Learn About Mastectomy Procedures and Operating Room Basics

Understanding Mastectomy: What It Is and Why It's Performed

A mastectomy is a surgical procedure in which a surgeon removes all or part of breast tissue. This operation is one of the most common cancer treatments in the United States, with over 150,000 mastectomies performed annually according to the American Cancer Society. The procedure may be recommended for people diagnosed with breast cancer, those at very high risk of developing it, or in rare cases, for other medical reasons.

There are several types of mastectomies, and the specific type recommended depends on individual medical circumstances. A total (or simple) mastectomy removes all breast tissue but leaves the skin, areola, and nipple intact in most cases. A modified radical mastectomy removes all breast tissue along with lymph nodes under the arm. A skin-sparing mastectomy preserves more of the breast skin to allow for reconstruction. A nipple-sparing mastectomy keeps the nipple and areola while removing breast tissue underneath. Surgeons choose the approach based on cancer stage, location, size, and whether reconstruction is planned.

The reasons for recommending mastectomy vary. For people with breast cancer, it may be the primary treatment or combined with other therapies like radiation or chemotherapy. For those with a very high genetic risk (such as BRCA1 or BRCA2 mutations), a preventive mastectomy may reduce cancer risk by up to 95 percent. Some individuals choose this option even without a cancer diagnosis based on family history and personal preference.

Before surgery, patients typically meet with their surgical team to discuss which type of mastectomy is appropriate for their situation. Questions to ask include whether lymph nodes will be removed, if reconstruction is an option, what the recovery timeline looks like, and what complications are possible. Understanding the specific procedure planned helps set realistic expectations for the hospital experience and recovery period.

Practical Takeaway: There is no single mastectomy procedure—the approach varies based on medical need and personal preferences. Learning which type is being recommended and why provides important context for preparing for surgery.

The Operating Room Environment and Setup

Modern operating rooms are highly controlled environments designed to keep patients safe during surgery. An operating room for a mastectomy typically contains specialized equipment, multiple staff members, and strict protocols to prevent infection and manage complications. The room temperature is maintained at a cool 62-68 degrees Fahrenheit to reduce the risk of bacterial growth and help keep the patient's body temperature stable during the procedure.

The surgical team typically includes the breast surgeon, an anesthesiologist or nurse anesthetist, surgical nurses, a surgical technician, and sometimes other specialists. The breast surgeon has specialized training in cancer treatment and reconstruction techniques. The anesthesiologist manages pain control and keeps the patient unconscious and comfortable throughout the procedure. Surgical nurses and technicians handle instruments, monitor equipment, and assist the surgeon. This team coordination is essential for patient safety.

The operating room contains several key pieces of equipment. The surgical table is adjustable and positions the patient to give the surgeon optimal access to the breast area. Monitors display vital signs including heart rate, blood pressure, oxygen levels, and breathing patterns throughout the surgery. Anesthesia equipment delivers medications and oxygen. Surgical lights provide bright illumination of the surgical area. Electrocautery equipment seals blood vessels to reduce bleeding. Suction devices remove fluids from the surgical area so the surgeon has a clear view of what they're working on.

Before entering the operating room, patients go through preparation steps. An IV line is placed in the arm to deliver fluids and medications. The surgical site is marked and verified—correct site surgery protocols require the surgeon to mark the area being operated on with the patient while awake. The patient is positioned on the surgical table, and anesthesia is administered. Once the patient is asleep and properly positioned, the surgical team prepares the skin with antibacterial solutions and drapes the area with sterile cloths, leaving only the surgical site exposed.

Practical Takeaway: The operating room is a organized space with trained personnel and specialized equipment all working together to keep the patient safe. Understanding the basic setup and team roles can reduce anxiety about the surgical environment.

Pre-Operative Preparation and What to Expect

Preparation for a mastectomy typically begins several weeks before the scheduled surgery date. Patients receive written instructions about what to do before arriving at the hospital. Generally, patients are instructed to stop eating and drinking at midnight the night before surgery—this fasting requirement prevents stomach contents from entering the lungs during anesthesia. Even water, gum, and hard candies must be avoided. Some medications may be continued with a small sip of water, while others are stopped days in advance. The surgical team provides a specific medication list indicating which to take and which to skip.

In the days before surgery, patients typically have a pre-operative appointment with their surgical team. During this visit, medical history is reviewed, blood tests are performed to ensure the patient is healthy enough for surgery, and an EKG (heart tracing) may be done if the patient is over 50 or has heart conditions. Patients discuss any allergies, current medications, supplements, and previous reactions to anesthesia. This information is crucial because some supplements and medications can increase bleeding risk or interact with anesthesia.

The night before surgery, most patients are instructed to shower or bathe using a special antibacterial soap provided by the hospital, or they may use regular soap and water. On the morning of surgery, patients should shower again with the antibacterial soap if instructed. Jewelry, watches, rings, and piercings should be removed. Makeup, nail polish, and nail extensions should be taken off so medical staff can monitor skin color and nail bed perfusion during surgery. Patients should wear loose, comfortable clothing that's easy to remove, as they'll change into a hospital gown upon arrival.

Arrival time is typically 1-2 hours before the scheduled surgery. At the hospital, patients check in, provide identification and insurance information, and are taken to a pre-operative area. Here, nurses place an IV line, review medications and allergies one final time, and answer any remaining questions. The anesthesiologist or nurse anesthetist meets with the patient to discuss anesthesia options and risks. The surgeon visits to confirm the surgical plan and answer final questions. This may feel rushed, but it's an appropriate time to mention any new concerns or changes since the initial consultation.

Practical Takeaway: Pre-operative preparation involves specific instructions that should be followed closely—fasting, medication adjustments, and hygiene routines all contribute to a safer surgery. Writing down questions beforehand helps patients get their concerns addressed during pre-operative visits.

The Mastectomy Procedure: Step-by-Step Overview

Once the patient is asleep under anesthesia, the surgical team positions them on the operating table, typically lying on their back with the arm on the side of the surgery extended outward. The breast area is cleansed with antiseptic solution multiple times to reduce bacteria on the skin. Sterile drapes are placed around the surgical site, creating a sterile field. Only the breast area and surrounding skin remain exposed to the surgical team.

The surgeon makes an incision to access the breast tissue. The location and length of the incision depend on the type of mastectomy and whether reconstruction is planned. For a total mastectomy, the incision is typically horizontal (running side to side) or follows the natural skin lines of the breast. The surgeon carefully cuts through the skin and underlying tissue to reach the breast tissue. Electrocautery seals small blood vessels as they're cut to minimize bleeding and improve visibility of the surgical area.

The surgeon then separates the breast tissue from the underlying muscle and chest wall. This requires careful technique to preserve the muscle if reconstruction is planned later. In a modified radical mastectomy, the surgeon removes lymph nodes from under the arm through the same incision or sometimes through a separate smaller incision. A sentinel lymph node biopsy may be performed—this involves injecting dye or a radioactive tracer to identify which lymph nodes the cancer cells are most likely to drain to, allowing the surgeon to remove only those specific nodes rather than all nodes in the area.

As the tissue is removed, the surgeon checks the margins—the edge of the remaining tissue—to ensure no cancer cells remain. The surgeon may send tissue samples to the pathology lab during surgery (called intraoperative consultation) to verify margins are clear. Once the surgeon is satisfied that all breast tissue has been removed, hemostasis is achieved—meaning all bleeding is controlled. The incision is closed layer by
