What Is Kidney Disease and How It Develops

Your kidneys are two bean-shaped organs, each about the size of a fist, that sit on either side of your spine below the rib cage. These organs perform one of the most important jobs in your body: filtering waste products and extra water from your blood to make urine. When your kidneys work properly, they remove about 120 to 150 quarts of blood daily to produce approximately 1 to 2 quarts of urine. Beyond filtering waste, your kidneys also regulate blood pressure, produce hormones that help create red blood cells, and activate vitamin D to keep bones strong.

Kidney disease develops when the kidneys lose their ability to filter waste effectively. According to the National Kidney Foundation, approximately 37 million American adults have chronic kidney disease, though many don't know it because early stages often have no symptoms. The disease progresses through five stages, ranging from mild kidney damage with normal or high filtration rates to complete kidney failure requiring dialysis or transplant.

Several conditions can damage the kidneys over time. Diabetes is the leading cause of kidney disease, accounting for about 30 to 40 percent of cases in the United States. High blood pressure is the second leading cause, responsible for 25 to 30 percent of cases. Other causes include glomerulonephritis (inflammation of the kidney's filtering units), inherited diseases like polycystic kidney disease, recurrent kidney infections, prolonged use of certain medications including some over-the-counter pain relievers, and autoimmune diseases like lupus.

Understanding how kidney disease develops matters because early detection can slow or prevent progression. The disease typically develops silently—a person can lose up to 90 percent of their kidney function before noticing any symptoms. This is why health organizations recommend regular kidney function screening for people with diabetes, high blood pressure, a family history of kidney disease, or those over age 60.

Practical takeaway: Learning your kidney numbers through regular blood and urine tests can identify kidney disease in early stages when treatment options are most effective. Ask your healthcare provider about getting tested if you have risk factors.

Recognizing Symptoms and Risk Factors

One of the most challenging aspects of kidney disease is that many people experience no symptoms until significant damage has occurred. When symptoms do appear, they often develop gradually and may be subtle enough to attribute to other conditions. Common signs include fatigue and weakness, caused by the buildup of waste products and anemia from insufficient kidney production of erythropoietin, a hormone that stimulates red blood cell formation. Swelling in the feet, ankles, hands, or face can occur when kidneys cannot regulate fluid balance properly.

Other symptoms people with kidney disease may experience include changes in urination patterns (going more or less frequently, or changes in appearance or amount of urine), difficulty concentrating or experiencing "brain fog" from uremic toxins, persistent itching from waste product accumulation in the blood, nausea and vomiting, muscle cramps, shortness of breath, and high blood pressure that's difficult to control. Some people develop a metallic taste in their mouth or notice they need less insulin if they have diabetes.

Risk factors that increase your likelihood of developing kidney disease include diabetes (the single largest preventable risk factor), high blood pressure, family history of kidney disease, older age (risk increases significantly after age 60), obesity, and smoking. Certain ethnic and racial groups, including African Americans, Hispanic Americans, Native Americans, and Asian Americans, have higher rates of kidney disease. This disparity relates to higher rates of diabetes and hypertension in these populations, as well as potential genetic factors and differences in healthcare access.

Additionally, certain medications and habits increase kidney disease risk. Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen can damage kidneys, especially with long-term use. Some antibiotics, particularly older ones, can harm kidney function. Prolonged dehydration strains the kidneys, as does excessive salt intake. People with autoimmune diseases, recurrent kidney stones, or a history of acute kidney injury face elevated risk.

Practical takeaway: Review your personal and family health history with your doctor. If you have any risk factors, discuss whether regular kidney function testing makes sense for you. Knowing your risk status allows you to make informed decisions about lifestyle changes and monitoring.

Understanding Kidney Disease Stages and Measurements

Doctors classify kidney disease into five stages based on how well your kidneys filter waste, measured by something called the glomerular filtration rate (GFR). The GFR is calculated from a simple blood test measuring creatinine, a waste product your muscles produce. This number estimates how many milliliters of waste your kidneys filter per minute per 1.73 square meters of body surface area. Understanding these stages helps you and your healthcare provider track disease progression and plan treatment.

Stage 1 kidney disease means you have normal or high GFR (90 or above) but show signs of kidney damage through protein in your urine or imaging abnormalities. Many people at this stage don't know they have kidney disease. Stage 2 involves mild reduction in GFR (60 to 89) with kidney damage. Stage 3 is divided into two parts: Stage 3a with GFR of 45 to 59, and Stage 3b with GFR of 30 to 44. Both involve moderate reduction in kidney function. At these stages, kidney disease may be detected for the first time during routine medical visits.

Stage 4 means severe reduction in kidney function with GFR between 15 and 29. People typically begin preparing for renal replacement therapy at this stage. Stage 5 is kidney failure with GFR below 15, requiring dialysis or kidney transplant to sustain life. According to the Centers for Disease Control and Prevention, about 1 in 3 American adults with chronic kidney disease don't know they have it, emphasizing the importance of understanding these stages.

Beyond GFR, doctors also measure albumin-to-creatinine ratio (ACR) in urine, which shows how much protein is leaking into your urine. Healthy kidneys don't allow significant protein to pass into urine, so protein presence indicates kidney damage. Blood pressure readings, blood potassium levels, blood phosphorus levels, and hemoglobin levels (to assess anemia) all provide important information about kidney health and guide treatment decisions.

Practical takeaway: Ask your healthcare provider to explain your specific GFR number and what stage you're at if you've been diagnosed with kidney disease. Understanding this number helps you track changes over time and participate meaningfully in treatment planning with your medical team.

Medical Treatment Options for Kidney Disease

Treatment for kidney disease depends on the underlying cause, how advanced the disease is, and your overall health. For people with diabetes, maintaining tight blood sugar control through medication, diet, and lifestyle changes can slow kidney damage significantly. Research shows that controlling blood pressure is equally important. Most people with kidney disease benefit from blood pressure medications called ACE inhibitors or angiotensin receptor blockers (ARBs), which not only lower blood pressure but also protect the kidneys specifically.

People with kidney disease often need additional medications to manage complications. Diuretics may reduce fluid buildup and high blood pressure. Phosphate binders taken with meals prevent phosphorus accumulation in the blood, which damages bones and blood vessels. Erythropoiesis-stimulating agents (ESAs) may be prescribed to address anemia by stimulating red blood cell production. Vitamin D supplements maintain bone health, and calcium supplements may be recommended depending on individual needs.

Dietary management plays a crucial role in slowing kidney disease progression. Healthcare providers may recommend limiting sodium intake to help control blood pressure, restricting phosphorus and potassium to prevent mineral imbalances, limiting protein to reduce kidney workload (though amount varies by individual stage), and controlling fluid intake in advanced stages. A kidney disease dietitian can provide personalized recommendations based on your specific numbers and medical situation.

For advanced kidney disease, renal replacement therapy becomes necessary. Hemodialysis uses a machine to filter waste and excess fluid from blood, typically done three times weekly for three to five hours. Peritoneal dialysis uses the lining of your abdomen to filter waste, performed either continuously (CAPD) or overnight with a machine (APD). Kidney transplantation offers another option, with transplanted kidneys lasting 10 to 20 years on average, though transplant requires finding a compatible donor
