Understanding Hemorrhoids: What the Medical Evidence Shows

Hemorrhoids are swollen blood vessels in the rectal and anal area. They're one of the most common medical conditions, affecting roughly 75% of people at some point in their lives, according to the American Society of Colon and Rectal Surgeons. Despite how common they are, many people feel embarrassed discussing them, which can delay getting information and relief.

There are two main types of hemorrhoids: internal and external. Internal hemorrhoids occur inside the rectum, where there are fewer pain-sensing nerves. People with internal hemorrhoids often notice bright red blood on toilet paper or in the toilet bowl, but may not feel pain. External hemorrhoids develop under the skin around the anus and are typically more uncomfortable because the area has many nerve endings. They can cause itching, pain, and a hard lump you can feel.

Hemorrhoids develop when pressure builds up in the veins of the rectum and anus. Common causes include straining during bowel movements, prolonged sitting on the toilet, chronic constipation or diarrhea, pregnancy, heavy lifting, and a low-fiber diet. Age can also play a role—hemorrhoids become more common after age 30, though they can happen at any age.

Most hemorrhoids are not serious, though they can be bothersome and affect daily life. Bleeding from hemorrhoids is usually minor, but any rectal bleeding should be discussed with a healthcare provider to rule out other conditions. A self-check guide helps you recognize common hemorrhoid symptoms and understand whether the information you find matches your own situation.

Practical takeaway: Learning to identify hemorrhoid symptoms and their common causes helps you distinguish them from other conditions and decide whether to seek medical information or care.

Recognizing Common Hemorrhoid Symptoms

Symptom recognition is the first step in understanding your own health situation. Different types of hemorrhoids produce different signs, and knowing what to look for makes it easier to describe your symptoms if you speak with a healthcare provider.

Common symptoms of internal hemorrhoids include:

• Bright red blood on toilet paper, in the toilet bowl, or on stool
• Feeling like you haven't completely emptied your bowels
• Mucus discharge from the anus
• Itching or irritation in the anal area
• A lump protruding from the anus (in advanced cases)

Common symptoms of external hemorrhoids include:

• Pain or discomfort, especially when sitting or during bowel movements
• Itching and irritation around the anus
• Swelling around the anus
• A hard, tender lump near the anus that you can touch
• Bleeding (less common with external hemorrhoids than internal)

The severity of symptoms varies widely. Some people experience minor itching, while others have significant pain or bleeding. Symptoms may come and go, with some people having periods without symptoms followed by flare-ups. Flare-ups often happen during times of constipation, diarrhea, or increased straining.

It's important to note that rectal bleeding can come from other sources besides hemorrhoids, including anal fissures, polyps, inflammatory bowel disease, or infections. If you notice blood, a healthcare provider should evaluate it. Self-check guides help you document what you're experiencing but cannot replace professional medical evaluation.

Practical takeaway: Keep track of your symptoms for a week or two before speaking with a healthcare provider. Note when symptoms occur, what makes them worse or better, and whether you see bleeding or discharge. This information helps healthcare providers understand your situation.

Risk Factors and Lifestyle Connections

Understanding what increases hemorrhoid risk helps explain why you might be experiencing symptoms. While anyone can develop hemorrhoids, certain factors make them more likely. A self-check guide reviews these risk factors so you can identify which ones may apply to your situation.

Dietary factors play a significant role. The average American gets about 15 grams of fiber per day, but health organizations recommend 25 to 35 grams daily. Low fiber intake leads to harder stools and increased straining during bowel movements, which puts pressure on rectal veins. Constipation affects about 15% of the general population and is a major hemorrhoid contributor. Dehydration worsens constipation, as the body reabsorbs water from stool when intake is insufficient.

Lifestyle habits also matter:

• Prolonged sitting, especially on the toilet—spending more than 5 to 10 minutes sitting while straining increases pressure
• Heavy lifting or strenuous exercise without proper breathing technique
• Holding your breath during physical effort instead of exhaling
• Spending extended time in positions that increase abdominal pressure
• Ignoring the urge to have a bowel movement, which can lead to harder stools later

Certain life stages increase hemorrhoid risk. Pregnancy causes hemorrhoids in 20 to 30% of pregnant women due to increased blood volume, hormonal changes, and pressure from the growing baby. Older adults experience hemorrhoids more frequently because blood vessel walls weaken with age. Some medical conditions also increase risk, including liver disease, which affects blood flow, and inflammatory bowel conditions like Crohn's disease or ulcerative colitis.

Practical takeaway: Review the risk factors listed above and identify which ones apply to you. Many are modifiable through diet, hydration, and lifestyle changes. Understanding your personal risk factors helps you evaluate whether symptoms might be hemorrhoid-related and what changes might help.

What You Can Do at Home: Management and Prevention Information

Many people manage hemorrhoid symptoms at home without medical treatment. A self-check guide provides information about approaches that research shows can reduce discomfort. These are not treatments that will cure hemorrhoids, but rather strategies that may reduce symptoms and prevent flare-ups.

Dietary changes form the foundation of hemorrhoid management. Increasing fiber gradually through whole grains, fruits, vegetables, legumes, and nuts makes stools softer and easier to pass, reducing strain. Aim to add fiber slowly over a few weeks to avoid digestive discomfort. Drinking more water—typically 6 to 8 glasses daily—helps soften stool. Some people find that increasing water intake alone helps their symptoms noticeably.

Bathroom habits matter more than many people realize:

• Don't delay bowel movements—go when you feel the urge
• Limit time on the toilet to 5 to 10 minutes to reduce pressure
• Avoid straining; if nothing happens after a minute or two, get up and try again later
• Use soft toilet paper or wet wipes, or try a bidet to reduce irritation
• Don't force bowel movements with excessive pushing

Over-the-counter products available in most drugstores may provide temporary symptom relief. Topical creams and ointments containing hydrocortisone or witch hazel can reduce itching and discomfort. Medicated pads also offer relief. Stool softeners, available over-the-counter, may help if constipation is a problem. A self-check guide explains what these products are, how they work, and what to expect from them, though it cannot recommend specific products.

Physical habits also help. Sitting in a warm bath for 10 to 15 minutes several times daily, especially after bowel movements, can reduce pain and itching. Some people find that ice packs wrapped in cloth provide numbing relief. Regular physical activity—even a daily 20-minute walk—helps with bowel regularity and overall circulation.

Practical takeaway: Start with dietary changes and increased water intake, as these are the most effective and have no side effects. Track any improvements over two to four weeks. If symptoms persist or worsen despite these changes, that's useful information to share with a healthcare provider.
