What Is an Incentive Spirometer and How Does It Work

An incentive spirometer is a handheld medical device that measures how much air your lungs can hold and how well they function. It looks like a plastic tube with a chamber containing a ball or piston inside. When you breathe in through the mouthpiece, the air you inhale pushes the ball or piston upward. The device displays how much air you've drawn in, typically measured in milliliters (mL). A normal adult can usually inhale between 3,000 to 5,000 mL of air in a single breath, though this varies based on age, sex, body size, and overall health.

The spirometer works by creating resistance as you inhale. This resistance encourages your lungs to work harder and expand more fully. Think of it like exercise equipment for your lungs. Each time you use it, you're training your respiratory muscles to function more efficiently. The device includes visual markers or a scale that shows your current performance and often has a target goal line. Your goal is to reach or exceed that target during each use.

Healthcare providers prescribe incentive spirometers most often to patients recovering from surgery, those with respiratory conditions, or people who are bedridden. Studies show that regular use of an incentive spirometer can help prevent complications like pneumonia and atelectasis (collapsed lung tissue) in post-operative patients. Research published in the Journal of the American College of Surgeons found that patients who used incentive spirometers after surgery had lower rates of respiratory complications compared to those who did not.

The device comes in different models, but they all operate on the same basic principle: encouraging deep breathing by providing visual feedback. Some spirometers have electronic displays, while others use simple mechanical indicators. The type your healthcare provider recommends depends on your specific condition and what your lungs need to accomplish during recovery or treatment.

Practical Takeaway: An incentive spirometer is a tool that measures your lung capacity and encourages deeper breathing through visual feedback. Understanding how the device measures air volume and provides targets helps you use it more effectively during your recovery or treatment.

Why Your Healthcare Provider Might Recommend an Incentive Spirometer

Healthcare providers recommend incentive spirometers for several medical situations. After surgery, especially chest or abdominal surgery, patients often breathe shallowly because deep breathing causes discomfort. Shallow breathing can allow fluid to accumulate in the lungs or cause portions of the lungs to collapse. An incentive spirometer encourages patients to take deeper breaths, which keeps the lungs expanded and prevents these complications. Studies indicate that post-surgical patients who use incentive spirometers have infection rates that are about 50 percent lower than those who do not use them.

People with chronic lung conditions may use spirometers to monitor changes in their lung function over time. Conditions like chronic obstructive pulmonary disease (COPD), asthma, and cystic fibrosis all affect how much air the lungs can hold and move. Regular spirometry measurements provide information about whether the condition is stable, improving, or worsening. This information helps healthcare providers adjust treatment plans when necessary.

Patients who are immobilized or bedridden for extended periods benefit from incentive spirometer use. When people cannot move around, their lung function naturally declines. Using an incentive spirometer helps maintain respiratory strength and prevent complications from inactivity. This is particularly important for elderly patients or those recovering from serious illnesses.

People with neuromuscular diseases that affect breathing, such as muscular dystrophy or Parkinson's disease, may use spirometers to track how their respiratory muscles are functioning. Early detection of declining lung capacity allows for interventions before breathing becomes severely compromised. Additionally, some patients use incentive spirometers as part of pulmonary rehabilitation programs designed to improve overall lung health and exercise tolerance.

Incentive spirometry is also recommended for smokers preparing for surgery or those trying to understand their baseline lung function. The measurements provide objective data about current respiratory capacity, which can be motivating for people making lifestyle changes to improve their health.

Practical Takeaway: Incentive spirometers are recommended after surgery, for managing chronic lung conditions, during periods of immobility, and for tracking respiratory health changes. Knowing why your provider recommends one helps you understand its role in your specific health situation.

How to Use an Incentive Spirometer Correctly

Using an incentive spirometer properly is essential for getting accurate measurements and receiving the full benefit of the device. First, read the specific instructions that come with your spirometer, as different models may have slightly different procedures. Before you begin, sit upright in a chair or bed with good posture. Slouching or lying flat can restrict your lung expansion and give inaccurate results. Make sure you are not eating, drinking, or chewing gum immediately before using the device.

Hold the spirometer upright in front of you so you can see the measurement scale clearly. Place the mouthpiece in your mouth and close your lips firmly around it to create a seal. Breathe out normally to clear your lungs of air. Then, inhale slowly and deeply through your mouth as if you are drawing air through a straw. The key is to breathe in steadily rather than quickly or forcefully. A quick, sharp inhalation does not give your lungs time to fully expand.

Continue inhaling until you reach your target volume, which will be marked on your device. Hold your breath at the top of your inhalation for about three to five seconds. This pause allows the air to reach all parts of your lungs. Then exhale slowly through your mouth. Remove the spirometer and rest for a few breaths before repeating the process. Most healthcare providers recommend using the spirometer five to ten times per session, with sessions spread throughout the day—typically three to four times daily.

Keep a log of your measurements and note them down each time you use the device. This record helps you and your healthcare provider track progress and identify any decline in lung function. If you feel dizzy, lightheaded, or short of breath during use, stop immediately and rest. These symptoms, while uncommon, indicate that you should consult your healthcare provider about your technique.

Common mistakes include breathing in too quickly, not sealing your lips properly around the mouthpiece, holding the device at an angle rather than upright, or not breathing in as deeply as possible. Each of these errors can result in lower measurements that do not reflect your true lung capacity. If your measurements seem low compared to previous results, check your technique before assuming your lung function has declined.

Practical Takeaway: Proper technique involves sitting upright, creating a good seal around the mouthpiece, inhaling slowly and deeply, holding briefly, and tracking your results. Practicing correct form from the beginning ensures you get accurate information about your lung capacity.

Understanding Your Spirometer Measurements and Setting Goals

Your spirometer displays a measurement called forced vital capacity (FVC), which represents the total amount of air you can exhale after taking the deepest breath possible. This number is measured in milliliters. A typical adult might have an FVC of 3,500 to 5,000 mL, depending on factors like age, height, sex, and physical fitness. Your healthcare provider will establish a target goal for you based on your specific situation and medical history. If you are recovering from surgery, your goal might be to reach 75 to 80 percent of your predicted normal capacity within a certain timeframe.

Predicted normal values are calculated using standardized formulas that account for your age, height, sex, and ethnicity. For example, a healthy 40-year-old man who is 5 feet 10 inches tall might have a predicted FVC of around 4,800 mL. If your actual measurement is 3,600 mL, that represents 75 percent of your predicted value. Healthcare providers use these percentages to determine whether your lung function is normal, mildly reduced, moderately reduced, or severely reduced.

When you start using an incentive spirometer, your initial measurement establishes your baseline. This is your starting point. Your goal is typically to improve this baseline over days or weeks, depending on your situation. Post-surgical patients usually show the most dramatic improvements in the first two to three weeks as they recover and are able to breathe more deeply. Someone with a chronic lung condition might focus on maintaining their baseline rather than improving it, with the goal being to prevent further decline.
