Understanding Colitis: What It Is and How It Develops

Colitis is inflammation of the colon, which is the large intestine. The colon's job is to absorb water and salts from digested food and store waste until it leaves the body. When the colon becomes inflamed, this process breaks down, leading to a range of symptoms. There are several types of colitis, each with different causes and characteristics.

Ulcerative colitis is one of the most common forms. It occurs when the immune system attacks the lining of the colon and rectum, creating ulcers and sores. This is an autoimmune condition, meaning the body's defense system mistakenly targets healthy tissue. According to the Crohn's and Colitis Foundation, approximately 1.3 million Americans have inflammatory bowel disease, with ulcerative colitis affecting roughly 745,000 people.

Crohn's disease is another inflammatory bowel disease that can affect any part of the digestive tract, though it often involves the colon. Unlike ulcerative colitis, which only affects the innermost lining, Crohn's can penetrate deeper into the intestinal walls. This difference affects treatment approaches and symptom severity.

Infectious colitis occurs when bacteria, viruses, or parasites enter the colon and cause inflammation. Common culprits include Clostridium difficile (C. difficile), Salmonella, Shigella, and Campylobacter. This type of colitis is often temporary and resolves once the infection clears.

Ischemic colitis happens when blood flow to the colon is reduced. This can occur in older adults, people with heart disease, or those with certain blood clotting conditions. The reduced blood supply means the colon doesn't receive enough oxygen, leading to inflammation and damage.

Microscopic colitis and lymphocytic colitis are forms where inflammation is only visible under a microscope during colonoscopy. Despite the inflammation being too small to see with the naked eye, patients experience significant symptoms. These conditions are sometimes called "invisible" colitis because the colon appears normal during routine examination.

Practical Takeaway: Understanding which type of colitis a person has is crucial because different types require different treatment approaches. A healthcare provider can determine the type through testing and medical history.

Recognizing Colitis Symptoms

Colitis symptoms vary depending on the severity of inflammation and the type of colitis present. Some people experience mild symptoms, while others face severe, debilitating effects. Recognizing these symptoms early can lead to faster diagnosis and treatment.

Abdominal pain and cramping are among the most common symptoms. The pain typically occurs in the lower abdomen and can range from mild discomfort to severe cramping. Many people describe the pain as sharp or knife-like, particularly during flare-ups. The pain often worsens before bowel movements and improves temporarily after passing stool.

Diarrhea is another hallmark symptom of colitis. People with ulcerative colitis may experience 10 or more bowel movements per day during severe flare-ups. The stool may be watery and may contain blood, mucus, or pus. This frequent diarrhea can lead to dehydration and electrolyte imbalances if not managed properly. According to medical literature, the frequency and consistency of bowel movements often serve as indicators of disease activity.

Rectal bleeding and bloody stools occur when ulcers in the colon or rectum rupture. The amount of blood can vary from small streaks on toilet paper to more substantial bleeding. While seeing blood in the stool is alarming, it's a common symptom in inflammatory bowel disease and doesn't necessarily indicate a life-threatening situation.

Urgency and tenesmus—the constant urge to have a bowel movement even when the bowel is empty—are distressing symptoms. People may rush to the bathroom 15 or more times daily, sometimes multiple times per hour during flare-ups. This constant urgency disrupts daily activities, sleep, and work.

Systemic symptoms extend beyond the digestive tract. Fever, fatigue, weight loss, and loss of appetite commonly accompany colitis. Some people develop joint pain, eye inflammation, or skin problems. These extra-intestinal symptoms suggest the inflammation affects the whole body, not just the colon.

Symptom severity often fluctuates. People may experience periods of remission when symptoms are minimal or absent, followed by flare-ups when symptoms intensify dramatically. The unpredictability makes planning daily activities challenging for many patients.

Practical Takeaway: Keeping a symptom diary that tracks bowel movements, pain levels, food intake, and stress can help identify patterns and provide valuable information for healthcare providers during appointments.

Diagnostic Tests for Colitis

Diagnosing colitis requires a combination of medical history, physical examination, and specialized testing. No single test confirms colitis, so doctors use multiple approaches to reach an accurate diagnosis.

Blood tests provide important information about inflammation and overall health. Complete blood counts measure red and white blood cells—high white blood cell counts suggest active inflammation or infection. Tests measuring C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) indicate the degree of inflammation in the body. Albumin levels reveal whether the patient is receiving adequate nutrition, as diarrhea and malabsorption can deplete protein stores. Iron studies assess for anemia, which frequently develops in people with colitis due to blood loss and poor iron absorption.

Stool tests identify infectious causes of colitis. Stool cultures detect bacterial infections like Salmonella or Shigella. Testing for C. difficile toxins is essential because this bacterium causes a particular type of infectious colitis. Stool tests also measure fecal calprotectin, a marker of intestinal inflammation that helps distinguish inflammatory bowel disease from irritable bowel syndrome.

Colonoscopy is the gold standard for diagnosing colitis. During this procedure, a doctor inserts a thin, flexible tube with a camera into the rectum and guides it through the colon. This allows direct visualization of the intestinal lining. The doctor can observe inflammation, ulcers, bleeding, or other abnormalities. Biopsies—small tissue samples—are taken during colonoscopy to examine under a microscope. These biopsies confirm the type of colitis and rule out other conditions like cancer or infectious diseases.

Sigmoidoscopy examines only the lower portion of the colon and rectum using a shorter tube than colonoscopy. This procedure may be recommended when symptoms primarily affect the lower intestine or as an initial screening tool. While less comprehensive than colonoscopy, it's less invasive and requires less extensive preparation.

Computed tomography (CT) scans create detailed images of the abdomen and pelvis. These scans help identify complications like perforation, toxic megacolon (severe dilation of the colon), or abscesses. CT scans are particularly useful when colonoscopy cannot be performed safely.

Flexible sigmoidoscopy with biopsy helps diagnose microscopic colitis, where inflammation isn't visible to the naked eye. The biopsies show characteristic patterns of inflammation under magnification that confirm the diagnosis.

Practical Takeaway: Patients should discuss which diagnostic tests their doctor recommends and why, as this helps them understand their condition better and prepare mentally and physically for procedures.

Treatment Options for Colitis

Treatment for colitis depends on the underlying cause, severity of symptoms, and individual patient factors. The goal is to reduce inflammation, manage symptoms, and prevent complications. Treatment often involves a combination approach using medications and lifestyle modifications.

Aminosalicylates (5-ASAs) are anti-inflammatory medications commonly used for mild to moderate ulcerative colitis. These drugs work by reducing inflammation in the colon lining. Mesalamine is the most commonly prescribed 5-ASA medication and comes in various forms: oral tablets, capsules that release medication throughout the intestine, enemas, and suppositories. The form prescribed depends on which part of the colon requires treatment. For example, suppositories target the rectum, while oral medications treat the entire colon. Side effects are generally mild and may include headaches, nausea, or abdominal discomfort.
