What Is Nursemaid Elbow and How Does It Happen

Nursemaid elbow, medically known as radial head subluxation, is a common injury in young children. It occurs when the radius bone (one of the bones in the forearm) partially slips out of place at the elbow joint. This injury typically affects children between ages 1 and 4, though it can happen in older children as well.

The injury happens because the ligaments holding the radius bone in place in young children are looser and more flexible than in adults. When a child is pulled or swung by the arms—such as being lifted quickly by one arm, swung in play, or pulled up after a fall—the radius bone can slip partially out of its normal position. The injury often occurs so suddenly that neither the child nor the adult may feel or hear anything unusual happen.

Common situations that lead to nursemaid elbow include:

• An adult lifting a child quickly by one arm
• A child being swung or playing roughly with an adult who pulls on their arm
• Rolling over in bed in a particular way
• Falling on an outstretched arm
• An older sibling pulling a younger child
• A child being jerked away from danger

Parents and caregivers often don't realize the injury has happened because the child may not cry at the exact moment it occurs. Instead, the child typically stops using the affected arm within minutes to hours after the incident.

Practical Takeaway: Understanding how nursemaid elbow occurs helps caregivers recognize the injury and know when to seek medical attention. Being aware of arm-pulling situations can help prevent this injury from happening in the first place.

Recognizing the Signs and Symptoms of Nursemaid Elbow

The symptoms of nursemaid elbow are fairly distinct, which helps parents and caregivers identify when this injury may have occurred. The most obvious sign is that a child stops using the affected arm. The child may hold the arm still against their body in a bent position and refuse to move the elbow or rotate the forearm (turning the arm from palm-up to palm-down).

A child with nursemaid elbow typically shows these signs:

• Refuses to use the injured arm
• Holds the arm bent at the elbow and rotated inward
• Cries or complains of pain when the arm is moved
• May have minimal swelling or bruising (though not always)
• Appears anxious about moving the arm
• May not remember exactly when or how the injury happened

An important thing to know is that a child with nursemaid elbow typically does not have severe swelling or significant bruising on the outside of the elbow. The injury is inside the joint, not a break in the bone. This is one reason why the injury can be easy to miss if parents aren't familiar with it.

Some children may complain of general arm pain or pain in the wrist or hand rather than pointing specifically to the elbow. This happens because young children sometimes have difficulty pinpointing exactly where pain originates. Young children may also be too upset or scared to communicate clearly about their symptoms.

Symptoms typically appear within a few minutes to a few hours after the injury occurs. If a child had an arm-pulling incident earlier in the day and is now refusing to use that arm, nursemaid elbow may be the cause.

Practical Takeaway: The key sign to watch for is a child who suddenly stops using one arm and holds it still in a bent position. If this follows an arm-pulling incident, seeking medical evaluation is important to confirm the diagnosis.

Medical Diagnosis and When to Seek Care

When you suspect a child may have nursemaid elbow, a visit to a doctor, urgent care center, or emergency room is typically needed. A medical professional can examine the arm and confirm whether the radius bone is out of place. In most cases, doctors can diagnose nursemaid elbow based on the child's history and a physical examination without needing X-rays.

During the examination, a doctor will ask questions about what happened before the child stopped using the arm. They will gently examine the elbow, forearm, and wrist. They may try to gently move the arm in different directions to assess where the child feels pain and how the joint moves. A child with nursemaid elbow will typically have difficulty or pain when the forearm is rotated (turned) with the elbow bent.

Medical professionals may order X-rays if they want to rule out other injuries like a broken bone, particularly if:

• There is significant swelling or bruising
• The child is older and nursemaid elbow is less common
• The story of how the injury happened doesn't fit with typical nursemaid elbow patterns
• The child doesn't improve after treatment
• There are other signs of injury

You should seek medical care right away if a child has stopped using an arm after a pulling incident, if there is significant swelling or deformity, if the arm is very painful, or if there are signs of other injuries. Nursemaid elbow is not a medical emergency in the same way that a severe break might be, but it should be evaluated and treated by a medical professional to reduce pain and get the arm working again.

Seeking care within a few hours of the injury is preferable. The longer the radius bone stays out of place, the more muscle tension can build up around the joint, which may make treatment slightly more challenging, though still very successful.

Practical Takeaway: A doctor can usually diagnose nursemaid elbow through examination and questions about what happened. Contact a healthcare provider if a child suddenly stops using an arm after a pulling incident, rather than waiting to see if it improves on its own.

Treatment Options for Nursemaid Elbow

The good news about nursemaid elbow is that it responds very well to treatment. The primary goal of treatment is to move the radius bone back into its correct position. This procedure is called a reduction. Once the bone is back in place, the pain typically goes away quickly, and the child can usually start using the arm normally again within hours to days.

The most common treatment method involves a doctor gently manipulating the arm. The doctor will carefully move the child's forearm from a palm-down position to a palm-up position (called supination) while the elbow is bent. This gentle rotation often causes the radius bone to slip back into place. Many children feel immediate relief once the bone is repositioned. Some children may cry during the procedure because they are scared or because of anticipatory anxiety, but the actual manipulation is typically not painful once the bone is back in place.

In some cases, a doctor may use a different technique depending on the specific situation or the child's comfort level. The doctor might:

• Perform the reduction while the child is sitting in a parent's lap
• Have the parent hold and comfort the child during the procedure
• Use gentle, gradual movements rather than a quick motion
• In rare cases, give medication to help the child relax before the procedure

After the reduction, the arm is typically immobilized in a sling or splint for a period of time—usually 1 to 3 weeks, depending on the child's age and the doctor's preference. During this time, the child should avoid activities that could cause the injury to happen again, such as swinging or pulling on the arm.

In very rare cases, a child may need multiple attempts at reduction, or the radius bone may not stay in place on the first try. This is uncommon, occurring in only a small percentage of cases. If this happens, further imaging may be needed, and the doctor may try different approaches.

The success rate for treatment is very high. Studies show that more than 95% of cases resolve successfully with simple reduction and immobilization. Complications are rare when the injury is treated promptly.
