What Liver Function Tests Measure and Why They Matter

Liver function tests (LFTs) are blood tests that measure how well your liver is working. Your liver is one of the largest organs in your body and handles many important jobs. It breaks down nutrients from food, removes waste products, makes proteins needed for blood clotting, and stores energy. When your liver isn't functioning properly, these tests can detect problems early, sometimes before you notice any symptoms.

There are several different measurements included in a standard liver function test panel. Each one tells doctors something different about your liver's health. Some tests measure enzymes that leak from liver cells when they're damaged. Others measure substances your liver produces or processes. Together, these measurements paint a picture of liver health that would be impossible to see from just one test alone.

Doctors order liver function tests for many reasons. You might have them as part of a routine checkup, if you have symptoms like fatigue or jaundice, if you take medications known to affect the liver, or if you have a condition like hepatitis or cirrhosis. You might also have them if you drink alcohol regularly, since alcohol can damage liver cells over time. Some people get LFTs before starting certain medications to establish a baseline of normal liver function.

The results from liver function tests are measured in standard units that appear on your lab report. Numbers can be reported in different ways depending on which lab performs the test. This is why it's important to look at the reference range listed on your specific results—what's normal at one lab might be slightly different at another lab, though the differences are usually small.

Understanding what each measurement means helps you have better conversations with your healthcare provider about your health. While you shouldn't try to diagnose yourself based on these numbers, knowing what they represent allows you to ask informed questions and understand the recommendations your doctor makes.

Practical Takeaway: Liver function tests measure different aspects of how your liver works. Learning what each test measures helps you understand what your doctor is checking and why these tests matter for detecting liver problems.

Understanding ALT and AST Levels

ALT (alanine aminotransferase) and AST (aspartate aminotransferase) are enzymes found primarily in liver cells. When liver cells are damaged or die, these enzymes leak into the bloodstream where they can be measured by blood tests. Think of these enzymes like alarm bells—when they're elevated, it signals that something is damaging liver cells.

Normal ALT levels typically range from 7 to 56 units per liter of blood, though this varies slightly between labs. Normal AST usually ranges from 10 to 40 units per liter. When these numbers are higher than normal, it indicates liver cell damage. However, having elevated ALT or AST doesn't automatically mean you have serious liver disease. These enzymes can be elevated for many reasons, some temporary and some more serious.

ALT is considered more specific to the liver than AST. While AST is found in liver cells, it's also present in heart muscles, skeletal muscles, and kidneys. This means an elevated AST could indicate damage to any of these organs, not necessarily the liver. ALT, on the other hand, is found mainly in the liver, so an elevated ALT is usually a more reliable sign of liver-specific problems.

Common causes of elevated ALT and AST include viral hepatitis, fatty liver disease, alcoholic liver disease, cirrhosis, liver cancer, and certain medications. Even intense exercise can temporarily raise these levels because muscle damage releases similar enzymes. Acetaminophen overdose is a well-known cause of very high ALT and AST levels, sometimes in the thousands.

Your doctor looks at the degree of elevation to help determine what might be causing it. Mild elevations (two to three times the normal range) might indicate fatty liver disease or mild hepatitis. Very high elevations (ten times normal or more) often suggest acute viral hepatitis or drug-induced liver injury. Your doctor will also look at these numbers in context with your other liver tests and your medical history.

Sometimes doctors calculate the AST-to-ALT ratio to help narrow down what type of liver problem might be present. In viral hepatitis, ALT is typically higher than AST. In alcoholic liver disease, AST is often higher than ALT. This ratio, combined with other tests, helps doctors piece together the puzzle of what's affecting your liver.

Practical Takeaway: ALT and AST are enzymes that leak from damaged liver cells into the blood. Higher levels indicate liver cell damage, but the degree and pattern of elevation help your doctor determine what might be causing the damage.

Interpreting Bilirubin and Alkaline Phosphatase Results

Bilirubin is a yellowish substance created when your body breaks down old red blood cells. Your liver processes bilirubin and sends it through bile ducts into your digestive system. If your liver isn't working properly or if bile ducts are blocked, bilirubin builds up in your blood, which can turn your skin and eyes yellow—a condition called jaundice.

There are two types of bilirubin measured in liver tests: unconjugated (indirect) bilirubin and conjugated (direct) bilirubin. Total bilirubin is the combination of both types. Normal total bilirubin is usually less than 1.2 milligrams per deciliter of blood. When bilirubin is elevated, your doctor looks at which type is high to figure out where the problem is occurring.

If unconjugated bilirubin is high, it suggests your liver isn't processing bilirubin effectively. This might happen with hepatitis, cirrhosis, or conditions that damage liver cells. If conjugated bilirubin is high, it suggests your liver processed the bilirubin but can't get it out through the bile ducts. This might indicate a blocked bile duct or a problem within liver cells that prevents bilirubin from being excreted.

Alkaline phosphatase (ALP) is another enzyme found in liver cells, bone, and other tissues. Normal alkaline phosphatase typically ranges from 44 to 147 units per liter, though normal ranges vary with age and sex. Alkaline phosphatase is particularly useful for detecting bile duct problems. When bile ducts are blocked or inflamed, alkaline phosphatase levels often rise significantly.

If your alkaline phosphatase is elevated but your ALT and AST are normal, your doctor may suspect a bile duct problem rather than liver cell damage. If ALT, AST, and alkaline phosphatase are all elevated together, it suggests more widespread liver damage. High alkaline phosphatase can also result from bone disease, pregnancy, or certain infections, so your doctor considers your complete clinical picture.

One useful test to distinguish liver problems from bone problems is gamma-glutamyl transferase (GGT). GGT is found mainly in the liver. If alkaline phosphatase is high but GGT is normal, the high alkaline phosphatase likely comes from bone rather than the liver. If both are high, a liver problem is more likely.

Practical Takeaway: Bilirubin shows whether your liver can process and remove waste products. Alkaline phosphatase helps detect bile duct problems. Together with other tests, these measurements show where liver function is breaking down.

Albumin and Total Protein: Measuring Liver Production

While ALT, AST, and bilirubin measure damage or obstruction in the liver, albumin and total protein measure what your liver actually produces. Albumin is a protein made by your liver that carries nutrients, hormones, and medications through your bloodstream. It also helps maintain proper fluid balance in your body. Normal albumin levels are typically between 3.4 and 5.4 grams per deciliter of blood.

Albumin is particularly important because it shows your liver's synthetic function—its ability to make new proteins. When your liver is damaged, one of the first things that suffers is its ability to produce enough albumin. Low albumin can develop gradually over months or years in chronic liver disease, or it can drop quickly in acute liver failure. This is why albumin is considered one of the best indicators of how severely your liver is damaged.

Total protein includes albumin plus other proteins like globulins. Normal total protein ranges from 6.0 to 8.
