Understanding Lyme Disease and Why Testing Matters

Lyme disease is an infection caused by bacteria called Borrelia burgdorferi, which is spread through tick bites. According to the Centers for Disease Control and Prevention (CDC), there are approximately 300,000 cases of Lyme disease reported in the United States each year, though some estimates suggest the actual number may be higher. The disease is most common in the Northeast, Mid-Atlantic, and Upper Midwest regions, particularly during warmer months when ticks are most active.

The infection begins when an infected tick bites a person and transmits the bacteria into the bloodstream. Early symptoms often include a characteristic circular rash called erythema migrans, which appears in about 70-80% of infected people. However, not everyone develops this rash, and symptoms can vary widely from person to person. Some people experience flu-like symptoms such as fever, chills, fatigue, body aches, and headaches within 3 to 30 days of infection.

If left untreated, Lyme disease can progress to more serious complications affecting the joints, heart, and nervous system. Late-stage Lyme disease may cause arthritis, neurological problems, and heart rhythm issues. This progression highlights why early detection through testing is important. Testing allows healthcare providers to confirm whether symptoms are actually caused by Lyme disease and to start treatment promptly if needed.

Understanding your testing options is a key part of managing potential Lyme disease exposure. Different tests work in different ways, and the timing of testing affects which tests are most useful. Some tests work better early in infection, while others are more reliable several weeks after symptoms begin. This guide explores the various testing options available so you can discuss them with your healthcare provider.

Practical Takeaway: If you believe you may have Lyme disease, knowing about testing options before you visit your healthcare provider can help you have an informed conversation about which test might be appropriate for your situation.

Two-Tier Testing: The Standard Approach to Lyme Disease Diagnosis

The CDC and most medical organizations recommend a two-step testing process for Lyme disease diagnosis. This approach combines two different types of tests to increase accuracy and reduce false results. Understanding how this two-tier system works can help you understand what to expect if your healthcare provider recommends testing.

The first tier consists of an ELISA test, which stands for enzyme-linked immunosorbent assay. This test looks for antibodies that your immune system produces in response to Borrelia burgdorferi bacteria. Antibodies are proteins your body creates when fighting an infection. The ELISA is relatively quick, inexpensive, and widely available at most laboratories. However, the ELISA has limitations. Early in infection (within the first week or two), antibodies may not have developed yet, so the test could return a negative result even though infection is present. Additionally, ELISA tests can sometimes produce false positives, meaning they indicate Lyme disease when it is not actually present.

The second tier uses a Western blot test, which also detects antibodies but in a more specific way. The Western blot separates proteins from the Borrelia burgdorferi bacteria and tests whether antibodies react to specific bacterial proteins. This test is more specific than ELISA, meaning it is better at confirming that a positive ELISA result actually indicates Lyme disease. The CDC criteria for a positive Western blot require that certain protein bands appear on the test results. At least five specific protein bands must be present for a positive result to be considered reliable.

The two-tier approach works this way: if your ELISA test is negative, your healthcare provider may stop testing there, as the likelihood of Lyme disease is considered low. If your ELISA is positive or borderline, the Western blot is performed on the same blood sample. A positive ELISA followed by a positive Western blot confirms Lyme disease diagnosis. This system helps prevent both false positives and false negatives, making it the most widely recommended testing method by organizations including the CDC, American College of Rheumatology, and Infectious Diseases Society of America.

Practical Takeaway: If your healthcare provider orders Lyme disease testing, ask whether they are using the two-tier approach, as this is the standard method recommended by major medical organizations.

Early Testing: When Standard Tests May Not Work

One of the biggest challenges with Lyme disease testing is timing. The antibodies that tests look for don't appear immediately after infection occurs. This means that if you test too early, you may get a negative result even though you have Lyme disease. Understanding this timing is crucial for interpreting test results accurately.

During the first week of infection, antibody levels are typically too low to detect with standard ELISA or Western blot tests. In fact, studies show that standard two-tier testing misses approximately 50% of early Lyme disease cases in the first week of illness. IgM antibodies, which are the first type of antibody your body produces, may take 3 to 4 weeks to reach detectable levels. IgG antibodies, which develop later, may take 4 to 6 weeks or longer to appear in sufficient quantities to show up on tests.

For this reason, if you have symptoms consistent with early Lyme disease—particularly if you have the characteristic erythema migrans rash and remember a recent tick bite—healthcare providers sometimes recommend starting treatment based on clinical symptoms rather than waiting for test confirmation. The CDC acknowledges that treatment based on clinical presentation, especially in endemic areas during tick season, can be appropriate even before test results are available.

Alternative testing options for early-stage disease exist but are not widely available through standard laboratories. Some specialized laboratories offer tests that detect bacterial DNA directly, such as PCR (polymerase chain reaction) testing. These tests look for actual bacterial genetic material rather than antibodies. However, PCR testing is not routinely recommended by the CDC for Lyme disease diagnosis because the bacteria may not always be present in sufficient quantities in blood samples even when infection is present.

Another consideration is C6 peptide testing, which some laboratories offer. This test detects antibodies against a specific component of the Borrelia burgdorferi bacteria called the C6 peptide. Some research suggests this test may detect infection somewhat earlier than standard ELISA tests, but it is not part of the CDC-recommended two-tier protocol and is not widely covered by insurance.

Practical Takeaway: If you suspect early Lyme disease based on symptoms and tick exposure, discuss with your healthcare provider whether treatment might be recommended before test results are available, rather than waiting for antibodies to develop.

Testing After Initial Symptoms: Reliability and Interpretation

Once you are past the initial week or two of infection, standard two-tier testing becomes more reliable. This is when most people seek medical attention anyway, as symptoms typically become noticeable within 3 to 30 days of infection. By this point, antibodies have usually developed to levels that tests can detect.

For someone in the second to eighth week of illness, the two-tier ELISA and Western blot approach works well and is highly accurate when both tests are positive. However, interpretation still requires care. A positive ELISA alone is not diagnostic; the Western blot confirmation is necessary. Additionally, a positive test does not indicate when infection occurred—it only shows that at some point you were exposed to Borrelia burgdorferi bacteria. Some people may test positive from previous infections or in rare cases from laboratory error.

One important consideration is that after treatment for Lyme disease, antibodies remain in your blood for months or even years. This means that a positive test does not distinguish between active infection and past infection. If you were treated for Lyme disease months ago and later test positive again, this positive result does not necessarily mean you have a new infection or that the previous infection was not cured. Conversely, if you were recently treated and tested, a positive result is expected and does not indicate treatment failure.

This fact creates challenges for people who believe they have recurring Lyme disease or "chronic Lyme disease." Standard antibody tests cannot determine whether ongoing symptoms are from active infection or from other causes. Some organizations promote alternative tests or claim to diagnose persistent Lyme disease through non-standard methods, but these are not validated by major medical organizations. The Infectious Diseases Society of America and CDC maintain that once Lyme disease is treated with appropriate antibiotics, the infection is cured and persistent symptoms typically have other causes.
