Understanding Dry Mouth: What Happens in Your Body

Dry mouth, medically known as xerostomia, occurs when your salivary glands don't produce enough saliva to keep your mouth moist. Saliva plays a critical role in your oral health and overall well-being. It helps you chew food, swallow, and digest meals. Saliva also contains protective substances that fight bacteria and fungi, preventing tooth decay and oral infections. When saliva production drops, these protective functions diminish, leaving your mouth vulnerable to various problems.

Your body produces about one to one and a half liters of saliva daily under normal circumstances. This production happens constantly, but increases when you eat, chew gum, or experience stress. Three major salivary glands—the parotid glands near your ears, submandibular glands under your jaw, and sublingual glands under your tongue—work together to maintain this flow. When these glands malfunction or produce less saliva than needed, dry mouth develops.

The sensation of dry mouth can range from mildly uncomfortable to severely problematic. Some people describe it as a sticky feeling, while others report difficulty speaking or eating. The condition can be temporary, lasting a few hours or days, or chronic, persisting for months or years. Understanding whether your dry mouth is occasional or ongoing helps determine what might be causing it and what steps you can take.

Dry mouth affects people of all ages, though it becomes more common as people get older. Research shows that approximately 10% of the general population experiences dry mouth, but this number increases to 25-30% in people over age 65. This age-related increase occurs because salivary gland function naturally declines with age, and older adults often take multiple medications that can affect saliva production.

Practical takeaway: Pay attention to how often your mouth feels dry and when it happens. Noting patterns—such as whether dryness occurs all day, only at night, or after certain activities—provides valuable information for understanding potential causes and discussing the issue with a healthcare provider.

Common Medical Causes of Dry Mouth

Numerous medical conditions can trigger dry mouth by affecting salivary gland function. Diabetes stands out as one of the most common culprits. When blood sugar levels remain high over time, they can damage the salivary glands and reduce their ability to produce saliva. People with poorly controlled diabetes frequently report dry mouth as a bothersome symptom. Similarly, thyroid disorders, particularly hypothyroidism (underactive thyroid), can reduce saliva production. The thyroid gland influences many body functions, including how much saliva your body makes.

Autoimmune diseases represent another significant category of dry mouth causes. Sjögren's syndrome specifically targets salivary and tear glands, causing both dry mouth and dry eyes. In this condition, the immune system mistakenly attacks the glands that produce moisture, leading to their gradual destruction. Rheumatoid arthritis and lupus can also cause dry mouth as part of their systemic effects. People with these conditions should monitor their mouth moisture and report changes to their healthcare team.

Cancer treatment frequently causes temporary or permanent dry mouth. Chemotherapy drugs can damage salivary glands, and radiation therapy to the head and neck area often causes significant salivary gland damage. The extent of dryness depends on the type and intensity of treatment. Many people undergoing cancer treatment experience this side effect, and healthcare providers typically discuss it beforehand so patients can prepare and manage it effectively.

Neurological conditions also play a role. Parkinson's disease, stroke, Bell's palsy, and other nervous system disorders can interfere with the nerve signals that trigger saliva production. When nerves don't communicate properly with salivary glands, the glands may produce less saliva or none at all. Additionally, conditions affecting the mouth structure itself, such as scleroderma (a disease affecting connective tissue), can make it difficult for saliva to flow normally even if glands produce adequate amounts.

Infections and inflammatory conditions deserve mention as well. Oral thrush (a fungal infection) often develops in people with dry mouth because the protective antibodies in saliva aren't present in sufficient quantities. Gum disease and mouth sores become more common when saliva levels drop. Some people with HIV/AIDS experience dry mouth due to both the condition itself and medications used to treat it.

Practical takeaway: If you have a chronic medical condition, ask your healthcare provider whether dry mouth is a known side effect or complication. Understanding the connection between your condition and dry mouth helps you anticipate the problem and take preventive measures.

Medications That Can Cause Dry Mouth

Medications represent one of the most common causes of dry mouth, particularly in older adults who take multiple prescriptions daily. More than 400 medications list dry mouth as a possible side effect. Understanding which of your medications might contribute to dryness helps you work with your healthcare provider to find solutions. It's important to note that medications that cause dry mouth can still be necessary and beneficial—the goal is managing the side effect, not necessarily stopping the medication.

Antihistamines used to treat allergies and cold symptoms frequently cause dry mouth. Common over-the-counter antihistamines like diphenhydramine and loratadine reduce mucus production throughout the body as part of how they work. Decongestants, often combined with antihistamines in cold medicines, have similar drying effects. People who regularly take allergy or cold medications may notice their mouth feels particularly dry during allergy season or when they have a cold.

Antidepressants and anti-anxiety medications commonly cause dry mouth as a side effect. Selective serotonin reuptake inhibitors (SSRIs), tricyclic antidepressants, and benzodiazepines can all reduce saliva production. For people managing depression, anxiety, or other mental health conditions, this side effect must be weighed against the medication's benefits. Many people adapt to this side effect over time, or healthcare providers can adjust dosages or switch to alternative medications with fewer mouth-drying properties.

Blood pressure medications, particularly diuretics (water pills) and certain ACE inhibitors, frequently cause dry mouth. These medications work by affecting fluid balance in the body, which can reduce saliva production along with other body fluids. People with high blood pressure taking these medications should mention dry mouth to their healthcare provider, as alternative medications might be available that control blood pressure equally well with fewer mouth-drying effects.

Pain medications, especially opioids, are well-known for causing dry mouth. Anticonvulsants used for seizure disorders or nerve pain, muscle relaxants, and chemotherapy drugs all frequently cause dryness. Radiation therapy and certain cancer treatments damage salivary glands directly. Additionally, medications for urinary incontinence, Parkinson's disease, and certain psychiatric conditions can reduce saliva production.

Over-the-counter medications matter too. Pain relievers, cough drops, and other OTC products can contribute to dry mouth. Some people don't realize that regularly using multiple medications—both prescription and over-the-counter—can compound the drying effect. Keeping a complete list of all medications you take, including vitamins, supplements, and over-the-counter products, helps healthcare providers identify potential culprits.

Practical takeaway: Review your medication list with your pharmacist or healthcare provider, specifically asking which medications might cause dry mouth. Don't stop taking prescribed medications without guidance, but discuss alternatives or additional strategies for managing the dryness.

Lifestyle and Environmental Factors Contributing to Dry Mouth

Beyond medical conditions and medications, various lifestyle and environmental factors can reduce saliva production or increase mouth dryness. Dehydration ranks as one of the most straightforward causes. When your body doesn't have enough water, it conserves fluids for vital organs, and saliva production decreases. This can happen during illness, hot weather, strenuous exercise, or simply not drinking enough water throughout the day. People who drink primarily caffeinated or alcoholic beverages instead of water may experience chronic mild dehydration and associated dry mouth.

Breathing patterns significantly affect mouth moisture. People who breathe through their mouths rather than their noses—whether due to nasal congestion, habit, or sleep-related issues like sleep apnea—often experience dry mouth. Mouth breathing allows saliva to evaporate more rapidly from the oral tissues. This problem intensifies at night when many people naturally breathe through their mouths while sleeping. Those
