Understanding Clavicle Fractures and Repair Options

The clavicle, commonly called the collarbone, is a long bone that runs from your breastbone to your shoulder blade. It plays a crucial role in connecting your arm to your torso and helps you move your arm freely. Clavicle fractures are among the most common bone breaks, accounting for approximately 2-3% of all fractures in adults and up to 35% of shoulder injuries in children and young adults.

A clavicle fracture typically occurs from a direct blow to the shoulder, a fall onto an outstretched arm, or trauma from sports or accidents. The break can happen in different locations along the bone: at the lateral end (near the shoulder), the medial end (near the breastbone), or the middle section, which is the most common fracture site, representing about 80% of all clavicle breaks.

Clavicle fractures are classified by severity. A simple fracture has one break line with two bone fragments. A comminuted fracture involves the bone breaking into three or more pieces. Displaced fractures occur when the broken ends of bone shift out of alignment, while non-displaced fractures have pieces that remain relatively aligned. The type of fracture determines which repair methods may be considered.

Many people experience immediate pain at the injury site, swelling, bruising, and difficulty moving the arm. Some individuals report a grinding sensation or hear a cracking sound at the moment of injury. The shoulder may droop downward due to loss of support from the broken bone. Understanding the nature of your fracture and the available repair methods helps you work with medical professionals to determine the best path forward.

Takeaway: Clavicle fractures vary in location and severity, and recognizing your specific injury type is the first step toward understanding what information about repair options may be relevant to your situation.

Non-Surgical Treatment Approaches for Clavicle Fractures

Many clavicle fractures heal successfully without surgery. Approximately 90-98% of clavicle fractures can be managed through non-surgical treatment, depending on the fracture type and individual factors. Non-surgical management typically focuses on pain control, immobilization, and gradual rehabilitation to restore strength and movement.

The initial phase of non-surgical treatment involves immobilization using a sling or figure-eight bandage. A sling supports the arm and prevents movement that could disrupt healing. Most people wear immobilization devices for 2-4 weeks, though duration varies based on fracture severity and healing progress. During this period, ice application during the first 48-72 hours helps reduce swelling, and over-the-counter pain relievers may manage discomfort. Rest is essential during early healing, meaning avoiding activities that stress the shoulder.

As healing progresses, rehabilitation becomes increasingly important. Physical therapy typically begins with gentle, passive range-of-motion exercises where a therapist or the individual moves the arm without muscular effort. After 3-4 weeks, active-assisted exercises transition to active exercises where the individual controls movement. By 6-8 weeks, strengthening exercises begin, focusing on muscles around the shoulder and upper back. The timeline varies based on individual healing rates and fracture characteristics.

Healing typically occurs over 8-12 weeks for most fractures, though complete functional recovery may take several months. During this time, regular follow-up appointments help monitor healing progress through physical examination and sometimes imaging studies. Healthcare providers can assess when it is safe to gradually return to normal activities and sports.

Takeaway: Non-surgical treatment involves a structured approach combining immobilization, pain management, and progressive rehabilitation exercises to support the body's natural healing process over several weeks to months.

Surgical Repair Methods for Clavicle Fractures

While most clavicle fractures heal without surgery, certain situations may warrant surgical intervention. Surgical repair becomes a consideration for completely displaced fractures, particularly in the middle portion of the bone, as these have higher rates of non-union (failure to heal) and malunion (healing in an improper position). Studies indicate that displaced midshaft clavicle fractures have non-union rates of 15-30% with non-surgical treatment, compared to less than 5% with surgical repair.

The most common surgical technique uses open reduction and internal fixation (ORIF) with a plate and screws. A surgeon makes an incision over the fracture site, realigns the bone fragments, and secures them with a metal plate affixed with screws. This approach directly visualizes the fracture and allows precise alignment. The plate provides stability that supports early movement and rehabilitation. Some surgeons prefer intramedullary nailing, where a metal rod is inserted through the center of the bone to hold fragments together.

Another surgical option involves percutaneous pinning, where the surgeon inserts metal pins through the skin and bone to hold fragments in place. This method creates smaller incisions and may result in less tissue damage compared to plate fixation. However, pins are typically temporary and require removal in a second procedure after healing occurs.

Surgical intervention may also be recommended for fractures involving the lateral third of the clavicle near the shoulder joint, fractures with associated injuries to nerves or blood vessels, open fractures where the bone pierces the skin, or fractures in patients with high healing demands, such as athletes. Post-surgical rehabilitation follows a structured timeline, beginning with immobilization for 3-6 weeks, followed by progressive physical therapy. Most people return to normal activities within 3-4 months after surgical repair, though return to high-impact sports may take longer.

Takeaway: Surgical repair options provide direct stabilization of bone fragments and may be considered for fractures at high risk of healing complications or in individuals with specific functional demands.

Recovery Timeline and Rehabilitation Expectations

Understanding what to expect during recovery helps individuals plan appropriately and maintain realistic expectations. Recovery from a clavicle fracture occurs in overlapping phases, each with distinct characteristics and milestones. The specific timeline varies based on fracture severity, age, overall health, and treatment approach.

The acute phase spans the first 2-4 weeks after injury. During this time, the body's natural inflammatory response promotes healing. Pain and swelling are typically most intense during this phase. Immobilization protects the healing bone by limiting movement that could displace fragments. Most people experience significant pain reduction by the end of this phase, though some discomfort persists with certain movements. Activities are restricted to light, pain-free movements only.

The intermediate phase occurs from weeks 4-8 (or 6-8 weeks after surgical repair). During this period, bone healing progresses and the fracture site stabilizes. Physical therapy becomes more active, introducing controlled range-of-motion exercises and beginning gentle strengthening work. Many people return to light daily activities like desk work or reading during this phase. Pain continues to decrease, though fatigue from healing and rehabilitation may occur.

The late phase extends from weeks 8-12 and beyond, depending on individual healing rates. Strengthening exercises intensify, focusing on restoring shoulder strength and endurance. Progressive weight-bearing activities resume. By 12 weeks, most people achieve significant functional improvement and can return to many regular activities. However, complete healing, including bone remodeling and full strength restoration, may continue for 6-12 months. Return to sports or high-demand activities typically occurs in the 3-6 month range, with individual variation based on activity type and healing progress.

Takeaway: Clavicle fracture recovery follows a predictable progression from acute immobilization through intermediate rehabilitation to advanced strengthening, with most people achieving functional recovery within 3-4 months and complete healing over a longer period.

Potential Complications and When to Seek Medical Attention

While most clavicle fractures heal successfully, certain complications can occur. Understanding these potential issues helps individuals recognize when medical attention is needed. The most common complication is non-union, where the fracture fails to heal completely. Non-union occurs in approximately 5-10% of non-surgically treated fractures and less than 5% of surgically repaired fractures. Signs of non-union include persistent pain months after injury, continued visible deformity, and lack of progress on imaging studies.
