Understanding Head Lice: What They Are and How Infestation Happens

Head lice are small insects about the size of a sesame seed that live on the human scalp and feed on blood. These parasites have been a problem for humans for thousands of years. According to the Centers for Disease Control and Prevention (CDC), head lice affect millions of people each year, with estimates suggesting between 6 and 12 million infestations occur annually in the United States alone, particularly among children ages 3 to 11.

Lice have six legs with claws designed to grip hair shafts. They cannot jump or fly, but they move quickly across the scalp. Adult lice typically live about 30 days on a person's head. During this time, female lice lay eggs called nits. These nits are cemented to hair shafts close to the scalp and hatch within 7 to 10 days. The nits are often easier to see than the adult lice themselves, appearing as small, oval-shaped, tan or white clusters attached to individual hairs.

Transmission happens through direct head-to-head contact with someone who has lice. Contrary to popular belief, lice do not spread through dirty hair or poor hygiene. Clean hair is actually just as susceptible to lice as dirty hair. Lice may also spread through sharing personal items like combs, brushes, hats, scarves, pillows, or headphones, though direct contact is the most common way infestation occurs. Animal lice cannot infect humans, so pets cannot be a source of human head lice.

Common signs of lice infestation include itching on the scalp, neck, and behind the ears. Some people notice small red bumps or sores from scratching. The itching sensation may take several weeks to develop after infestation begins, so a person can have lice and not realize it during the early stages. Finding nits or live lice during a careful examination of the scalp is the most reliable way to confirm an infestation.

Practical takeaway: Lice infestation is not a sign of poor cleanliness or hygiene. If you suspect lice, examine the scalp carefully, particularly behind the ears and at the nape of the neck where lice and nits are most commonly found.

Over-the-Counter Treatment Options

Over-the-counter (OTC) lice treatments are available without a prescription and are often the first option people try. The most common active ingredient in OTC treatments is permethrin, a synthetic insecticide. Permethrin-based products are typically applied to damp hair and left on the scalp for 10 minutes, then rinsed out. Many brands recommend a second application 7 to 10 days after the first treatment to eliminate lice that may have hatched from nits after the initial application.

Another OTC option contains pyrethrins, which are natural insecticides derived from chrysanthemum flowers. Pyrethrin-based treatments work similarly to permethrin products but break down more quickly on the scalp. These products are often applied for 10 minutes and then rinsed out.

In recent years, newer OTC products have entered the market with different mechanisms of action. Some products contain dimethicone, an ingredient that works by coating and suffocating lice rather than using insecticide chemicals. These products may appeal to people concerned about chemical exposure. Benzyl alcohol is another non-insecticide option used in some OTC treatments.

It is important to note that lice resistance to traditional treatments like permethrin has become increasingly common. The CDC and dermatologists report cases where lice do not respond to OTC treatments, particularly in certain geographic regions. This means that even when instructions are followed correctly, OTC products may not eliminate lice in every situation.

When using OTC treatments, following package directions exactly is critical. This includes applying the product for the specified amount of time, using enough product to coat all hair, and completing follow-up treatments as directed. Failure to follow directions correctly is one reason OTC treatments sometimes fail. Additionally, treating the infested person alone may not be sufficient if other household members also have lice; all infested individuals should be treated.

Practical takeaway: OTC treatments can work, but success depends on using the correct product for the correct amount of time. If an OTC treatment does not eliminate lice after two applications, prescription options may be necessary.

Prescription Lice Treatments

When OTC treatments fail or lice resistance is suspected, healthcare providers may recommend prescription medications. One of the most commonly prescribed options is malathion. Malathion is an organophosphate insecticide that is applied to damp hair, left to air dry naturally, and remains on the scalp for 8 to 12 hours. This longer contact time allows it to be effective against both lice and nits without requiring a second application in most cases. However, malathion has a strong odor and takes time to dry, which can be inconvenient for some people.

Ivermectin is another prescription option that works differently from traditional topical treatments. It is taken as an oral medication rather than applied to the scalp. A dose is given, and a second dose is typically administered 7 to 10 days later. Ivermectin is absorbed into the bloodstream and kills lice through ingestion when they feed on blood. The CDC lists ivermectin as an acceptable treatment option, though it is not as frequently prescribed as other alternatives.

Spinosad is a prescription treatment derived from naturally occurring soil bacteria. It is applied to damp hair for 10 minutes and then rinsed out. Spinosad is effective against both lice and their eggs, meaning a second application may not be necessary. One advantage of spinosad is that it may be less likely to encounter resistance issues compared to longer-used chemicals, though resistance cases have been reported.

Benzyl alcohol lotions are available by prescription in higher concentrations than OTC versions. These work by suffocating lice and are considered a lower-toxicity option. Like other topical treatments, they are applied to the scalp and hair for a specified time before rinsing.

The choice of prescription treatment depends on factors such as the patient's age, any allergies or sensitivities, previous treatment attempts, and local lice resistance patterns. A healthcare provider can make recommendations based on individual circumstances.

Practical takeaway: If OTC treatments have not worked after two attempts, contacting a healthcare provider about prescription options is the next logical step. Different prescription medications have different application methods and timelines, so understanding how to use the prescribed medication correctly is essential.

Non-Chemical and Home Remedies

Many people search for non-chemical alternatives to treat lice, and several methods are discussed in medical and educational literature. One method involves using a fine-tooth comb specifically designed for lice (called a nit comb) to manually remove lice and nits from the hair. This method requires patience and regular combing sessions, typically every 2 to 3 days for 2 to 3 weeks. While combing alone can reduce lice populations, research suggests it is most effective when combined with other treatments rather than used alone.

Petroleum jelly, mayonnaise, and coconut oil are home remedies that some people apply to the scalp under the theory that they suffocate lice by blocking their breathing. While these substances may cause discomfort to lice, scientific evidence supporting their effectiveness is limited. A study published in the journal Pediatric Dermatology examined various oils and found mixed results, with some oils showing modest effectiveness but none matching the results of standard medical treatments.

Wet combing involves using a regular comb through wet hair to remove lice, often combined with hair conditioner. This method is sometimes recommended as a way to monitor for lice or reduce populations, but like manual combing alone, it is not considered a reliable standalone treatment method.

The National Association of School Nurses and other health organizations note that non-chemical methods are time-intensive and their effectiveness is difficult to verify. Some families choose these methods for personal reasons, but they require significant commitment and careful monitoring to work. If non-chemical methods are chosen, follow-up checks and treatments may be necessary if lice persist after one to two weeks.
