Understanding Melanoma and How It Develops

Melanoma is a type of skin cancer that starts in cells called melanocytes. These are the cells in your skin that produce melanin, the pigment that gives your skin its color. While melanoma is less common than other types of skin cancer, it is more serious because it spreads to other parts of the body more often. According to the American Cancer Society, an estimated 99,780 new cases of melanoma were expected to be diagnosed in the United States in 2022, with about 7,650 deaths from the disease.

The development of melanoma typically begins with changes in melanocytes. These cells may start to grow abnormally due to damage from ultraviolet (UV) radiation from the sun or from tanning beds. Unlike normal skin cells that grow in an orderly way, melanoma cells grow in a disorganized manner and can accumulate in the skin. The process of becoming melanoma can take years or even decades, though in some cases it may happen more rapidly.

Several factors influence how melanoma develops. UV exposure is the primary environmental risk factor. People with fair skin, a family history of melanoma, many moles, or a personal history of severe sunburns have increased risk. Age also plays a role—while melanoma can occur at any age, it is most common in adults over 50. However, it is also one of the most common cancers in people under 30, particularly in women.

Understanding the basic development process helps explain why early detection matters. When melanoma is caught and treated in its early stages, when it has not spread beyond the skin, the five-year survival rate is around 99 percent. This contrasts sharply with melanoma that has spread to distant organs, where the five-year survival rate drops to about 27 percent, according to the National Cancer Institute.

Practical Takeaway: Learning about how melanoma develops can help you recognize why sun protection and skin monitoring are important. Melanoma growth often starts with UV damage to skin cells, a process that typically takes time to develop into cancer.

The Early Stages: Melanoma in Situ and Localized Growth

Melanoma in situ is the earliest stage of melanoma. At this stage, the cancerous cells are present only in the outer layer of the skin, called the epidermis, and have not invaded deeper layers. This is also called Stage 0 melanoma. The cancer cells have not spread to lymph nodes or other organs. When caught at this stage, melanoma is highly treatable, with cure rates exceeding 95 percent after surgical removal.

In the localized stage, melanoma may have begun to grow into the dermis, the layer of skin beneath the epidermis. This is Stage 1 melanoma. At this point, the tumor is still confined to the skin where it originated and has not spread to nearby lymph nodes or distant sites. The thickness of the melanoma, measured in millimeters, becomes important here. Thin melanomas (less than 1 millimeter thick) have better outcomes than thicker ones. The Breslow thickness, named after the pathologist who developed the measurement, is one of the most important factors in determining prognosis.

During early-stage melanoma, there may be no symptoms other than a change in the appearance of a mole or the development of a new pigmented spot on the skin. The ABCDE rule helps identify suspicious changes: Asymmetry (one half does not match the other), Border irregularity (ragged or uneven edges), Color variation (multiple colors in one lesion), Diameter (larger than a pencil eraser, about 6 millimeters), and Evolution (any change in size, shape, or color over time). A spot that fits several of these criteria warrants examination by a dermatologist.

The timeline for early-stage melanoma varies widely. Some melanomas grow slowly over years, while others progress more rapidly. Research published in the journal JAMA Dermatology found that the median time from first appearance to diagnosis was about 5 years for melanoma patients, though this varied considerably. Growth rate can be influenced by the specific genetic mutations present in the cancer cells, the person's immune system, and other individual factors.

Practical Takeaway: Early-stage melanoma often shows no symptoms besides changes in a mole's appearance. Monitoring your skin for changes using the ABCDE rule and having suspicious spots evaluated promptly increases the likelihood of catching melanoma when it is most treatable.

Intermediate Stages: When Melanoma Spreads to Lymph Nodes

Stage 2 melanoma refers to a thicker primary tumor (typically more than 1 millimeter) that remains localized to the skin but may be at higher risk of spreading. Stage 3 melanoma involves spread to regional lymph nodes or in-transit metastases (cancer cells appearing in the skin between the original tumor and nearby lymph nodes). At this point, the cancer has left the original site but has not yet reached distant organs.

The progression to stage 3 represents a significant shift in the disease. When melanoma cells spread to nearby lymph nodes, it means the cancer has begun to travel through the lymphatic system. Lymph nodes are part of the body's immune system and act as filters for fluid and cells. When cancer cells reach these nodes, they can grow there and potentially continue traveling to other parts of the body. Approximately 5-10 percent of melanoma patients present with stage 3 disease at diagnosis, according to cancer registries.

The timeline for melanoma to progress from stage 1 to stage 3 can vary greatly. Some patients may have their primary melanoma removed and never experience spread to lymph nodes. Others may develop lymph node involvement relatively quickly—within months to a few years. Factors that influence this timeline include the aggressiveness of the specific melanoma cells (determined by genetic characteristics), the person's immune response, and the location of the primary tumor. Melanomas on the trunk and lower extremities tend to have different patterns of spread than those on the face or arms.

During the intermediate stages, patients typically undergo additional testing to determine if spread has occurred. This may include sentinel lymph node biopsy, in which a surgeon removes and examines the first lymph node(s) to which cancer would likely spread. Imaging tests like CT scans or PET scans may also be used. These tests help determine the stage and guide treatment decisions. The five-year survival rate for stage 3 melanoma ranges from about 40-79 percent, depending on how many lymph nodes are involved and other prognostic factors.

Practical Takeaway: When melanoma spreads to regional lymph nodes, it becomes more serious but may still be treated with surgery and other therapies. Understanding whether spread has occurred requires additional testing, which is why staging is an important part of melanoma evaluation.

Advanced Stages: Melanoma That Spreads to Distant Sites

Stage 4 melanoma, also called metastatic melanoma, means the cancer has spread to organs beyond the lymph nodes closest to the original tumor. Common sites of metastasis include the brain, lungs, liver, and bones. Metastatic melanoma is more difficult to treat than earlier stages, though newer treatment options have improved outcomes in recent years. Approximately 4 percent of melanoma patients are diagnosed with stage 4 disease, though some earlier-stage patients develop metastatic disease later.

The timeline from primary melanoma diagnosis to metastatic disease varies considerably. Some patients may have metastatic disease present at the time of their initial diagnosis. Others may have a disease-free period of months or even years before distant metastases develop. Research indicates that the median time to development of metastatic disease, when it occurs, is roughly 2-3 years after initial diagnosis, though some sources report longer intervals. This variation reflects differences in tumor biology and individual patient factors.

Metastatic melanoma grows in the specific organ environments where it has spread. When melanoma reaches the brain, for example, it may cause neurological symptoms. Lung metastases might cause respiratory symptoms or be found incidentally on imaging. Liver metastases can affect organ function. The presence and location of metastases, along with the number of involved organs, helps determine prognosis. According to the American Cancer Society, the median overall survival for patients with metastatic melanoma was approximately 8-12 months with older treatment approaches, though newer immunotherapies and targeted treatments have extended survival for some patients to several years or
