Understanding Concussions: What They Are and How They Happen

A concussion is a type of brain injury caused by a bump, blow, or jolt to the head. It can also happen when the body is hit hard enough that the brain moves rapidly inside the skull. According to the Centers for Disease Control and Prevention (CDC), approximately 2.5 million emergency department visits each year involve concussions in the United States alone. While some people think concussions are rare or only happen in serious accidents, they actually occur during everyday activities.

Concussions can happen in many different situations. Sports-related concussions are common—the CDC reports that high school athletes experience an estimated 11,000 concussions per year during practice and competition. However, concussions also occur outside of sports. Car accidents, falls (especially in older adults), workplace injuries, and bicycle accidents are frequent causes. A person doesn't need to lose consciousness to have a concussion, which surprises many people. In fact, most people who experience a concussion do not lose consciousness.

The brain is surrounded by fluid that cushions it inside the skull. When a concussion occurs, the impact can cause the brain to move within this fluid-filled space, disrupting normal brain function. This disruption happens at the cellular level and can affect how the brain sends and receives signals. The severity can range from mild to more serious, though medical professionals typically describe concussions as mild traumatic brain injuries.

Understanding what a concussion actually is helps people recognize when they or someone else might have experienced one. The key is recognizing that a concussion results from impact or movement, not necessarily from a visible injury to the head. Many people walk away from incidents that cause concussions and don't immediately realize they've been injured.

Practical Takeaway: Concussions are brain injuries that can result from many types of accidents and impacts. They don't require loss of consciousness and can happen to anyone in various situations—sports, falls, car accidents, or everyday mishaps.

Recognizing Concussion Symptoms in Children and Adults

Concussion symptoms can appear immediately after an injury or develop over hours or even days. The symptoms vary from person to person, and not everyone experiences the same signs. The CDC identifies several categories of symptoms that may indicate a concussion. Physical symptoms include headache, dizziness, balance problems, nausea, fatigue, sensitivity to light, and sensitivity to noise. Some people report that their neck feels stiff or that they have trouble sleeping.

Cognitive symptoms affect thinking and memory. A person with a concussion might have trouble concentrating, feel confused or disoriented, experience memory problems, or have difficulty understanding or responding to questions. They may say things like "I feel foggy" or "I can't seem to focus." These cognitive changes can be subtle—someone might seem like themselves but struggle with tasks they normally handle easily.

Emotional and mood-related symptoms can also occur. People recovering from concussions sometimes report feeling irritable, sad, nervous, or emotional. They may feel anxious without a clear reason or notice that their mood changes more quickly than usual. Some describe feeling "not quite right" without being able to pinpoint exactly what feels different.

Symptoms in young children present differently because children may not be able to describe what they're experiencing. Parents and caregivers should watch for appearing dazed, excessive crying, not wanting to play, balance problems when walking or running, or a change in how the child plays or behaves. Older children and teens may express their symptoms more clearly but might minimize them, especially if they want to return to sports or activities.

It's important to note that someone with a concussion might not experience all of these symptoms. One person might have a headache and dizziness, while another might primarily notice trouble concentrating and mood changes. Recovery times vary too—some people feel better within a few days or weeks, while others take longer to return to normal.

Practical Takeaway: Concussion symptoms fall into four categories: physical (headache, dizziness), cognitive (trouble concentrating, memory problems), emotional (irritability, mood changes), and sleep-related. Symptoms may appear immediately or develop later, and not everyone experiences the same signs.

When to Seek Medical Attention After a Head Injury

Not every bump to the head requires a hospital visit, but certain situations warrant immediate medical evaluation. The CDC and medical organizations provide clear guidance about when someone should seek emergency care. A person should go to an emergency department or call emergency services if they experience severe headache, repeated vomiting, loss of consciousness (even briefly), difficulty waking up, slurred speech, severe confusion, inability to remember events, seizures, weakness or numbness, or look increasingly drowsy or difficult to wake.

Beyond emergency situations, many people benefit from seeing a healthcare provider after a head injury even if symptoms seem mild. A doctor can perform an assessment to determine whether a concussion occurred and provide guidance for recovery. This is particularly important for children and young people whose brains are still developing. Research shows that getting proper medical evaluation early leads to better understanding of what happened and clearer recovery guidance.

Some workplaces and schools have protocols for handling potential head injuries. Athletic trainers at schools often perform initial evaluations of athletes suspected of having concussions. These professionals use various tests to assess brain function and determine whether someone should be removed from activity. Parents who are unsure whether an injury warrants medical attention can call their child's pediatrician or their own primary care doctor to discuss what they observed.

Documentation matters in recovery. When someone receives medical evaluation, the healthcare provider creates a record of the injury and symptoms. This documentation helps guide recovery decisions and return-to-activity timelines. It also creates a baseline if the person experiences another concussion in the future—research shows that repeat concussions may carry additional risks, so knowing about previous injuries is medically important.

The phrase "getting cleared to return to play" comes from medical evaluation. A healthcare provider determines when someone is ready to resume sports or other activities based on symptom resolution and testing. This clearance process protects people from returning too quickly when their brain is still recovering.

Practical Takeaway: Seek emergency care for severe symptoms like loss of consciousness or repeated vomiting. For suspected concussions without emergency symptoms, contact a healthcare provider for evaluation and guidance. Medical documentation helps track recovery and informs return-to-activity decisions.

Concussion Recovery: What Happens After Diagnosis

Recovery from a concussion follows a general pattern, though the timeline varies significantly between individuals. Most people with concussions recover fully, but recovery doesn't mean returning to normal activities immediately. The brain needs time to heal at the cellular level, and returning to normal too quickly can delay recovery or potentially cause further problems.

The most commonly recommended approach during early recovery is physical and cognitive rest. Physical rest means limiting strenuous exercise and activities that elevate heart rate significantly. This doesn't mean complete bed rest—gentle activities like short walks are often appropriate. Cognitive rest means reducing mental effort, which can include limiting screen time, reducing schoolwork or work demands, avoiding complex problem-solving, and minimizing distractions. Both types of rest give the brain space to recover without additional stress.

Many recovery plans follow a gradual return-to-activity approach. A person might begin with very limited activities and very slowly increase activity levels as symptoms improve. This graduated approach allows medical providers to monitor whether activities cause symptoms to return or worsen. For students, this might mean attending school part-time initially, then increasing hours as tolerated. For athletes, it typically involves a multi-stage return-to-play protocol supervised by medical professionals, beginning with light aerobic exercise and progressing to sport-specific skills and finally to full competition.

Recovery isn't always linear. Some people improve steadily over days or weeks. Others experience good days and bad days. A day of more activity might trigger symptoms to return temporarily, which is why the gradual progression is important. Understanding that setbacks can happen helps people avoid discouragement and helps them recognize that temporary symptom return during recovery doesn't necessarily mean something is wrong.

For most people, symptoms resolve within a few weeks. However, some experience symptoms that persist longer—a condition sometimes called post-concussion syndrome. Research from the Journal of Athletic Training suggests that about 10-15% of people have symptoms lasting beyond the typical recovery window. These individuals may benefit from specialized rehabilitation focused on particular symptoms like balance problems or persistent headaches.

Practical Takeaway: Recovery typically involves physical and cognitive rest initially,
