What Is Milia and How Does It Form

Milia are small, white or yellowish bumps that appear on the skin, usually on the face, eyelids, cheeks, nose, or forehead. These bumps are typically 1 to 2 millimeters in diameter, though some can be slightly larger. The condition is extremely common and affects people of all ages, from newborns to adults. Many people notice milia appear without any obvious cause, which can be concerning, but understanding how these bumps form helps explain why they occur.

Milia develop when dead skin cells become trapped beneath the skin's surface. Normally, skin cells naturally shed and fall away as part of the body's regular renewal process. However, sometimes these dead cells don't shed properly and instead get trapped in small pockets under the outer layer of skin called the epidermis. When keratin—a natural protein found in skin, hair, and nails—accumulates in these pockets, it hardens and creates the small white bump you see on the surface.

There are two main types of milia. Primary milia occurs when keratin becomes trapped without any prior skin injury. This type often appears on babies and can also develop in adults. Secondary milia forms after some type of skin trauma or damage, such as a burn, rash, prolonged sun exposure, or aggressive skin treatments like chemical peels or laser therapy. Both types are harmless and pose no risk to your health, though many people choose to address them for cosmetic reasons.

Risk factors that increase the likelihood of developing milia include having oily or combination skin, using heavy creams or oils, sun damage, and certain genetic factors. Some medications can also contribute to their formation. Understanding these underlying causes can help you prevent new milia from forming while addressing existing ones.

Practical Takeaway: Milia form when dead skin cells and keratin become trapped beneath the skin's surface. Recognizing this process helps you understand that milia are a normal skin occurrence, not a sign of poor hygiene or serious skin disease.

Who Develops Milia and Why It Occurs More in Some People

While milia can affect anyone regardless of age, gender, or skin type, certain groups of people develop them more frequently than others. Understanding who is most likely to experience milia can help you take preventive steps if you fall into a higher-risk category.

Infants and young children commonly develop milia, with research showing that approximately 40% of newborns experience these bumps at some point. In babies, milia typically appear on the cheeks, nose, and chin and often resolve on their own within a few weeks without any treatment. Pediatricians generally recommend leaving infantile milia alone, as they usually disappear naturally as the baby's skin matures and the natural shedding process becomes more efficient.

Adults with certain skin characteristics are more prone to milia. People with oily or combination skin types are particularly susceptible because excess sebum can interfere with normal skin cell shedding. Additionally, individuals who have experienced significant sun damage accumulate more dead skin cells on their skin's surface, making them more likely to develop trapped keratin pockets. People living in sunny climates or those who spend considerable time outdoors without sun protection face higher milia risk.

Genetic factors also play a role in milia development. If your parents or other close relatives frequently developed milia, you may be more genetically predisposed to the condition. Some people naturally have skin that sheds more slowly or less efficiently, creating an environment where dead cells accumulate more readily.

Certain skincare and lifestyle habits increase milia formation. Using heavy, occlusive moisturizers—particularly those containing high amounts of oils or waxes—can trap dead skin cells beneath the surface. Similarly, heavy makeup, especially thick foundations that aren't removed thoroughly each day, can contribute to clogged pores and milia development. People who use certain medications, such as topical corticosteroids applied to the face for extended periods, sometimes notice increased milia formation.

Those who have recently undergone skin treatments like chemical peels, laser resurfacing, or aggressive microdermabrasion are at higher risk for secondary milia. This occurs because these treatments create temporary inflammation and damage to the skin barrier, which can lead to disrupted skin shedding patterns.

Practical Takeaway: Milia risk increases with oily skin, sun damage, certain genetics, heavy skincare products, and recent skin treatments. Identifying which factors apply to you helps you make targeted changes to your skincare routine and habits.

Distinguishing Milia From Other Skin Conditions

Milia can sometimes be confused with other common skin conditions, and proper identification is important because different conditions require different approaches. Learning the characteristics that distinguish milia from similar-looking bumps helps you determine whether the bumps on your skin are actually milia or something else that may need different treatment.

The most common condition confused with milia is closed comedones, commonly called whiteheads. While milia and whiteheads can look similar at first glance, they have important differences. Whiteheads form when pores become clogged with sebum and dead skin cells, and they typically occur in areas where you have oily skin or active breakouts. Whiteheads respond well to acne-fighting ingredients like salicylic acid or benzoyl peroxide. Milia, by contrast, contain only keratin and don't respond to acne treatments. Additionally, milia are typically deeper beneath the skin surface and cannot be easily extracted like a whitehead can be.

Another condition sometimes mistaken for milia is syringoma, which are small, harmless tumors of the sweat glands. Syringomas appear as clusters of small, flesh-colored or yellowish bumps, often on the lower eyelids and cheeks. Unlike milia, syringomas don't typically resolve on their own and usually require professional removal. A dermatologist can distinguish between these two conditions through visual examination.

Keratosis pilaris (KP) is another condition occasionally confused with milia, though they look quite different upon closer inspection. Keratosis pilaris appears as small, rough, red or skin-colored bumps, usually on the upper arms, thighs, or cheeks. These bumps feel like small bumps of rough texture and are associated with keratin buildup around hair follicles. Unlike milia, keratosis pilaris creates a sandpaper-like texture and typically spans a larger area of skin.

Flat warts caused by the human papillomavirus (HPV) can sometimes resemble milia, though warts are usually slightly raised and may have a different texture. Warts caused by HPV are contagious and often grow or spread over time, whereas milia are not contagious and remain stable in size and number unless new ones form.

The key distinguishing features of milia include their small size (usually 1-2 millimeters), white or cream color, smooth surface, and location typically on the face. They don't respond to acne treatments, they're not contagious, and they don't contain the inflammatory components that characterize acne or other dermatological conditions.

Practical Takeaway: Milia are distinguished by their small size, white color, keratin-only content, and lack of response to acne treatments. If you're uncertain whether bumps are milia or another condition, consulting a dermatologist provides definitive identification.

Professional Treatment Options for Milia Removal

While milia often resolve on their own over time, many people choose professional treatment for faster removal, particularly if the bumps appear on visible areas like the face. Several evidence-based professional treatments can effectively remove milia, and understanding these options helps you make informed decisions about which approach may work best for your situation.

One of the most common professional treatments is manual extraction, sometimes called micro-extraction or comedone extraction. During this procedure, a dermatologist uses specialized sterile instruments to carefully open the skin and remove the trapped keratin. This method is quick, relatively painless, and shows immediate results. The main advantage is that it provides instant removal without downtime. However, manual extraction requires a skilled professional to avoid scarring or skin damage, and new milia may eventually form in the same location if the underlying cause isn't addressed.

Chemical peels represent another effective professional option, particularly for treating multiple milia or preventing new ones from forming. During a chemical peel, a dermatologist
