Understanding ADHD and ADD: What They Are

ADHD stands for Attention-Deficit/Hyperactivity Disorder, while ADD stands for Attention-Deficit Disorder. These terms describe conditions that affect how the brain processes information, manages attention, and controls impulses. The main difference between them is that ADHD involves hyperactivity and impulsivity symptoms, while ADD typically does not include significant hyperactivity.

Both conditions are neurodevelopmental disorders, meaning they involve differences in how the brain develops and functions. According to the Centers for Disease Control and Prevention (CDC), approximately 6.1 million children in the United States have been diagnosed with ADHD. In adults, studies suggest that between 2.5% and 4.4% of the population has ADHD, though many cases go undiagnosed.

These conditions are not the result of poor parenting, too much screen time, or lack of discipline. Research shows that genetics play a significant role—if a parent has ADHD, their children are more likely to have it as well. Brain imaging studies have revealed that people with ADHD may have differences in brain structure and function, particularly in areas related to attention, impulse control, and reward processing.

It is important to understand that ADHD and ADD are not character flaws or signs of low intelligence. Many people with these conditions are highly intelligent and creative. The challenge lies in managing attention and impulse control, not in cognitive ability.

Practical Takeaway: Understanding that ADHD and ADD are neurobiological conditions—not behavioral problems or choices—helps reduce stigma and opens the door to recognizing symptoms in yourself or others.

Common Symptoms in Children

Children with ADHD or ADD often display a range of symptoms that can appear in different combinations and intensities. Symptoms typically become noticeable between ages 3 and 6, though diagnosis often occurs during school years when academic and social demands increase. The symptoms fall into two main categories: inattention and hyperactivity-impulsivity.

Inattention symptoms include difficulty sustaining focus on tasks, appearing not to listen when spoken to directly, struggling to organize tasks and activities, frequently losing necessary items like homework or keys, and being easily distracted by unrelated stimuli. A child might start homework and become sidetracked within minutes, or lose track of multi-step instructions. These children often seem forgetful in daily activities like remembering chores or appointments.

Hyperactivity-impulsivity symptoms manifest as fidgeting or squirming in seats, difficulty remaining seated when expected, excessive running or climbing (in situations where it is inappropriate), inability to engage quietly in activities, acting as though "driven by a motor," talking excessively, blurting out answers before questions are finished, difficulty waiting turns, and interrupting or intruding on others. A child might struggle to sit still during meals or classroom instruction, constantly move from one activity to another, or dominate conversations.

It is crucial to note that all children show some of these behaviors occasionally. The key difference with ADHD and ADD is that symptoms are persistent, occur across multiple settings (home, school, and social environments), and significantly impair functioning. A child diagnosed with ADHD typically shows symptoms for at least six months, and these symptoms were present before age 12.

• Difficulty focusing on non-preferred tasks while hyperfocusing on preferred activities
• Frequent forgetfulness about daily tasks and responsibilities
• Chronic disorganization of belongings and schedules
• Impulsive decision-making without considering consequences
• Emotional sensitivity and difficulty managing frustration
• Sleep problems, including difficulty falling asleep or staying asleep

Practical Takeaway: If a child shows several inattention or hyperactivity symptoms across different settings for six months or longer, this information may warrant a conversation with a healthcare provider who can conduct a thorough evaluation.

Recognizing ADHD and ADD Symptoms in Adults

Adult ADHD and ADD often looks different from the childhood presentation, and many adults go undiagnosed throughout their lives. As people mature, they may develop coping strategies that mask symptoms, making recognition more challenging. However, adults with untreated ADHD and ADD may experience significant difficulties in work, relationships, and personal organization.

Adults with inattention symptoms often report chronic disorganization, difficulty managing finances, frequent job changes due to boredom or difficulty meeting expectations, chronic lateness, and challenges with time management. An adult might consistently miss deadlines despite good intentions, struggle to follow through on projects, or find it difficult to complete administrative tasks like paying bills or organizing documents. Many adults describe feeling overwhelmed by the number of tasks they need to accomplish.

Hyperactivity in adults may not appear as obvious physical restlessness but rather as mental restlessness, difficulty relaxing, constant engagement in multiple activities, and a preference for high-stimulation environments. Adults might need to move around while thinking, have trouble sitting through meetings or movies, feel bored easily, and pursue multiple interests simultaneously. Some adults describe feeling a constant internal sense of urgency or restlessness.

Impulsivity in adults can manifest as frequent interruptions in conversations, hasty decision-making without considering consequences, excessive spending, relationship difficulties due to reactive responses, and difficulty with emotional regulation. Adults may experience intense emotional reactions that feel disproportionate to situations, struggle to "think before acting," and have a history of impulsive life choices.

Additional symptoms in adults include difficulty prioritizing tasks, chronic procrastination, hyperfocus on interesting topics while neglecting other responsibilities, difficulties maintaining relationships due to forgetfulness or seeming inattentiveness, and lower self-esteem due to repeated failures despite capability. Many adults with undiagnosed ADHD develop anxiety or depression as secondary conditions.

• Persistent difficulty organizing work and living spaces
• Trouble maintaining focus during meetings, conversations, or reading
• Chronic procrastination and missed deadlines
• Frequent job changes or career dissatisfaction
• Relationship conflicts related to forgetfulness or inattentiveness
• Difficulty managing finances and paying bills on time
• Restlessness or feelings of internal agitation
• Impulsive spending, eating, or decision-making

Practical Takeaway: Adults experiencing long-standing patterns of disorganization, difficulty focusing, impulsivity, or emotional dysregulation may explore whether ADHD or ADD could be a factor by discussing their experiences with a healthcare provider.

How ADHD and ADD Are Diagnosed

Diagnosis of ADHD and ADD involves a comprehensive evaluation by a qualified healthcare professional, typically a pediatrician, psychiatrist, psychologist, or neuropsychologist. There is no single test that definitively diagnoses these conditions. Instead, professionals use multiple assessment methods to gather information and rule out other possible causes of symptoms.

The evaluation process typically begins with a detailed clinical interview where the healthcare provider asks about developmental history, symptom onset and progression, family history of ADHD or related conditions, medical history, medications, school or work performance, and how symptoms impact daily functioning. The provider will ask about symptoms across different settings and at different times. Parents of children are usually asked to provide information about behavior at home, and teachers may be asked to describe behavior at school.

Standardized rating scales and questionnaires are commonly used as part of the evaluation. For children, the Vanderbilt Assessment, Conners Rating Scale, and ADHD Rating Scale are frequently used. For adults, the Adult ADHD Self-Report Scale (ASRS) and Conners Adult Rating Scale are common tools. These instruments ask specific questions about symptom frequency and severity and help quantify the presence and impact of symptoms.

Psychological and educational testing may be included to measure attention, executive function, and cognitive abilities, and to identify learning disabilities that might co-occur with ADHD. Continuous Performance Tests and other computerized assessments can measure sustained attention and reaction time. Intelligence testing can determine if symptoms are related to intellectual disability rather than ADHD.

Medical evaluation is important to rule out other causes of symptoms such as thyroid problems, sleep disorders, hearing difficulties, or medication side effects
