Understanding Hemorrhoids: Causes and Symptoms

Hemorrhoids are swollen veins in the lower part of the rectum and anus. They affect millions of people in the United States each year, with estimates suggesting that about 1 in 20 Americans experience hemorrhoid symptoms at some point in their lives. Despite their prevalence, many people feel embarrassed discussing them, which can delay seeking treatment.

Hemorrhoids develop when increased pressure builds up in the veins around the anus and rectum. Several factors can contribute to this pressure. Chronic constipation is one of the most common causes, as straining during bowel movements puts extra stress on these delicate blood vessels. Pregnancy can also trigger hemorrhoids due to increased abdominal pressure and hormonal changes. Other contributing factors include sitting for long periods, particularly on the toilet; a low-fiber diet that leads to hard stools; heavy lifting; obesity; and aging, as the tissues supporting veins weaken over time.

There are two main types of hemorrhoids. Internal hemorrhoids occur inside the rectum, where they are usually painless but may cause bleeding. External hemorrhoids develop under the skin around the anus and often cause itching, pain, and swelling. Some people develop both types simultaneously, which is called mixed hemorrhoids.

Common symptoms include bright red blood on toilet paper or in the bowl after a bowel movement, itching around the anus, pain or discomfort during bowel movements, a lump or swelling near the anus, and mucus discharge. Symptoms can vary significantly from person to person. Some individuals experience only mild itching, while others have substantial bleeding or pain. Understanding your specific symptoms helps guide treatment decisions.

Takeaway: Keep track of your hemorrhoid symptoms for a few weeks before seeing a doctor. Note when symptoms occur, what makes them better or worse, and whether you see bleeding. This information helps your healthcare provider determine the best treatment approach for your situation.

At-Home Treatment Options That May Reduce Symptoms

Many people can manage mild to moderate hemorrhoid symptoms using treatments available at home. These approaches focus on reducing inflammation, promoting healing, and preventing future flare-ups. According to the American Gastroenterological Association, most hemorrhoids respond well to conservative treatment within one to two weeks.

Dietary changes form the foundation of home treatment. Increasing fiber intake helps soften stools and reduces straining during bowel movements. Doctors typically recommend consuming 25 to 35 grams of fiber daily through foods like whole grains, legumes, fruits, and vegetables. Examples include eating oatmeal for breakfast, snacking on berries, adding beans to lunch, and choosing whole-wheat bread. Drinking adequate water—typically 8 to 10 glasses daily—also softens stools and makes bowel movements easier. Some people find that gradually increasing fiber intake over several days, rather than all at once, reduces bloating and gas.

Topical treatments can provide relief from itching and discomfort. Over-the-counter creams and ointments containing ingredients like hydrocortisone, witch hazel, or phenylephrine may reduce inflammation and swelling. Witch hazel, a natural astringent, has been used traditionally for hemorrhoid relief. Hydrocortisone cream can reduce itching and inflammation, though it should not be used for extended periods without medical guidance. Some products combine multiple ingredients for enhanced relief. Always follow package directions and consult a pharmacist if you are uncertain about using these products alongside other medications.

Sitz baths are another effective home treatment. A sitz bath involves sitting in warm water for 10 to 15 minutes, several times daily and especially after bowel movements. You can use a regular bathtub filled with 3 to 4 inches of warm (not hot) water, or purchase a specialized sitz bath basin that fits over a toilet seat. Warm water promotes blood circulation, reduces muscle tension, and soothes discomfort. Adding Epsom salt or essential oils like lavender may enhance the soothing effect, though plain warm water alone works well.

Lifestyle adjustments also play a crucial role in managing hemorrhoids. Avoid sitting on the toilet for extended periods, as this increases pressure on hemorrhoidal veins. Try to have bowel movements within five to ten minutes. Use soft toilet paper or wet wipes, or rinse with water in a bidet if available. Wear loose, breathable cotton clothing to reduce irritation. Take breaks from sitting by standing, walking, or lying down when possible. Regular physical activity, such as a 20 to 30-minute daily walk, promotes healthy digestion and reduces constipation.

Takeaway: Start with dietary changes and increased water intake as your first home treatment step. Add sitz baths and over-the-counter topical treatments as needed. Most people see improvement within one to two weeks of consistent home care. Track which treatments work best for your body so you can adjust your routine accordingly.

Over-the-Counter Products and How to Use Them Safely

The over-the-counter hemorrhoid market includes numerous products, from creams to pads to suppositories. Understanding these options helps you make informed choices about treatment. The Food and Drug Administration oversees these products to ensure safety and accurate labeling, though people should still follow instructions carefully and know when to stop using a product.

Topical creams and ointments represent the most popular over-the-counter category. Common active ingredients include hydrocortisone (a mild steroid that reduces inflammation), phenylephrine (a decongestant that shrinks swollen tissue), and witch hazel (an astringent that may reduce irritation). Products like Preparation H, Anusol, and Hemorrhoidal HC all work through these mechanisms. Most creams should be applied to the affected area up to four times daily or after bowel movements. Some people apply them with their fingers, while others use applicator tubes included in packages. Always wash your hands after application unless you are using an applicator.

Medicated pads or wipes offer convenient alternatives to creams. These typically contain witch hazel, hydrocortisone, or other soothing ingredients. Brands like Tucks pads are widely available and can be used after each bowel movement. Many people prefer pads because they are cleaner to apply than creams and do not stain clothing. Simply unfold the pad and gently wipe the affected area. You can also use these pads to apply moisture and cleansing action while sitting on the toilet.

Suppositories are inserted into the rectum to deliver medication directly to internal hemorrhoids. Common ingredients include cocoa butter, glycerin, and hydrocortisone. Suppositories may feel strange to use initially, but they are straightforward: remove the wrapper, wet the suppository tip slightly if desired, insert it gently into the rectum, and remain seated for a few minutes. Most people use suppositories once or twice daily, typically in the morning or evening. Suppositories may be especially useful if you have internal hemorrhoids causing pain or bleeding.

Important safety considerations apply to all over-the-counter products. Do not use any product for longer than seven days without consulting a doctor, as prolonged use of topical steroids can thin the skin. If you have other medical conditions or take medications, mention your hemorrhoid treatment to your doctor or pharmacist, as some interactions are possible. Avoid using multiple products simultaneously unless your doctor recommends it, as this can irritate the area further. If symptoms worsen or new symptoms develop, stop using the product and see your healthcare provider. Pregnant or breastfeeding women should check with their doctor before using any new product.

Takeaway: Choose one product type to start—either a cream, pad, or suppository—based on whether your hemorrhoids are external or internal. Use it consistently for three to seven days while also implementing dietary and lifestyle changes. If symptoms persist beyond seven days, or if bleeding is heavy, contact your doctor rather than continuing self-treatment.

When to See a Doctor About Your Hemorrhoids

While many hemorrhoids improve with home treatment, certain situations warrant a medical evaluation. Knowing when to see a doctor prevents complications and ensures you receive appropriate care. Roughly 75 percent of people with hemorrhoid symptoms never see a healthcare provider, yet professional evaluation can rule out other conditions and provide treatment options beyond home care.
