Learn About Bone Marrow Testing and Medical Reasons

What Is Bone Marrow and Why It Matters

Bone marrow is a soft, spongy tissue found inside the center of most bones in your body. It may sound unusual, but bone marrow plays one of the most critical roles in keeping you healthy. This tissue is responsible for producing blood cells—the cells that carry oxygen, fight infections, and help your blood clot when you get cut. Without properly functioning bone marrow, your body cannot make enough of these essential cells.

There are two main types of bone marrow: red bone marrow and yellow bone marrow. Red bone marrow is where your body actually manufactures blood cells. This includes red blood cells, which transport oxygen throughout your body; white blood cells, which defend against infections; and platelets, which help stop bleeding. Yellow bone marrow contains mostly fat cells and serves as a backup energy source for your body. As you age, more of your bone marrow turns from red to yellow, which is a normal part of aging.

Bone marrow is found in several locations throughout your skeleton. The ribs, sternum (breastbone), pelvis, spine, and the ends of long bones like your femur (thighbone) all contain active bone marrow. In adults, the pelvis and sternum contain the most active bone marrow production.

When doctors suspect problems with blood cell production, they may order bone marrow testing. These tests examine whether your bone marrow is producing the right number and types of blood cells. Testing can reveal conditions such as leukemia, lymphoma, certain anemias, infections, and other blood disorders. Understanding what bone marrow is and why testing matters can help you feel more informed if your doctor recommends this procedure.

Practical Takeaway: Bone marrow produces the blood cells your body needs to function. If you're experiencing symptoms like persistent fatigue, frequent infections, or unexplained bleeding, your doctor may recommend bone marrow testing to determine the cause.

Types of Bone Marrow Tests Explained

There are two main types of bone marrow tests that doctors use to gather information: bone marrow aspiration and bone marrow biopsy. These procedures are different but are often performed together during a single office visit. Understanding what each test does can reduce anxiety and help you know what to expect.

A bone marrow aspiration involves using a thin needle to withdraw a small sample of the liquid portion of bone marrow. The doctor inserts the needle through the skin and into the bone, typically at the hip (pelvis) in adults or sometimes the sternum. Once the needle reaches the marrow cavity, gentle suction pulls out a small amount of the liquid marrow. This sample contains blood cells and can be examined under a microscope to count cell types and look for abnormal cells. The aspiration usually takes just a few minutes. Many patients describe feeling pressure and a brief pulling sensation, though the area is numbed first. The sample obtained is typically placed on glass slides and sent to a laboratory for analysis.

A bone marrow biopsy is slightly different. Instead of withdrawing liquid marrow, the doctor uses a slightly larger needle to remove a small solid sample of bone marrow tissue—about the size of a grain of rice. This tissue sample shows how densely packed the marrow cells are and reveals the overall structure of the bone marrow. A biopsy provides information that aspiration alone cannot, such as whether fibrosis (scarring) is present in the marrow. The biopsy is processed differently than aspiration samples, often requiring special preparation before microscopic examination.

Sometimes doctors perform just an aspiration, just a biopsy, or both procedures together. The choice depends on what information is needed. For example, if a doctor suspects leukemia, both tests together give the most complete picture. In other cases, one test may provide sufficient information. Your doctor will explain which test or tests you need and why.

Practical Takeaway: Bone marrow aspiration removes liquid marrow to check cell types, while a biopsy removes solid tissue to examine marrow structure. Your doctor will determine which test best matches your situation.

Medical Reasons Doctors Order Bone Marrow Tests

Doctors order bone marrow tests when blood tests suggest a problem with blood cell production or when they need to investigate unexplained symptoms. There are many medical reasons for this testing. Understanding these reasons can help you grasp why your doctor believes testing is necessary for your care.

One common reason is investigating anemia—a condition where you don't have enough red blood cells or hemoglobin. While some anemias are caused by iron deficiency and can be managed with iron supplements, others result from problems in bone marrow production. Bone marrow testing can determine whether your bone marrow is making red blood cells properly or whether something is preventing cell production. Aplastic anemia, for example, occurs when bone marrow produces too few blood cells. Testing confirms this diagnosis.

Leukemia, a cancer of white blood cells, is another significant reason for bone marrow testing. When blood tests show abnormal white blood cell counts or unusual cell shapes, bone marrow testing can confirm whether leukemia is present and identify which type. According to the American Cancer Society, approximately 60,000 new leukemia cases are diagnosed in the United States each year. Bone marrow testing is essential for diagnosing and classifying these cases.

Lymphoma—cancer of the lymph system—may also require bone marrow testing to determine if cancer cells have spread to the bone marrow. Similarly, multiple myeloma, a cancer of plasma cells in the bone marrow, is diagnosed and monitored through bone marrow testing. Doctors may also order bone marrow tests to investigate persistent infections that don't respond to treatment, to check for storage diseases where harmful substances accumulate in cells, or to evaluate fevers of unknown origin where the cause hasn't been identified despite other testing.

Additionally, patients receiving chemotherapy or other treatments that affect bone marrow may have bone marrow tests to monitor how well their marrow is recovering. People with certain genetic disorders or those being evaluated before a bone marrow transplant also undergo this testing.

Practical Takeaway: Bone marrow testing investigates blood cell production problems, confirms cancer diagnoses, tracks treatment response, and identifies unexplained blood disorders. If your doctor recommends testing, ask specifically what condition or symptom they're investigating.

What to Expect During the Bone Marrow Test Procedure

Knowing what happens during a bone marrow test can reduce anxiety and help you prepare mentally and physically. While the procedure may sound uncomfortable, it typically takes 15 to 30 minutes total, including preparation time, and modern techniques and anesthesia make it manageable for most patients.

Before the procedure begins, you'll meet with your doctor to discuss the test and sign consent forms. The medical team will review your medical history, current medications, and any allergies. It's important to mention if you take blood thinners like warfarin or aspirin, as these may affect the procedure. You'll be positioned lying on your side or stomach, depending on which site (usually the hip) the doctor is using. A pillow may be placed under your hip to make the position more comfortable.

The doctor will clean the area with an antiseptic solution and apply a local anesthetic to numb the skin and bone. You may feel a slight stinging sensation as the anesthetic is injected, but this numbness takes effect within a minute or two. Some doctors also offer light sedation to help you relax, though many patients have the procedure awake. Once you're numb, the doctor will insert the aspiration needle and you'll likely feel pressure and perhaps a pulling sensation as the marrow is withdrawn. This part usually lasts less than a minute. If a biopsy is also being done, the doctor may use a slightly different needle to obtain the tissue sample.

Throughout the procedure, stay as still as possible and let the medical team know if you experience pain—not just pressure, but actual pain—as this means the anesthetic may need to be strengthened. After the samples are collected, pressure is applied to stop any bleeding, and a bandage is placed over the site. You can typically return to normal activities the next day, though you may feel sore at the puncture site for a few days.

Practical Takeaway: Bone marrow testing is a brief outpatient procedure performed with numbing medication. Tell your doctor about any blood thinners you take and report pain during the procedure so they can adjust anesthesia if needed.
