Understanding MELD Scores: What They Measure

The MELD score stands for Model for End-Stage Liver Disease. It is a numerical rating system that doctors use to measure how sick a person's liver is and how urgently they might need a transplant. The score ranges from 6 to 40, with higher numbers indicating more severe liver disease.

The MELD score calculation uses three blood test results. The first is the International Normalized Ratio (INR), which measures how well the blood clots. A higher INR means the liver is not making clotting factors properly. The second measurement is bilirubin, a yellow substance that builds up when the liver cannot process it correctly. The third is creatinine, which measures kidney function because advanced liver disease often affects the kidneys too.

Doctors combine these three measurements using a specific mathematical formula to create the MELD score. The score reflects the patient's risk of dying from liver disease within the next three months without a transplant. This is important because it helps determine the order in which patients receive donor livers. A person with a MELD score of 35 is sicker and has higher priority for transplant than someone with a score of 20.

Different MELD score ranges tell different stories about liver health. A score between 6 and 9 suggests the liver disease is mild. Scores from 10 to 19 indicate moderate disease. A score of 20 to 29 shows more advanced disease, and scores of 30 or higher indicate very severe liver disease requiring urgent care.

The MELD score was developed in 2000 by researchers at the Mayo Clinic and became the official tool for organ allocation in the United States in 2002. It replaced an older system that was based mainly on how long patients had been waiting for a transplant. The new system prioritized based on actual medical need.

Practical Takeaway: Understanding your MELD score and what the number means helps you have informed conversations with your medical team about your liver health and what to expect.

How MELD Scores Are Calculated

The MELD score formula looks complicated, but understanding its basic parts helps clarify why your doctor ordered certain blood tests. The calculation uses natural logarithms and specific coefficients for each of the three measurements: INR, bilirubin, and creatinine.

The INR test measures how many seconds it takes for blood to clot compared to normal blood. When the liver is damaged, it produces fewer clotting proteins. An INR of 1.0 is normal. A person with liver disease might have an INR of 2.0, 3.0, or higher. The MELD calculation gives more weight to higher INR values because severe clotting problems signal serious liver damage.

Bilirubin is measured in milligrams per deciliter (mg/dL) of blood. Normal bilirubin is less than 1.2 mg/dL. In advanced liver disease, bilirubin can rise to 10, 20, or even higher levels. High bilirubin causes yellowing of the skin and eyes, a condition called jaundice. The MELD score reflects that elevated bilirubin is a sign of liver failure.

Creatinine measures kidney function through blood tests. Normal creatinine is around 0.7 to 1.3 mg/dL depending on the laboratory. In end-stage liver disease, kidney failure often develops. The MELD calculation includes creatinine because patients with both liver and kidney failure have poorer outcomes.

The actual MELD formula is: 3.78 × ln(bilirubin) + 11.2 × ln(INR) + 9.57 × ln(creatinine) + 6.43. The "ln" means natural logarithm, which is a mathematical function. However, patients do not need to calculate this themselves. Hospitals and clinics have computer systems that automatically calculate MELD scores from the blood test results.

MELD scores are typically updated regularly, sometimes monthly or more often for patients on transplant waiting lists. Changes in blood test results lead to changes in the MELD score. If treatment improves liver function, the score may decrease. If the liver worsens, the score increases.

Practical Takeaway: Your MELD score changes based on your blood test results, which is why doctors order these tests regularly to track your liver disease status over time.

Conditions That Affect Your MELD Score

Many different liver diseases can result in high MELD scores. Cirrhosis, which is scarring of the liver, is one of the most common reasons for high scores. Cirrhosis develops over years from various causes and eventually prevents the liver from functioning properly.

Hepatitis C virus infection causes chronic liver inflammation and damage. Before effective treatments became available around 2014, hepatitis C was a leading reason people needed liver transplants. Today, medications can cure hepatitis C in most patients, which can improve liver function and lower MELD scores.

Hepatitis B virus also causes long-term liver damage and cirrhosis in some patients. Unlike hepatitis C, hepatitis B cannot be cured, but antiviral medications can slow or stop liver damage.

Alcohol-related liver disease develops from years of heavy drinking. It ranges from fatty liver disease to alcoholic hepatitis to cirrhosis. Stopping alcohol use can sometimes allow the liver to heal, particularly in earlier stages.

Non-alcoholic fatty liver disease (NAFLD) occurs when fat builds up in the liver without heavy alcohol use. It is increasingly common in people with obesity and diabetes. Some people develop inflammation and scarring, a condition called non-alcoholic steatohepatitis (NASH).

Primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC) are autoimmune conditions where the body's immune system damages the bile ducts in the liver. These diseases develop slowly but can lead to cirrhosis.

Hemochromatosis is a genetic condition where the body absorbs too much iron, which damages the liver over time. Autoimmune hepatitis occurs when the immune system attacks the liver. Wilson's disease is a rare genetic condition where copper builds up in the liver and brain.

Acute liver failure, where the liver stops working suddenly over days or weeks, also results in very high MELD scores and requires urgent care. Causes include acetaminophen overdose, drug reactions, autoimmune hepatitis, and viral infections.

Practical Takeaway: Many different conditions can cause high MELD scores, and each condition has different treatment options, so knowing your specific diagnosis helps guide your medical care decisions.

What Your MELD Score Means for Your Health

Your MELD score provides doctors with a number that predicts your risk without a liver transplant. A score of 15 or higher generally indicates that the benefit of transplantation outweighs the risks of surgery and lifelong immunosuppression medications. Below 15, the risk of waiting for a transplant is usually lower than the surgical risk.

MELD scores in the range of 15 to 19 mean a person has a roughly 4% risk of dying within three months without a transplant. Scores of 20 to 24 increase the risk to about 7% over three months. Scores of 25 to 29 raise the risk to approximately 15%, and scores of 30 and above indicate a risk of over 20% in just three months. These statistics show why MELD score matters for prioritizing donor organs.

A high MELD score means your liver is failing and you need closer medical monitoring. You may need to visit your doctor more frequently, take more medications, or adjust your diet and activities. It may mean reducing salt intake, monitoring fluid retention, taking diuretics, or taking medications to prevent bleeding from enlarged veins in the stomach and esophagus.

Your MELD score also affects your quality of life. As liver disease progresses, people often experience fatigue, mental confusion (called hepatic encephalopathy), fluid buildup in the abdomen (ascites), and bleeding from the digestive tract. Managing these complications becomes more intensive as the MELD score rises.
