Learn About Kidney Stones in Women

What Are Kidney Stones and How Do They Form

Kidney stones are hard mineral deposits that form inside your kidneys when certain substances in your urine become concentrated. These substances include calcium, oxalate, uric acid, and phosphate. When your urine contains high levels of these minerals and you don't drink enough water, crystals can form and stick together over time, creating stones that range in size from a grain of salt to a golf ball.

The formation process typically happens in stages. First, crystals begin to form when minerals and salts accumulate in your urine. These tiny crystals are too small to cause problems, but as more minerals deposit around them, the crystals grow larger. Eventually, the stone becomes large enough to cause symptoms or block urine flow. This process can take weeks, months, or even years to develop.

According to the National Kidney Foundation, approximately 1 in 11 people in the United States will experience a kidney stone at some point in their life. Women are less likely to develop kidney stones than men—about 1 in 16 women compared to 1 in 12 men—but the rates in women have been increasing over the past 20 years. Studies show that African American women have a lower rate of kidney stones than white women, while Hispanic women fall somewhere in between.

Several risk factors increase the likelihood of stone formation. Dehydration is one of the most common causes because concentrated urine provides the perfect environment for crystal formation. Medical conditions like urinary tract infections, hyperparathyroidism, and certain metabolic disorders can also increase your risk. Family history plays a role too—if your parents or siblings had kidney stones, your chances are higher. Additionally, certain medications and dietary choices can contribute to stone development.

Practical Takeaway: Understanding how kidney stones form helps explain why staying hydrated and maintaining certain dietary habits are so important for prevention. If you have a family history of kidney stones or notice symptoms, this information can help you recognize when to seek medical evaluation.

Symptoms Women Experience With Kidney Stones

Kidney stone symptoms can vary significantly depending on the stone's size and location. Many people don't experience any symptoms until the stone starts moving through the urinary tract—a process called passing the stone. This is when symptoms typically become noticeable and sometimes severe.

The most common symptom is sudden, severe pain on the side or back of your body, below the ribs. This pain, called renal colic, can be extraordinarily intense and may come in waves. The pain might start in your back and move toward your lower abdomen and groin as the stone travels down the urinary tract. Women sometimes describe it as the worst pain they've ever experienced. The severity of pain doesn't always match the stone's size—even small stones can cause intense discomfort.

Other symptoms that may accompany kidney stone pain include nausea and vomiting, which occur in about 50% of people with kidney stones. You might also notice changes in your urine, such as pink, red, or brown discoloration caused by blood in the urine. Some people experience painful urination, a constant urge to urinate, or only passing small amounts of urine at a time. Fever may develop if the stone causes a urinary tract infection.

For women, kidney stone symptoms can sometimes be confused with gynecological issues like ovarian cysts or pelvic inflammatory disease because of the pain location. However, kidney stone pain is typically more constant and severe, while gynecological pain may be more crampy or related to your menstrual cycle. If you experience sudden, severe pain in your side or back along with urinary changes, medical evaluation is important to determine the cause.

The timing of symptom onset can vary. Some people never develop symptoms because the stone passes out of the body without causing obstruction. Others develop acute symptoms within hours as the stone travels through narrow passages in the urinary tract. If a stone becomes lodged in the ureter—the tube carrying urine from your kidney to your bladder—it may cause persistent pain and potential complications.

Practical Takeaway: Knowing the typical symptoms helps you recognize when to seek medical care. Sudden severe pain in your side or back, especially with urinary changes or nausea, warrants prompt medical attention to rule out kidney stones and other serious conditions.

Risk Factors Specific to Women

While men develop kidney stones more frequently overall, women face specific risk factors that can increase their stone formation risk. Understanding these factors helps women make informed choices about prevention and health monitoring.

Pregnancy and the postpartum period represent significant risk times for kidney stones in women. During pregnancy, hormonal changes increase urinary calcium excretion while decreasing urinary citrate—a substance that normally inhibits stone formation. The growing uterus can also put pressure on the ureters, slowing urine flow and allowing minerals to concentrate. Statistics show that pregnant women have a 2-3 times higher risk of developing kidney stones compared to non-pregnant women. The second and third trimesters carry the highest risk. Women who have had kidney stones during pregnancy have a higher likelihood of recurrence with future pregnancies.

Hormonal changes related to menstruation and menopause also affect kidney stone risk. Studies suggest that women who use hormone replacement therapy during menopause may have an increased risk of kidney stones. Some research indicates that postmenopausal women have higher stone formation rates than premenopausal women, though this relationship is still being studied.

Urinary tract infections are more common in women than men due to differences in anatomy, and frequent UTIs can increase kidney stone risk. Some types of UTIs are associated with stone-forming bacteria that can lead to infected stones. Women with chronic UTIs should be particularly attentive to prevention strategies.

Certain medications more commonly used by women can increase stone risk. These include some forms of vitamin D supplements, excessive vitamin C supplementation, and certain diuretics. Women with inflammatory bowel disease, which is more common in women than men, also face higher kidney stone risk due to increased urinary oxalate.

Dietary factors that are sometimes more common in women's eating patterns can contribute to stone formation. Low fluid intake combined with high sodium consumption, excessive protein intake, or a diet high in oxalate-rich foods can all increase risk. Additionally, some women restrict their calcium intake believing it prevents stones, when actually adequate calcium intake helps prevent stone formation by binding oxalate in the digestive system.

Practical Takeaway: Women should be particularly vigilant about hydration, especially during pregnancy and if taking hormone replacement therapy. Maintaining adequate calcium intake, managing UTIs promptly, and monitoring supplement use can significantly reduce kidney stone risk.

Diagnosis and Medical Evaluation

When you experience symptoms suggesting kidney stones, medical professionals use several diagnostic tools to confirm the diagnosis and determine the best treatment approach. Understanding these tests helps you know what to expect during evaluation.

The first step usually involves a physical examination and discussion of your symptoms and medical history. Your doctor will ask about the location and severity of pain, whether you've noticed blood in your urine, and whether you have a family history of kidney stones. This information helps guide which tests are most appropriate.

Imaging studies are crucial for kidney stone diagnosis. A CT scan, particularly a non-contrast helical CT scan, is the gold standard imaging test. It can detect stones as small as 1-2 millimeters and works regardless of stone composition. The scan also shows the stone's exact location and size, which helps determine treatment options. An ultrasound is another imaging option that doesn't use radiation and can also detect stones, though it may miss very small ones. X-rays can detect some types of stones but not all, so they're often used in combination with other imaging.

Laboratory tests on your urine and blood provide important information. A urinalysis shows whether blood, crystals, or infection-causing bacteria are present in your urine. A blood test measures kidney function and electrolyte levels to ensure your kidneys are working properly. If a kidney stone is passed or removed, analysis of the stone's composition helps identify what caused it and informs prevention strategies. Stone composition types include calcium oxalate (most common, comprising about 70-80% of stones), calcium phosphate, uric acid, struvite, and cystine stones, each requiring different prevention approaches.

For women with recurrent kidney stones—defined as having two or more episodes—additional metabolic testing may be recommended. This testing involves 24-hour urine collection to measure calcium,
