What Bone Density Testing Measures and Why It Matters

Bone density testing, also called a DXA scan or DEXA scan, measures how much mineral content is packed into your bones. Think of bone like a sponge — it has solid material and spaces inside. As people age, especially after menopause in women, those spaces can get larger and the solid material thinner. This makes bones more fragile and more likely to break from a fall or injury. A bone density test shows whether your bones are strong, normal, or weaker than they should be for your age and sex.

The test uses very small amounts of X-rays to create images of your bones. It's painless and takes about 10 to 30 minutes. The machine measures bone density at specific sites, usually the hip, spine, and sometimes the forearm. Doctors compare your results to healthy young adults and to people your age to see how your bone health compares. The test produces a score called a T-score, which tells whether you have normal bone density, low bone density (called osteopenia), or osteoporosis.

Bone density testing is important because many people with weak bones don't know it until they break something. About one in three women and one in five men over age 50 will have an osteoporosis-related fracture in their lifetime, according to the National Osteoporosis Foundation. Knowing your bone density early can help you and your doctor plan steps to protect your bones and reduce fracture risk.

Practical takeaway: Bone density testing gives you concrete information about your bone strength. If you're over 50, have risk factors for weak bones, or have had a fracture, talking with your doctor about whether testing makes sense for you is a reasonable step.

Types of Bone Density Tests Available

Several different types of bone density testing exist, each with different uses and levels of detail. The most common is the DXA scan (dual-energy X-ray absorptiometry), which uses two different X-ray beams to measure bone mineral density. It's quick, uses very low radiation, and is considered the standard test for diagnosing osteoporosis. Most insurance plans cover DXA scans when your doctor orders them for medical reasons.

Another option is quantitative computed tomography (QCT), which uses a CT scanner to create 3D images of bone. This test can measure bone density in the spine and hip with very detailed images. QCT takes longer than DXA and uses more radiation, so it's usually reserved for specific situations, like when someone has spine problems that make a regular DXA hard to read, or when more detailed information is needed. QCT costs more and may not be covered by all insurance plans.

Peripheral DXA (pDXA) is a smaller, portable version of the regular DXA machine. It measures bone density at the wrist, shin, or heel instead of the hip and spine. This type of test is faster and uses even less radiation. Some people get pDXA screening at health fairs or pharmacies, though results from peripheral sites may not be as useful for predicting fracture risk as hip and spine measurements.

Quantitative ultrasound uses sound waves instead of X-rays to measure bone at the heel. It doesn't use radiation at all, which appeals to some people. However, ultrasound results don't always match DXA results, and doctors can't use ultrasound alone to diagnose osteoporosis. High-resolution peripheral quantitative CT (HR-pQCT) is a newer technology that creates very detailed images of bone structure at the wrist and shin, but it's mainly used in research settings, not routine clinical care.

Practical takeaway: DXA scanning is the standard, most widely available, and most studied test. If your doctor recommends bone density testing, DXA at the hip and spine is typically the starting point. Ask your doctor which test makes sense for your situation.

Who Should Consider Bone Density Testing

The U.S. Preventive Services Task Force recommends bone density screening for all women age 65 and older and for postmenopausal women under 65 who have risk factors for osteoporosis. For men, screening is recommended starting at age 70, or earlier if they have risk factors. However, many people outside these groups may also benefit from testing if they have certain medical conditions or life circumstances.

Risk factors that may suggest bone density testing include a family history of osteoporosis or hip fractures, a personal history of any broken bones as an adult, smoking, excessive alcohol use, low body weight, or a sedentary lifestyle. Certain medical conditions increase bone loss risk, including rheumatoid arthritis, celiac disease, inflammatory bowel disease, kidney or liver disease, and hyperthyroidism. Some medications also weaken bones over time, including long-term corticosteroid use and certain cancer medications.

Hormonal changes matter too. Women who went through menopause early (before age 45), whether naturally or surgically, lose bone density faster and may want to discuss testing with their doctor. People taking hormone replacement therapy, or those who stopped taking it, might benefit from knowing their bone density. Men with low testosterone levels also have increased fracture risk and might consider testing.

People with a history of falls, balance problems, or mobility issues may want to know their bone density since a fracture could have serious consequences for their independence. Anyone planning to take bone-strengthening medications would typically have a bone density test first to establish a baseline and confirm the diagnosis. Even without symptoms or obvious risk factors, talking with your doctor about bone health can help determine whether testing would be useful for you.

Practical takeaway: Age 65 and older (women) or 70 and older (men) is a clear marker for considering testing. If you have risk factors like smoking, low body weight, family history, or conditions like rheumatoid arthritis, discussing testing with your doctor makes sense even if you're younger.

What to Expect During and After a Bone Density Test

When you arrive for a DXA scan, there's no special preparation needed in most cases. You can eat normally, take your medications, and wear regular clothes. You'll need to remove items that might interfere with the scan, like jewelry, belts with metal, or watches. The technician will have you lie on a padded table, and the scanning arm moves slowly over your hip, spine, and sometimes forearm. You stay completely still for a few minutes while the machine takes images. Some people describe it as similar to getting an X-ray, but lying down and more relaxed. The whole appointment usually takes 10 to 30 minutes.

During the scan, you'll receive a very small amount of radiation — much less than a chest X-ray and roughly equivalent to a few days of natural background radiation from the environment. You won't feel anything during the test, and there are no side effects. Some machines are open on the sides, which works well for people with claustrophobia. Other machines are more enclosed, but still have open space around you.

After the scan, you can return to your regular activities immediately. There's nothing to recover from. Your results typically come back within a few days to a week. Your doctor will review the report with you and explain what your scores mean. The report includes your T-score (how your bones compare to a healthy young adult) and sometimes your Z-score (how your bones compare to others your age). You'll learn whether you have normal bone density, low bone density, or osteoporosis.

If your results show low bone density, your doctor may recommend follow-up steps like calcium and vitamin D testing, lifestyle changes such as increased exercise and dietary improvements, medications if appropriate, or another scan in the future to track changes. The timing of repeat scans varies — typically they're done every 2 to 5 years depending on your results and risk factors. Bring a list of your medications to your appointment since some medicines can affect bone density.

Practical takeaway: A bone density test is quick, painless, and involves minimal radiation. You'll get clear results within days that you and your doctor can discuss to make a plan for bone health.

Understanding Your Bone Density Test Results

Bone density results are reported using T-scores, which compare your bone density to that of a healthy 30-year-old adult. A T-score of -1.0 or higher is considered normal bone density. A score between -1.0 and -2.5
