Learn About Early Signs of Bowel Cancer

What Is Bowel Cancer and How Common Is It?

Bowel cancer, also called colorectal cancer, develops in the colon or rectum. The colon is the large intestine that helps your body absorb water and nutrients from food. The rectum is the last part of the colon that stores stool before it leaves your body. When cells in these areas grow abnormally and out of control, they can form tumors.

Bowel cancer is one of the most common cancers worldwide. According to the American Cancer Society, about 1 in 23 men and 1 in 25 women will develop colorectal cancer during their lifetime. In 2024, approximately 106,000 new cases of colon cancer and 46,000 cases of rectal cancer are expected in the United States. The good news is that when bowel cancer is found early, before it spreads to other parts of the body, survival rates are much higher. About 90% of people whose bowel cancer is detected at an early stage survive at least five years.

Bowel cancer typically develops slowly over many years. It often starts as small, harmless growths called polyps on the inside of the colon or rectum. Most polyps do not become cancer, but some types can change into cancer cells over time. This slow growth means there is often a window of time to find and treat the cancer before it becomes serious.

Risk increases with age. Most people diagnosed with bowel cancer are over 50 years old. However, cases in younger people are rising. Between 2000 and 2020, the rate of early-onset bowel cancer (in people under 55) increased by about 1-2% per year.

Practical takeaway: Understanding that bowel cancer develops slowly and is often preventable through screening makes knowing the early signs important for your health decisions.

Common Early Warning Signs You Should Know

Early-stage bowel cancer often causes no symptoms at all, which is why screening is so important. However, certain changes in your body may signal that something needs medical attention. Recognizing these signs and discussing them with your doctor is an important step in taking care of your health.

Changes in bowel habits that last more than a few weeks warrant attention. This includes diarrhea, constipation, or alternating between the two. You might also notice that your stools are narrower than usual, like a pencil or ribbon. The urge to have a bowel movement when your bowel is empty, or feeling like you haven't completely emptied your bowel, can also be early warning signs. These changes may feel minor, but if they persist for more than 2-4 weeks, they deserve a conversation with your healthcare provider.

Blood in or on your stool is another sign to take seriously. This might appear as bright red blood on the toilet paper, in the toilet bowl, or mixed into the stool itself. Some people notice dark, tar-like stools, which can indicate older blood from higher up in the digestive tract. While hemorrhoids (painful swollen veins in the rectum) are a more common cause of bright red bleeding, you should not assume this is the cause without medical evaluation.

Abdominal pain and bloating deserve attention if they continue. You might experience cramping, discomfort, or a feeling of fullness even when you haven't eaten much. Some people describe persistent gas or bloating that doesn't go away with dietary changes.

Unexplained weight loss can be a sign of bowel cancer, especially if you haven't been trying to lose weight. Losing 10 pounds or more without changes to your diet or exercise habits should be discussed with your doctor. This happens because cancer can affect how your body uses nutrients and can cause loss of appetite.

Fatigue or weakness that doesn't improve with rest may also indicate a problem. Anemia (low red blood cell count) caused by bleeding in the digestive tract can leave you feeling tired and short of breath.

Practical takeaway: Write down any changes in your bowel habits, appetite, weight, or energy levels and bring this information to your next doctor's appointment. Most of these symptoms have other causes, but your doctor needs to know about them to provide proper care.

Risk Factors That Increase Your Chances

Knowing your risk factors helps you understand why screening and awareness matter. Some risk factors you cannot change, while others relate to lifestyle choices you can modify.

Age is the strongest risk factor. Most bowel cancer diagnoses occur in people over 50. However, as mentioned earlier, rates in people under 50 are increasing, suggesting other factors beyond age play a role.

Family history matters significantly. If a parent, sibling, or child has had bowel cancer or polyps, your risk increases. Having two or more family members diagnosed before age 50 raises concern even more. If your family has had bowel cancer, your doctor may recommend earlier or more frequent screening.

Inflammatory bowel diseases like Crohn's disease and ulcerative colitis increase bowel cancer risk. People with these conditions have ongoing inflammation in their digestive tract for years, which can lead to cellular changes. If you have one of these conditions, discuss screening plans with your gastroenterologist.

Lifestyle factors play a substantial role. Diets high in red meat (beef, pork, lamb) and processed meats (bacon, sausage, deli meats) are linked to higher risk. Research shows that eating 50 grams of processed meat daily (about two slices of deli meat) increases bowel cancer risk by about 18%. Physical inactivity, obesity, smoking, and heavy alcohol use all increase risk as well.

Diabetes may increase risk. Studies show people with type 2 diabetes have a moderately higher risk of developing bowel cancer, possibly related to insulin resistance and inflammation.

Certain genetic conditions carry very high risk. Familial adenomatous polyposis (FAP) causes hundreds of polyps to develop, making cancer nearly certain without treatment. Lynch syndrome (hereditary nonpolyposis colorectal cancer, or HNPCC) causes about 2-4% of all bowel cancers. If you know your family carries these genes, genetic counseling and specialized screening are important.

Practical takeaway: Review your personal and family health history. If you have multiple risk factors, discuss with your doctor whether standard screening guidelines apply to you or whether earlier or more frequent screening makes sense for your situation.

How Bowel Cancer Develops: From Normal Cells to Cancer

Understanding how bowel cancer develops helps explain why early detection is so valuable. The process usually takes many years, giving you time to catch problems before they become serious.

The journey typically begins with a polyp. A polyp is a small growth on the inner lining of the colon or rectum. Most polyps are benign (not cancer) and never cause problems. However, certain types of polyps can slowly change into cancer. The most concerning polyps are adenomas. About 30-40% of adults over 50 have at least one adenoma, but only a small fraction ever become cancer.

The transformation from normal cells to cancer cells happens through accumulated genetic damage. Cells normally have built-in controls that prevent them from growing out of control and dividing forever. When DNA damage occurs—sometimes from inflammation, sometimes from inherited genetic changes, sometimes from lifestyle factors—these controls can fail. This doesn't happen overnight. Scientists believe it typically takes 10-15 years for a polyp to develop into invasive cancer. This slow timeline is the reason screening works so well.

During this progression, the polyp may grow larger. Small polyps (less than 6 millimeters) have almost no cancer risk. Medium polyps (6-9 millimeters) have a small risk. Larger polyps have higher risk. This size progression happens gradually over years.

Eventually, if a polyp is not removed, it may transform into early cancer that affects only the inner lining of the colon or rectum. At this stage, called carcinoma in situ, the cancer cells have not invaded deeper layers. If found now, treatment is often very successful with high survival rates.

Over time, cancer may grow deeper into the bowel wall, affecting muscle and connective tissue layers. Later, it may spread to nearby lymph nodes or organs. The stage of cancer (how far it has spread) at
