical Takeaway: Nursing homes serve people of different ages and backgrounds who need medical care or assistance beyond what family members can provide at home. They are not just end-of-life facilities—many residents stay temporarily while recovering or managing treatable conditions.

Types of Care and Services Provided

Nursing homes provide several layers of care tailored to different needs. Skilled nursing care is the most intensive level and includes services like wound care, medication management, catheter care, IV therapy, and physical rehabilitation. This type of care requires a licensed nurse or doctor to oversee treatment. According to the National Center for Health Statistics, approximately 63% of nursing home residents receive skilled nursing services. Skilled care is often what someone needs immediately after hospitalization—for example, a patient recovering from hip replacement surgery might need wound care, pain management, and physical therapy to regain mobility.

Intermediate care sits between skilled nursing and personal care. It includes basic medical support such as monitoring vital signs, managing chronic diseases, and assisting with medications. Residents at this level can participate more in self-care but still need staff support. Personal care or custodial care is the support with daily living activities. This includes bathing, dressing, grooming, toileting, and eating. While personal care sounds simple, it requires training in how to safely assist someone who may be frail, have mobility issues, or experience confusion.

Beyond medical and personal care, nursing homes offer many support services. Dietary services provide meals that meet nutritional needs and medical restrictions. Many facilities now have registered dietitians who create meal plans for residents with diabetes, heart disease, or swallowing difficulties. Activity programs include crafts, games, music, exercise classes, outings, and social events. These activities maintain mental stimulation and social connection. Most facilities offer occupational therapy and physical therapy to help residents maintain or improve their ability to perform daily tasks. Mental health services, including counseling and psychiatric care, are available at many facilities. Some facilities also offer specialized services like wound care centers, rehabilitation programs, or memory care units for people with dementia.

Practical Takeaway: Nursing homes provide a range of services from intensive medical care to help with daily activities, plus activities and therapies designed to maintain quality of life. When considering a facility, ask specifically about the services available for your loved one's particular health conditions.

How Staffing and Daily Operations Work

Nursing homes operate with a structured hierarchy of healthcare professionals. A medical director, usually a physician, oversees the clinical aspects of care. A nursing director or director of nursing manages all nursing staff and patient care policies. Registered nurses (RNs) have four-year degrees and handle complex medical tasks and supervision. Licensed practical nurses (LPNs) complete a shorter program and work under RN supervision to provide direct patient care. Certified nursing assistants (CNAs) help with bathing, dressing, toileting, and other personal care tasks. The Bureau of Labor Statistics reports that nursing homes employ approximately 750,000 certified nursing assistants, making them the largest group of direct care workers.

A typical day in a nursing home follows a schedule designed around residents' needs and preferences. Most facilities wake residents between 6 and 8 a.m. for breakfast. Morning routines include assisting with bathing, dressing, toileting, and oral care. Around mid-morning, some residents receive therapies like physical therapy or occupational therapy. Lunch is typically between 11:30 a.m. and 1 p.m. Afternoons may include activities, rest time, doctor visits, or visitors. Dinner is usually between 5 and 6 p.m. Evening routines include another round of personal care assistance, medications, and preparation for bed. Night shift staff monitor residents, respond to call lights, and handle any medical emergencies.

Staffing ratios vary by facility and state regulations. Federal regulations require at least one registered nurse on site at all times during the day and a licensed nurse (RN or LPN) on night shift. However, advocacy groups often note that these minimum ratios may not reflect the actual need for care. Residents can require assistance throughout the day and night, so staffing needs depend on how many residents need help and the complexity of their care. Some residents need help multiple times per day with basic tasks, while others need intensive monitoring for medical conditions. Staff also spend time on documentation, medication management, wound care, and communication with families and doctors. Many facilities struggle with staff turnover, which can affect continuity of care for residents.

Practical Takeaway: Nursing homes employ nurses, nursing assistants, and other healthcare professionals organized in a clear chain of command. Ask about staffing ratios and turnover rates when evaluating a facility, as adequate staffing directly affects the quality of care your loved one receives.

Payment Methods and Cost Structure

Nursing home care is expensive. According to 2023 data, the average cost of nursing home care in the United States ranges from $6,000 to $8,500 per month, with costs higher in some states like New York and California. These monthly costs cover room, board, meals, basic care, and facility operations. Specialized services like rehabilitation therapy, wound care, or specialized dementia care may cost more. Most families pay through a combination of methods rather than one single source.

Medicare, the federal health insurance program for people 65 and older, covers some nursing home costs under specific conditions. Medicare covers up to 100 days of skilled nursing care following a hospital stay of at least three days, but only when the care is medically necessary. Importantly, Medicare does not cover custodial or personal care that is not part of skilled nursing recovery. Many people mistakenly believe Medicare will pay for long-term nursing home stays; it will not. Medicare coverage typically ends when the resident no longer requires skilled nursing care or reaches the 100-day limit.

Medicaid is a joint state-federal program that does cover long-term nursing home care for people with limited income and assets. Unlike Medicare, Medicaid covers custodial care and can pay for years-long stays. Each state sets its own Medicaid policies, so coverage rules vary by location. Many nursing home residents rely on Medicaid to pay for their care. Private insurance policies sometimes include coverage for nursing home care, though this is becoming less common. Most families pay out-of-pocket for at least part of the nursing home costs. Some use long-term care insurance, a specialized insurance product designed to cover nursing home and home care expenses. Veterans and their spouses may be eligible for Veterans Affairs benefits that help cover nursing home costs. Families often use a combination of these payment sources, drawing down savings while pursuing other coverage options.

Practical Takeaway: Nursing home costs are substantial and rarely covered by one single payment source. Understanding which programs cover which services—Medicare for short-term skilled care, Medicaid for long-term or custodial care, and private payment for the remainder—helps families plan financially for this expense.
