Learn About Cortisol Testing and Results

What Cortisol Is and Why It Matters

Cortisol is a hormone your body makes naturally in two small glands called the adrenal glands, which sit on top of your kidneys. Your body produces cortisol throughout the day, and it plays several important roles in keeping you healthy. Cortisol helps regulate your blood sugar levels, manages inflammation, controls your blood pressure, and affects how your body responds to stress.

When you experience stress—whether physical stress from illness or exercise, or emotional stress from work or personal situations—your body releases more cortisol. This is part of your natural "fight or flight" response. Cortisol tells your heart to pump faster, sharpens your focus, and gives your muscles energy to respond to the stressful situation. This system worked well for our ancestors who faced physical dangers, but in modern life, many people experience ongoing stress that keeps cortisol levels elevated.

Cortisol follows a daily pattern called a circadian rhythm. Your cortisol levels are typically highest in the early morning, which helps you wake up and get out of bed. Throughout the day, cortisol gradually decreases, reaching its lowest point around midnight. This pattern helps regulate your sleep-wake cycle and energy levels. Understanding this natural rhythm is important because cortisol testing often requires timing your sample collection at specific times of day to get accurate results.

Problems can arise when cortisol levels stay too high or too low for extended periods. High cortisol levels over time may contribute to weight gain, muscle weakness, mood changes, sleep problems, and weakened immunity. Low cortisol levels can cause fatigue, low blood pressure, and difficulty managing stress. Because cortisol affects so many body systems, doctors sometimes order cortisol tests when patients report unexplained symptoms.

Practical Takeaway: Cortisol is a normal hormone that your body needs to function. Knowing that cortisol levels naturally change throughout the day and during stressful situations helps you understand why timing matters for testing.

Types of Cortisol Tests Available

Several different tests can measure cortisol levels, and doctors choose different tests depending on what information they need. Each test type measures cortisol in a different way and provides different information about how your body is producing this hormone.

The 24-hour urine cortisol test is one of the most commonly used cortisol tests. For this test, you collect all your urine over a 24-hour period in a special container provided by your healthcare provider. This test measures the total amount of cortisol your kidneys filter into your urine during an entire day. Because it captures cortisol over a full day, it gives doctors a picture of your overall cortisol production. This test is often recommended first because it tends to be more reliable than a single blood test and is less affected by the stress of getting tested.

Blood cortisol tests measure cortisol directly in your blood. These tests can be done at different times of day. An 8 a.m. cortisol blood test measures cortisol when it should be at its highest point according to your natural rhythm. A late-night or midnight cortisol test measures cortisol when it should be at its lowest. Some doctors order both morning and evening blood tests to see if your cortisol pattern is normal. Blood tests give quick results but can be affected by the stress and anxiety many people feel during blood draws, which can temporarily raise cortisol levels.

Saliva cortisol tests involve collecting saliva samples, usually at home, at specific times throughout the day—typically morning, afternoon, evening, and sometimes at midnight. You place saliva on special collection materials and send them to a laboratory. Saliva testing avoids the stress response that sometimes happens during blood draws, and it accurately reflects cortisol levels in your blood. Some research suggests saliva tests may be more accurate for detecting abnormal cortisol patterns, though both blood and saliva tests are considered acceptable.

Hair cortisol testing is a newer method where a small sample of hair is tested to measure average cortisol levels over approximately three months. This test reflects long-term cortisol exposure rather than a single moment in time. Hair testing is becoming more popular because it provides information about chronic stress exposure and requires no specific timing.

Practical Takeaway: Different cortisol tests measure different things—24-hour urine tests measure total daily production, blood tests measure cortisol at specific times, saliva tests track patterns throughout the day, and hair tests show long-term levels. Your doctor will recommend the test type that best fits your situation.

How Cortisol Testing Works and What to Expect

Understanding what to expect during cortisol testing helps you prepare and ensures you get accurate results. The process varies depending on which type of test your doctor orders, and following instructions carefully is important because improper sample collection can affect the accuracy of your results.

For a 24-hour urine collection test, your doctor's office will give you a large collection container and instructions. The process typically starts in the morning. You first urinate normally and discard that urine—this is important because you want to start fresh. Then, for the next 24 hours, you collect every bit of urine in the special container, including the first urine the next morning. You keep the container refrigerated or on ice throughout the collection period. Once complete, you return the full container to your doctor's office or laboratory. This test requires planning your day around the collection but is otherwise straightforward. Most people find the 24-hour collection manageable once they understand the instructions.

For blood cortisol tests, you visit a laboratory or your doctor's office. A healthcare worker draws a small blood sample from a vein in your arm, usually in the morning when cortisol should be highest. The entire process takes just a few minutes. If your doctor wants both morning and evening measurements, you may need two separate visits or appointments. Some labs offer late-evening appointments to accommodate late-night cortisol testing. The blood sample goes to a laboratory where it's analyzed. Results typically come back within a few days.

Saliva cortisol testing often happens at home, which many people find less stressful than going to a lab. Your healthcare provider sends you a collection kit with special collection materials and detailed instructions about timing. For a four-point cortisol test, you typically collect samples at waking, mid-afternoon, evening, and bedtime. You place saliva on collection materials according to instructions—this might mean spitting into a tube or letting saliva drip onto a collection stick. You usually mail the samples to the laboratory in a pre-addressed envelope. Results come back within one to two weeks. The challenge with saliva testing is remembering to collect samples at exactly the right times and following timing instructions precisely.

Hair cortisol testing requires cutting a small hair sample, usually about the thickness of a pencil, from near the scalp. This painless collection takes just a minute or two. You mail the sample to the laboratory, which then processes it to measure cortisol content. Results typically come back within two to three weeks.

Practical Takeaway: Each test type has different requirements, but all require following instructions carefully. Prepare by understanding which test you're having and what specific preparation or timing is needed for accurate results.

Preparing for Your Cortisol Test

How you prepare for cortisol testing can affect the accuracy of your results. Different preparation requirements exist depending on which test type you're having, and your healthcare provider should give you specific instructions. Understanding common preparation guidelines helps you get reliable results.

For any cortisol test, it's important to minimize unusual stress the day or two before testing. While you can't avoid all stress, extreme emotional stress or intense physical activity can raise cortisol levels temporarily and skew your results. If possible, plan testing for a day when your routine is relatively normal. However, you should still go about your day normally—the goal is to measure your typical cortisol levels, not to create an artificial low-stress situation.

Certain medications can affect cortisol measurements, so it's important to tell your healthcare provider about all medications and supplements you take, including over-the-counter medications, birth control pills, and herbal supplements. Medications like corticosteroids, which contain synthetic cortisol, will obviously affect test results. Even some other medications can influence cortisol levels. Your doctor may ask you to continue taking your regular medications or may recommend pausing certain medications before testing. Never stop taking medications without your doctor's permission.
