What Home Support Services Are and How They Work in California

Home support services, also called in-home supportive services (IHSS), are programs that allow people to receive care while staying in their own homes instead of moving to facilities like nursing homes or assisted living centers. In California, these services cover a range of tasks that help people manage daily life, including personal care, household chores, meal preparation, and help getting around.

The state's IHSS program is one of the largest in the nation. It serves hundreds of thousands of Californians each month. The program works by having the state pay for a caregiver—often a family member, friend, or hired worker—to come to someone's home and provide support. This means the person receiving care stays in their own environment, surrounded by familiar surroundings and people they know.

Services covered under IHSS include bathing and grooming, toileting and incontinence care, meal preparation and cleanup, laundry and ironing, shopping and errands, and help with mobility. Some recipients also receive services like paramedical care, which might include wound care or medication management performed by trained workers. The amount and type of service each person receives varies based on their situation and needs.

California's program operates through a combination of state and federal funding, with the state paying a larger share than most other states. This structure means the program can serve more people and offer more hours of care than programs in some other parts of the country. However, how much care someone receives depends on an assessment of what they actually need to stay safe and functional at home.

Practical takeaway: Understanding that home support services allow people to receive paid care in their own homes is the foundation for learning how the system works. These services range from basic help with daily tasks to more specialized medical care, and the specific services available depend on individual circumstances.

Who May Receive Home Support Services in California

Home support services in California are available to people who meet certain requirements. Generally, a person must be at least 65 years old, blind, or disabled. They must also live in California and be a U.S. citizen or qualified immigrant. Additionally, someone receiving these services must need help with activities of daily living—tasks like bathing, using the toilet, or preparing meals—that they cannot do alone.

The program serves a diverse population. Many recipients are older adults living on Social Security or other fixed incomes. Others are younger people with disabilities, including those with physical disabilities, developmental disabilities, or mental health conditions. Some recipients are people recovering from surgery or illness who need temporary help. Others need long-term, ongoing support to remain in their homes.

Income and resources matter for the program. A person's income level affects whether they might be found within program guidelines, and there are resource limits—meaning the amount of money or property someone owns can affect their situation. However, the program is designed to serve people with limited incomes, and most recipients are not wealthy. A person receiving Social Security, even a modest amount, may still be within the range the program considers.

An important point is that people do not need to be homebound to receive services. Someone might receive help with tasks even if they can go outside or do some activities independently. The program focuses on helping people with specific tasks they cannot manage on their own, not on whether they can leave their home.

The program also considers someone's support system. If a person has family members or friends available to provide unpaid care, that affects how much paid service they might receive. The program aims to supplement informal support, not completely replace it, though situations vary widely.

Practical takeaway: Home support services are available to people aged 65 and older, blind individuals, or people with disabilities who cannot perform daily tasks alone and meet income and citizenship requirements. The program serves people across many different life situations and disability types.

Types of Services Provided and What Tasks Are Covered

California's home support services cover many types of tasks that help people live independently at home. Personal care services include assistance with bathing, showering, grooming, dressing, and using the toilet. These services are available to people who cannot safely do these things alone. A support worker might help someone wash their hair, brush their teeth, or change clothes. For people with incontinence, workers provide assistance with toileting and cleaning up as needed.

Household services form another major category. Support workers can help with light housework like dusting, vacuuming, and sweeping. They can wash dishes and clean bathrooms. Laundry services include washing, drying, and ironing clothes. These tasks matter because someone who cannot physically manage them might face unhygienic living conditions or health risks without help.

Meal preparation is a significant service. A support worker might plan meals, shop for groceries, prepare food, and clean up afterward. For people who cannot safely use a stove or prepare meals independently, this service prevents malnutrition and reduces safety risks like burns or food poisoning. The worker can shop with the person or for them, depending on the person's abilities.

Paramedical services represent more specialized support. These include services like wound care, catheter care, colostomy care, and assistance with medications. A trained paramedical worker might help someone change a wound dressing, manage medical equipment, or help with other health-related tasks. These services are only available when ordered by a doctor and require specific training.

Protective supervision is available for people with cognitive impairments who need someone present to ensure their safety. Someone with advanced dementia or severe mental illness might need a worker present to prevent wandering, ensure they take medications, or keep them safe if they might harm themselves. A protective supervision worker stays in the home and watches over the person.

Practical takeaway: Home support services cover a wide range of tasks from basic personal care and household work to specialized medical assistance and supervision. The specific services available to a person depend on what help they actually need to stay safe and maintain their health.

How to Learn About Applying for or Accessing Services

The process for learning about home support services in California starts with contacting your local Social Services office or Adult Protective Services. Each county in California runs its own IHSS program, so the specific office you contact depends on where you live. County Social Services offices have information specialists who can explain what services exist and what the process involves.

A person interested in learning about services can call their county's Social Services office and ask to speak with someone about IHSS. They can ask questions about what services exist, what the process involves, and what information they would need to gather. County offices also have written materials explaining the program in different languages. Many offices have websites with information and contact details.

If someone is in a hospital or care facility and wants to understand options for going home, they can ask the hospital social worker or discharge planner about home support services. These professionals often know about IHSS and can provide information. They might help connect someone with their county office.

Getting information about services does not commit someone to anything. Learning about what services exist, how the process works, and what would be involved is an important first step. County workers can describe the program, explain what information is needed, and answer questions about how services work.

Someone can also ask family members to help them learn about services. A spouse, adult child, or other family member can call the county office with questions and help gather information. If someone cannot make phone calls easily, a family member can do this on their behalf.

Documentation matters in any process. Gathering information like medical records, income documents, and housing information ahead of time can be helpful. However, county offices will explain what documents are needed and help someone understand what to prepare.

Practical takeaway: Learning about home support services starts with contacting your county's Social Services office. County workers can explain what services exist, answer questions about how the process works, and provide information in writing or in your language.

Costs, Payments, and How the Program Finances Services

Home support services in California are paid for through a combination of state and federal money, primarily through Medi-Cal, which is California's Medicaid program. This means most people receiving IHSS services do not pay directly for the care. Instead, the state pays the support worker's wages. In 2024, California support workers typically earn between $16 and $18 per hour, depending on the county, though rates continue to change.
