Understanding Colon Cancer and Why Screening Matters

Colon cancer, also called colorectal cancer, develops in the large intestine or rectum. It is one of the most common cancers diagnosed in the United States. According to the American Cancer Society, about 1 in 23 men and 1 in 25 women will develop colorectal cancer in their lifetime. The good news is that colon cancer often develops slowly, and screening can find it at early stages when treatment is most effective.

Screening means testing for cancer in people without symptoms. When colon cancer is found early, before it spreads to other parts of the body, the five-year survival rate is about 90 percent. This is much higher than when cancer is found at later stages. This significant difference shows why screening is so important as a preventive health measure.

Most people who develop colon cancer have no family history of the disease. Risk factors include age (risk increases at 45 and goes up significantly after 50), certain inherited genetic conditions, inflammatory bowel disease, obesity, smoking, heavy alcohol use, and a diet high in red or processed meats. However, many people with risk factors never develop colon cancer, and some people without obvious risk factors do develop it.

Current medical guidelines recommend that people at average risk begin screening discussions with their doctor around age 45. Previously, the recommended age was 50, but this changed as younger people began developing colon cancer. People with a family history of colon cancer, or those with certain genetic conditions, may need to start screening earlier and may need different screening approaches.

Practical Takeaway: Understanding your personal risk factors and age helps you have an informed conversation with your doctor about whether screening is appropriate for you and which screening method might work best for your situation.

Colonoscopy: The Most Common Screening Method

Colonoscopy is the most widely used screening test for colon cancer in the United States. During this procedure, a doctor uses a long, flexible tube called a colonoscope to look inside the entire colon and rectum. The colonoscope has a small camera on the end that allows the doctor to see the intestinal lining on a monitor. If the doctor finds polyps (growths that can become cancer), they can remove them during the same procedure.

Before a colonoscopy, patients must complete a bowel preparation, often called "prep." This involves drinking a solution that clears the colon so the doctor can see clearly. The prep usually takes place the day before the procedure and can take several hours. While this is considered the most uncomfortable part of the process, it is necessary for the procedure to work well. Different prep solutions are available, and patients can discuss options with their doctor if they have concerns about the standard preparation.

During the actual colonoscopy, patients receive sedation so they are relaxed and usually do not remember the procedure. The procedure typically takes 20 to 30 minutes. After the procedure, patients need someone to drive them home since the sedation requires recovery time. Most people can return to normal activities the next day.

Colonoscopy can detect about 95 percent of polyps larger than 6 millimeters. It also has the ability to remove polyps during the procedure, which prevents them from becoming cancer. Doctors can take tissue samples (biopsies) if they see anything concerning. The procedure carries a small risk of bleeding or perforation (a hole in the colon), but serious complications are rare, occurring in about 1 to 2 per 1,000 procedures.

If no polyps are found during colonoscopy, the general recommendation is to repeat the procedure in 10 years for people at average risk. If polyps are found and removed, the doctor will recommend a follow-up timeline, which may be sooner than 10 years depending on what was found.

Practical Takeaway: Colonoscopy remains the gold standard for colon cancer screening because it can both detect and remove polyps in one procedure, but it requires advance preparation and recovery time with sedation.

Flexible Sigmoidoscopy and Other Scope-Based Tests

Flexible sigmoidoscopy is similar to colonoscopy, but it only examines the lower portion of the colon and the rectum. A flexible tube with a camera is inserted through the rectum and advanced about 2 feet into the colon. The procedure takes about 10 to 20 minutes and usually does not require sedation, though some discomfort may occur. Like colonoscopy, polyps can be removed if found.

Sigmoidoscopy misses cancers and polyps in the upper portions of the colon, which account for about 30 to 40 percent of colorectal cancers. Because of this limitation, sigmoidoscopy is often combined with another screening test for the upper colon, such as a fecal test. Some guidelines recommend sigmoidoscopy every 5 to 10 years, often in combination with fecal testing every 1 to 3 years.

CT colonography (also called virtual colonoscopy) is another option that uses CT scanning technology to create detailed images of the colon. During this procedure, the colon is filled with air or carbon dioxide to expand it, and a CT scanner takes pictures. A computer then creates a 3D map of the colon. This procedure requires the same bowel preparation as regular colonoscopy.

CT colonography can detect large polyps (usually 10 millimeters or larger) with reasonable accuracy, but it is less reliable for finding smaller polyps. If polyps are found on CT colonography, a follow-up colonoscopy is usually needed to remove them. CT colonography requires no sedation and carries no risk of perforation, which makes it appealing to some patients. However, it exposes patients to radiation, and the test is more expensive than standard colonoscopy.

Some medical centers offer colonoscopy with improved imaging techniques, such as narrow-band imaging or enhanced imaging, that may help doctors identify polyps more clearly. These techniques use special lighting and filters to highlight certain tissue patterns that might indicate polyps or cancer.

Practical Takeaway: Scope-based tests beyond standard colonoscopy offer alternatives for patients who cannot tolerate regular colonoscopy, though they may have limitations in detecting smaller polyps or require follow-up procedures.

Stool-Based Screening Tests

Stool-based tests detect cancer or precancerous polyps by analyzing a sample of stool (bowel movement). These tests offer the advantage of being non-invasive and can be done at home. Several types of stool-based tests are available.

Fecal immunochemical test (FIT) looks for blood in stool that is not visible to the naked eye. This blood may indicate cancer or polyps. FIT is more specific than older blood tests because it only detects human blood in the lower digestive tract. Patients collect a small sample of stool, usually with a special brush or spatula, and send it to a laboratory. FIT should be done every 1 to 2 years for screening. Studies show that annual FIT testing can detect about 75 to 80 percent of colorectal cancers.

Multitarget stool DNA test (also called FIT-DNA) looks for both blood and certain DNA changes in stool that are associated with cancer and large polyps. This test may detect more cancers and polyps than FIT alone, particularly larger polyps. This test is typically done every 3 years. The patient collects a complete stool sample and sends it to a laboratory.

Guaiac-based fecal occult blood test (gFOBT) is an older test that also looks for blood in stool but is less specific than FIT. This test is less commonly used now because newer tests are more accurate. If gFOBT is used, it should be done annually.

A significant advantage of stool-based tests is that they are simple to perform and do not require sedation or bowel preparation (though some tests require dietary restrictions). The disadvantage is that if the test finds something abnormal, a follow-up colonoscopy is needed to find and remove polyps or biopsy any cancer. Additionally, these tests are less effective at detecting polyps than colonoscopy, particularly smaller polyps that could become cancer over time.

Stool-based tests work best for detecting advanced polyps and cancers that are
